PODIATRISTS IN THE NEWS - PART 1
MA Podiatrist Discusses Bunions

Dr. James P. Ioli, chief of the podiatry service at Harvard-affiliated Brigham and Women's Hospital, cites several reasons some women develop bunions, including the following. Improper footwear. Women are 10 times more likely than men to develop bunions, and their choice of shoes -- particularly those that are too narrow across the base of the toes -- can make them worse. Genetics. Bunions run in families, because foot shape and structure are hereditary, and some types are more prone to bunions than others. Low arches, flat feet, and loose joints and tendons all increase the risk. The shape of the metatarsal head -- the top of the first bone in the foot at the joint with the big toe -- also makes a difference: if it's too round, the joint is less stable and more likely to deform when squeezed into shoes with narrow toes.
Dr. James Ioli

See a podiatrist. These health professionals can evaluate your bunion and provide additional solutions including custom-fitted shoe inserts to stabilize your foot, cortisone injections to relieve pain and inflammation, ultrasound therapy, and surgery to realign the joint. "The goal of surgery is to improve joint function," Dr. Ioli says. "We can't make the foot or bunion area perfect, but we can make it better."

PODIATRISTS IN THE NEWS - PART 2
Chronic Foot Pain Can be Prevented: MA Podiatrist

While your feet may seem pretty far removed from stress or chronic pain in other parts of the body, Dr. Teresa Burtoft, a podiatrist with the Foot Health Center of the Merrimack Valley, explained that the connection is more common than people realize. "The only time they start to figure it out is when they have chronic pain for a long time and they start to become emotionally distraught from the pain and not being able to do what they want to do," she said.

Dr. Teresa Burtoft

"People don't realize it can be a problem until they have it, but if they take the steps to prevent these problems, they're going to be a lot better." Wearing shoes that offer the proper support and cutting back on walking around barefoot are two of the easiest ways, Burtoft said, that people can prevent chronic ankle, knee, hip, and back pain.

MD Podiatrist Publishes Book on Pain Management 2nd Edition

Based upon success with thousands of patients as well as seven previously published articles in podiatry, diabetes, and family practice journals, Dr. Stuart Goldman presents a new paradigm for pain management, through which podiatrists can frequently successfully relieve symptoms associated with all the conditions mentioned above, often within one to two days of appropriate treatment.

Dr. Stuart Goldman and Walking Well Again

Walking Well Again: Neutralize the Hidden Causes of Pain presents original insights and techniques designed to guide podiatrists to better understand, investigate, and  frequently relieve chronic pain. Dr. David Armstrong says, "This book - like no other I can recall - bridges the gap between lofty academic research and immediately useful tips and tools."

PODIATRIC ECONOMICS
Podiatrists Rank #11 in 50 Highest-Paying Jobs in the Healthcare Industry.

Few industries are expected to grow as fast as the healthcare sector over the next 10 years. The Bureau of Labor Statistics estimates that employment in all healthcare occupations will grow 19 percent from 2014 to 2024, adding about 2.3 million jobs.

In general, specialized medical fields like anesthesiology, podiatry, and optometry offer high-paying career tracks. Not every top-paying job requires an MD. For example, podiatrists earned an average annual salary of $133,650, placing them at #11 on the list.

HEALTHCARE NEWS
CA Lawmakers Consider Dumping Health Insurers

California lawmakers are considering an audacious proposal that would substantially remake the state's healthcare system by eliminating insurance companies and guaranteeing coverage for everyone. The idea known as single–payer health care has long been popular on the left. It's gaining traction with liberals as President Donald Trump struggles with his efforts to repeal and replace the Affordable Care Act.

The measure would guarantee health coverage with no out–of–pocket costs for all residents of California, including people living in the country illegally. Private insurers would be barred from covering the same services, essentially eliminating them from the marketplace. Instead, a new state agency would contract with healthcare providers such as doctors and hospitals and pay the bills for everyone.

PRACTICE MANAGEMENT TIP OF THE DAY
Healthcare Fraud 101: Under False Claims Act - Part 3

3. The third step is to ensure that your practice does not retaliate against any possible whistleblowers. You cannot engage in a "witch hunt" to try to find a whistleblower or fire the person you have identified as the possible whistleblower. The FCA contains steep penalties for retaliation.

CODINGLINE CORNER
Query: Bone Plate and Screw Placement Coding

I recently performed a surgery at the outpatient center whereby I did a 1st metatarsal-phalangeal joint arthrodesis.  I used a Stryker bone plate with four screws. I am looking for a procedure code for my putting in the bone plate and screws. Please tell me what procedure code (CPT) to use so I get compensated for that. I know to use the CPT 28750 code for the procedure, but there should be an additional code for me to use for the placement of the bone plate and screws.

Barbara Rien, DPM, Boca Raton, FL

Response: You have the correct CPT coding for the arthrodesis of the 1st metatarsal-phalangeal joint. That’s the good news.  The bad news is that there is no additional payment for complex fixation hardware. So whether you use two crossed K-wires or use compression plates, cancellous screws, or monofilament wire, the reimbursement for the surgery is the same. Be my guest and add a “-22” modifier indicating greater complexity than a standard procedure, but I wouldn’t hold my breath for added reimbursement.

QUERIES (NON-CLINICAL)
Query: Insurance Eligibility Checking

We try to check insurance eligibility and our participation in our patient’s health insurance program before they arrive at our office.  At times, this is done when the appointment is made. Some time can be saved by using a service on line such as Availity, a corporation founded by Humana and BC/BS, for data on deductibles and co-pays. However, details of specific CPT code coverage, i.e., Orthotics  L3000, can only be completed via a long and arduous phone call to the patient’s health insurance company. Any suggestions on how to make this process more efficient? Is there a better service for finding this information?
