Lester J. Jones, DPM, MSEd, retired April 30, 2019 from Western University of Health Sciences’ College of Podiatric Medicine (CPM) after serving more than 13 years as a consultant, professor, executive associate dean, and interim dean while helping to establish the foundational bedrock upon which the College was built.

Previously Dr. Jones had served the academic arms of the podiatry profession at every level -- as Vice Chair of the Council on Podiatric Medical Education (CPME), President of the National Board of Podiatric Medical Examiners, and President of the American Board of Podiatric, Orthopedics, and Primary Podiatric Medicine (ABPOPPM).

CA Podiatrist Mourns Loss of Synagogue Heroine Daughter
 
Bereaved relatives, grieving fellow congregants, and an array of political leaders gathered to mourn the death of Lori Gilbert Kaye, one of the synagogue’s “pioneers”. A former Wells Fargo Bank employee, she helped get the congregation a loan in 1995 to build its house of worship blocks from Rancho Bernardo.

Kaye’s father, Richard S. Gilbert, DPM didn’t speak but listened from the front row, using a wheelchair. A retired podiatrist in Encinitas, Dr. Gilbert once was known as “Feets” as the San Diego Chargers team doctor for many years.

Diabetic Amputation Epidemic is Being Totally Ignored: CA Podiatrist 
 
On his regular rounds at the University of Southern California's Keck Hospital, Dr. David Armstrong sees a brutal injustice of American healthcare. Each week, dozens of patients with diabetes come to him with deep wounds, severe infections, and poor circulation -- debilitating complications of a disease that has spiraled out of control.

He works to save their limbs, but sometimes Armstrong and his team must resort to amputation to save the patient, a painful and life-altering measure he knows is nearly always preventable. "Amputations are an unnecessary consequence of this devastating disease," said Armstrong, professor of surgery at Keck School of Medicine of USC. "It's an epidemic within an epidemic. And it's a problem that's totally ignored."

Is it Time to Reform Your Forms? - Part 3
 
Exchanging health information is a two-way street. On one side, physicians aim to gather pertinent information from patients. On the other side, patients are looking for clear direction from practitioners. If you haven’t revised the documents you share with patients for a while, perhaps now is the time to review and update them. Here are a few suggestions to help you get started. 
 
3. Procedure explanation forms - Patients need to know exactly what to do in advance of and following all medical procedures. The most efficient and reliable way to ensure this is to provide them with simple, detailed, step-by-step instructions. Use bullet points, outline the chronological stages of the procedure (before/during/after), and add a variety of frequently asked questions to help guarantee patient understanding and compliance. You can also provide links to online resources that accurately depict what will be done. Too many surgeries and procedures are postponed or cancelled simply because of unclear instructions.

Query: The Non-Asterisk Diagnosis
 
It appears imperative that the MD or DO name be placed on the claim form for the date of service when billing for routine foot care. My question is if the diagnosis is a “non-asterisk diagnosis” (therefore, not requiring active care by the MD or DO), then why does their name need to be on the claim form? Is it possible the doctor does not have to inform Medicare that any physician is treating the non-asterisk disease (peripheral vascular disease)?

Response: There are 2 types of foot care that we perform as podiatrists. There is 1) routine foot care with qualifying conditions, that I have always termed “at risk foot care” and 2) treatment of mycotic nails. There are rules that govern each of these.
 
If you are providing at risk foot care to a patient, then there is information that needs to be placed on the claim form such as the physicians name, date last seen, etc. That is the only way for a patient to qualify for routine or at risk foot care.
 
If you are providing treatment for mycotic nails, then there needs to be different information present on the claim form and within the patient's chart that does not include the patient's physician, date last seen, etc. The information needed for treatment of mycotic nails includes 1) the diagnosis of onychomycosis, 2) a secondary diagnosis that may include pain or other qualifying diagnoses, 3) billing code 11720 or 11721 and 4) an active treatment plan should be in your chart. The treatment of mycotic nails does not/should not require the patient's MD/DO info.
 
Of course, checking with the LCD policy of your Medicare carrier is always indicated for any specifics that they may require, but these are the basics for the billing and charting for nail care.

The FDA has for quite some time had a published policy regarding laser management of onychomycosis. It might prove instructive for PM News readers to examine this policy. The FDA approves lasers for temporary improvement in nail appearance. They suggest that not only is there no evidence to conclude lasers are effective for cure of onychomycosis, but that lasers ought not be used in high risk patients (e.g.: PAD, DM). Onychomycosis has been demonstrated in multiple studies to be a risk factor for more serious pathology such as cellulitis and gangrene in the patient with DM. It warrants appropriate treatment, including oral antifungal therapy, when appropriate. The isolated use of lasers as management for onychomycosis denies the patient with DM of treatment and allows a continuation of risks associated with this condition.
 
Because lasers are not species specific, if a laser is to be employed for cosmesis, speciation is not required. A KOH or PAS is sufficient to confirm the diagnosis of onychomycosis. Finally, there are ample studies to suggest that oral therapy is safe and effective in the patient with diabetes.
 
In my opinion, considerations regarding the evaluation and management of this commonly encountered pathology have been misrepresented in the profession as the result of corporate influence in our CME meetings, as well as biased presentations by "thought leaders" on the subject. We have a duty to effectively care for onychomycosis in the patient with DM, as with all patients seeking care for this problem.
