PODIATRISTS IN THE NEWS
DC Podiatrist Discusses Athlete's Foot

No one wants to talk about Athlete's Foot because it's kind of gross. But just because you don't discuss it doesn't mean it won't happen to you. In fact, seven in 10 people get Athlete's Foot at some point, although many times it goes misdiagnosed as dry skin. "Athlete's Foot is basically a fungal infection of the top layer of your skin or epidermis," Joel Morse, DPM and president of the American Society of Podiatric Dermatology in Washington, DC, says.

Dr.  Joel Morse

Men are more likely to get it than women, and guys who workout are the most vulnerable of all, since the fungus thrives in warm, wet places, like the floors of gym locker rooms. Those soft cotton socks you love are doing nothing for your itchy feet. "Synthetics are the name of the game when it comes to preventing Athlete's Foot," Morse says. "Studies show they keep foot temperature lower and reduce moisture, the two things fungus needs to grow."

PODIATRISTS AND SPORTS
PA Podiatrist Psyched for Broad Street Run

After battling a hip injury for a year and a half, Dr. Brian Crispell is psyched to make his return to racing in Philadelphia. “Every competitive runner in southeastern Pennsylvania keys this race into their schedule,” said Crispell, who recently moved from Philadelphia to Upper Providence with his wife, Dana Kolesar. “The entire atmosphere is just electric. There are helicopters flying overhead, camera people all over the place, the mayor is there, the crowds are huge ... It’s very exciting.”

Dr. Brian Crispell

Before injuring himself while training for the Boston Marathon, Crispell, 39, had placed second at the 2013 Baltimore Marathon and posted four consecutive top 100 finishes in the Broad Street Run. “Years and years ago, I could run in the 55 (minute range) but I’m slowing down a little bit,” said Crispell, a podiatrist who frequently treats running-related injuries at his Riddle Hospital practice. “This year, I’m planning to be around 57 minutes, which might put me in the top five in my age group.” Crispell expects to run conservatively for the first five miles. “If I’m feeling good at City Hall, I’ll increase the pace a little bit,” he said.

PODIATRISTS AND FOOTWEAR
Flip-Flops Expose Feet to Infections: NY Podiatrist

As the weather warms up, it's hard to resist the urge to break out the truest sign of summer: flip-flops. But most experts are horrified by the idea. Here's why you should reserve your flip-flops for the beach, pool, spa, and shared showers — and keep your feet out of them, otherwise, according to Dr. Jackie Sutera, a podiatrist and spokesperson for the American Podiatric Medical Association.

Dr. Jacqueline Sutera

They expose your feet to bacteria, viral and fungal infections. Any time your feet get particularly filthy (i.e., any time you wear your flip-flops in public), they're likely covered in some nasty things likes Staphylococcus, a bacteria that can irritate the skin on your foot, in the best case, or lead to amputation in the worst-case scenarios. It depends on whether you have open wounds, like microwounds from exfoliation during a recent pedicure, or actual cuts, and your state of health when you pick up the bacteria.

PRACTICE MANAGEMENT TIP OF OF THE DAY
4 Ways to Handle Cyber Conflict and Negative Reviews - Part 2

Electronic remarks can be hurtful, to both your pride and your practice. Here are four ways to handle negative electronic interactions and potentially detrimental online reviews.

2. Respond with respect.

If you choose to reply to someone's objectionable remarks, do so with patience and a courteous attitude. State your point of view factually, disagree with dignity, and avoid getting into a squabble. Internet trolls and online rabble-rousers pride themselves on their ability to prod people to the point where they respond with anger and emotion. Once you've hit the post or comment button, your words will live forever in cyber-land. If you can't stick to the topic without belittling the writer of the offensive remarks, step away from the computer and cool down until you're able to be more objective.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Rando Mundo, DPM

Aetrex makes several models of footwear with Velcro heel closures. For example: the X532 model is an extra-depth shoe runner/walker with a heel closure. Also Aetrex has the Edge Runner, model number Q560 for men and model number Q580 for women. The closure at the rear of the Edge Runner is called a "lockdown heel."

Orthofeet now is offering Tie-Less sneakers with an adjustable heel counter. A heel-strap at the posterior aspect of the shoe enables to tighten the back of the shoe and fit it to the user’s foot. The flexible heel counter accommodates braces too.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Robert Scott Steinberg, DPM

Dr. Zuckerman is correct. Podiatrists don't know the difference between what is scientifically ESWT, and what is not! And, as he so correctly pointed out, it is high energy ESWT that has been proven to work. I stand by my experience. 90% of my patients were cured at 6 months. (The scientific literature stated: success is measured at 6 months, as it can take that much time for the PF to heal.) Low energy ESWT, if you can even call it ESWT, needed multiple treatments, which, at best, just provided temporary pain relief.

I first used the high energy Ossatron. Its protocol was to aim at the plantar fascia from the plantar aspect. It caused a lot of pain, since the sound wave bounced off the plantar surface of the calcaneus. Dornier's EPOS Ultra protocol is to aim at the plantar fascia from medial to lateral. Aiming is very accurate, since the Dornier had built-in ultrasound imaging. At the beginning of the procedure - low energy, low pulse rate, you could visualize the sound wave ripple. The computer program increased the energy and pulse rate to a therapeutic level. This week's poll would have been more accurate if it differentiated between different types of units available.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1A
From: Mike Rosenblatt, DPM

Most DPMs have to find a way to start their own professional practice, find a job, or associate with an existing practice. We know that's certainly difficult enough. But imagine how hard it is to start your own publication from scratch. Advertisers must be secured and there are hours spent in front of the computer screen making sure the latest edition goes out...including a printed magazine. Would YOU be up to that challenge? We only have a rough idea how difficult that would be. Somehow, Barry Block DPM, JD accomplished this. His readership is enormous, constituting over 17,000 subscribers.

This is nothing short of an amazing accomplishment. This project represents the very HEART of personal entrepreneurial effort. On first glance, it must have seemed impossible. But Barry did it, and we all look forward to reading his daily blog. I have been retired for years and I still read every new edition with interest and pleasure. Congrats Barry! You are a symbol of what hard work and personal entrepreneurial courage can accomplish. Best of all, you have given us a voice. "Who could ask for anything more?"

Mike Rosenblatt, DPM, San Jose, CA (Retired)
