Sandals and Flip-flops Can be Foot Peeling Culprits: NY Podiatrist
Sandal weather- Gotta love it—except when all that exposure brings to light (literally) that your feet aren’t in the best shape of their life. Maybe you notice rough patches, cracked skin, or peeling, for instance. "We are on our feet all the time and the constant pounding and walking can cause cracking and scaling of the skin," says Rebecca Pruthi, DPM, a podiatrist, and owner of Foot Care of Manhattan.

Many women, including Dr. Pruthi, love to wear strappy shoes in the summer. But as anyone who hasn't properly broken in their new heels knows, snug footwear can cause some serious friction. "Anything that causes friction can cause blisters, which can also result in scaling or peeling of the skin," Pruthi says. Flip-flops can also be a culprit of foot discomfort and peeling. "Naturally your body wants to slough off skin," Pruthi says, adding that she recommends patients get a pumice stone to rub on their feet in the shower to get the dead skin cells off.

Student-Doctor Maddy Werd, now entering her 4th year at Barry University School of Podiatric Medicine (BUSPM), just completed her first marathon - running the 26.2 miles with her father Matt Werd, DPM - at The Nashville Rock-n-Roll Marathon held in Nashville, Tennessee. Maddy is the APMSA student-liaison for the American Academy of Podiatric Sports Medicine (AAPSM) and is the American Podiatric Medical Student Association (APMSA) Class of 2026 Delegate for BUSPM.

VA Podiatrist Discusses Wound Skin Substitute Choices with Icelandic Scientist
Ben Pearl, DPM and Gunnar Johannsson, MD discussed the various types of skin substitutes available for wounds on his Podcast, Fit Foot U. Dr. Pearl mentioned that there are some companies that confuse physicians with information that is incorrect about the different types of skin substitutes. Gunnar said that while some human placenta grafts contain live cells, the structure in the graft matrix can be more beneficial for wound healing. The matrix three-dimensional structure found in fish skin can allow more fibroblast ingrowth, which can also help the elasticity of the healed wound.

Johannsson added that while some placenta grafts have live cells which can provide growth factors, the tissue structure is thinner and does not allow the same fibroblast proliferation. He dismissed the notion that somehow using human tissue was going to incorporate better than animal tissue. Finally, he noted that the source environment can provide advantages for certain grafts. As an example, fish skin from Icelandic cod, which comes from cold water, does not have a viral transmission risk, allowing it to be more gently processed compared to bovine or swine sources.

A life member of CPMA and APMA, Dr. Rice was a past president of CPMA and served as a national delegate representing our association to APMA. He enjoyed a remarkable 60-year career in podiatry in Norwalk, Connecticut, with the final 30 years spent practicing alongside his son, Dr. Andrew Rice. Our thoughts are with his family, colleagues, and all those who had the privilege to know him.

Query: MIPS Madness
I am a solo practitioner, and I am having an issue with the burdensome bureaucracy that is MIPS. I am really having a hard time understanding it! Is it me or are there so many hurdles to scale that it is not possible to do (is this on purpose)? Where is the APMA on this? Maybe one of our colleagues can enlighten me? What is it meant to accomplish? (I am asking sarcastically because I think I know the answer!). It is costly and time-consuming to try to keep up with it; it feels like a lose-lose scenario to the point of questioning why to even bother participating? I can't be the only one feeling this way-am I?

Query: Ankle Scope with OCD
I performed an ankle scope of the patient and found an osteochondral defect. I did an excise repair and drilled it. What code should I use to bill for it?

Response from thedoctorline.com: As you mentioned the ankle and not the subtalar joint and noted that some osseous defect was drilled out, then several CPT codes come to mind including: 29897 Arthroscopy, ankle (tibiotalar and fibulotalar joints), surgical; debridement, limited or 29898: Arthroscopy, ankle (tibiotalar and fibulotalar joints), surgical; debridement, extensive.
To answer your question specifically would require knowledge of the size of the defect(s) and were they contained within the same bone or multiple bones (e.g. tibial plafond and talar neck), was there a loose body in the joint, was the defect filled with bone, other graft material, or PRP (the latter perhaps not covered)? If it was a simple drill out and scope out, then 29897. Even if you drilled out and filled one site w/some boney implant material, then 29898 would be appropriate.

The article by Lium, et al. was a systematic review and dose-response meta-analysis. The article concluded that serum B12 concentration was "positively associated" with the risk of all-cause mortality, especially in older adults. There is a distinct difference between association and causal pathways, and therefore we must be careful in drawing conclusions from this one paper. B12 is necessary for many reasons as we are all aware, including RBC production and normal neurologic function. The Office of Dietary Supplements states that B12 has not been demonstrated to cause any harm, even at higher dosages. B12 is a water-soluble vitamin, and excess Vitamin B12 is eliminated in the urine.
There is no universal agreement which defines the "normal" serum levels of B12. Excess B12 may rarely cause symptoms, in less than 4% of patients affected, such as nausea, dizziness, or headache. Conversely, B12 deficiency is not uncommon in diabetic patients, particularly those taking metformin, as well as elderly patients. There is no established toxic level for B12, nor for other water-soluble vitamins such as thiamine or benfotiamine. Overdosage of thiamine is rare, but may cause paralysis, convulsion, respiratory, and/or cardiac failure for example.
I have long believed that many of our patients, particularly the elderly and diabetic patients, benefit from B12 supplementation, as well as benfotiamine, L-methyl folate supplementation. The widespread use of vitamin and supplement therapy is now included in discussions of the treatment of diabetic neuropathy as evidenced by recent clinical guidelines by the AAN, and recent publications in Diabetes Care. More importantly, those podiatrists who have employed these supplements and others (e.g.: alpha lipoic acid) have witnessed the positive effects on our patients, resulting in the widespread use of these supplements.
