OH Podiatrist Discusses Neurolysis for Neuropathy
 
Sometimes surgery can help address nerve damage, says Peter Highlander, DPM, a podiatrist based in Bellevue, Ohio. Through neurolysis, a surgeon may remove any scar tissue or constricting tissue around or within a nerve to release pressure. Dr. Highlander says that in his practice, certain nerve surgery procedures work better when the patient still has some sensation in the nerve.

Neuropathy can lead to complications during surgery as well, explains Dr. Highlander. That is because of a condition called Charcot neuropathic osteoarthropathy in which inflammation and unaddressed injuries subject bones, joints, and soft tissues (especially those in the feet) to microfractures and deformity. This can have many causes, including diabetic neuropathy.

RELEVANT RESEARCH
 
Objective - To measure the effect of neuromuscular stimulation of the common peroneal nerve on the microvascular blood flow within the wound bed of diabetic foot ulcers.
 
Research Design and Methods - 13 patients with diabetic foot ulcers (11 neuropathic, 2 neuro-ischemic) were recruited. Microvascular flux and pulsatility were measured in real time in the wound bed, and at the wound edge, using Laser Speckle Contrast Imaging. 
 
Results: In the 11 neuropathic ulcers, significant increases were seen in all microvascular parameters. Wound bed flux increased by 36% (95%CI 11%-68%, P = .002), and peri-wound flux increased by 92% (95%CI 46%-160%, P = .001). Pulsatility in the wound bed increased by 183% (95%CI 61%-517%, P = .005), while pulsatility in the peri-wound increases by 359% (95%CI 264%-455%, P = .001). Additionally, an increase in flux and pulsatility was observed in the neuroischemic ulcers.
 
Conclusions - Neuromuscular stimulation of the common peroneal nerve presents a mode of activating the leg muscle pump without additional pressure insult to the foot and has potential to support the microcirculation during wound-healing, effectively emulating exercise without any of the attendant risks of exercise to the neuropathic or neuro-ischemic foot.

RE: Elevated Serum Vitamin B12 Levels May be Linked to Adverse Health Outcomes

Recent studies indicate that persistently elevated serum vitamin B12 levels–as may be the result of high dose oral B12 supplementation (≥1 mg or 1000 mcg per day)–may be linked to adverse health outcomes. A meta-analysis by Liu, et al. of 22 cohort studies (92,000 individuals) showed a positive association between a high serum vitamin B12 concentration (>813 pg/ml) and all-cause mortality. This effect was greatest in older patients and is consistent with the work of other researchers. Serum vitamin B12 levels in the range of 400-500 pg/ml appear to likely optimize neurological function while reducing the risks associated with both deficiency and excess.
 
Given these new findings, healthcare providers who dispense or prescribe high dose oral vitamin B12 for their patient’s neuropathic symptoms should consider regularly monitoring their patients' vitamin B12 levels and adjusting supplementation as necessary.

I found this interesting article on the Stark law changes that occurred on Jan 1, 2022, and it also provides detailed explanation regarding designated health services (DHS) with respect to physician compensation in a group practice. After being in solo practice for nearly three decades, the only time I knew about the Stark law was regarding the exception of self-referral and in-office x-ray, and that was it. Boy, was I in for a rude awakening. So, this is a rather eye-opening article indeed and should be considered a warning to any new resident or fellow graduate or anyone who is planning on signing an employee contract in a hospital-based or group practice. Do your due diligence before signing the contract.
 
Outside of the volume of physician services that are a basis of compensation for physicians (usually a contracted percentage minus collection and non-claim overhead expenses), these ancillary services provided by a physician in a group practice better known as DHS are paid to the physician via...

There is insufficient space and time to comment here on a comprehensive review of the current state of the cellular tssue product (CTP) policy. Sufficient it is to say that the $10B price being currently paid for CTP is not a podiatry-centric issue. Wound care is big business and DFUs are only one type of wound receiving CTP. The proposed CTP policy does not address decubitus wounds, surgical dehiscence, or any other type of wound other than DFUs, and to some degree venous leg ulcers (VLU) of the lower extremities.
 
In the August 2025 issue of Podiatry Management, there will be a full-length discussion of the myriad of issues all wound care providers must face. The reality is that there must be a reasonably fair policy for all 3 entities involved: patients, providers, and CTP manufacturers. This, in order to provide more affordable cost-effective care for patients with DFUs and VLUs. Use of these expensive products for other than DFUs and VLUs must also be addressed.

Dr. Pearl posted a video about doctors' offices having to obtain a pre-determination in order for insurance to cover a hospital admission for a serious and potentially limb-threatening infection. In my 20 years of practice, I have never had to do that. Maybe the insurances he works with are different, but I think most hospitals have administrative staff on hand who contact the insurance companies immediately upon admission to get those authorizations in place.  
 
Hospitals have a very vested interest to make sure the admission services are covered or they lose out hugely. When a badly infected patient shows up at my office, we arrange a direct admit through the hospitalist team so the patient can avoid hours in the ER and the huge bill that comes along with the ER visit. This saves the insurance company and patient a lot of money and time.
 
Similarly, for things like MRIs and CTs, my office does not do any pre-authorizing for those. The imaging center that is providing the MRI and profiting from it should have staff to do that work. My staff only pre-auths surgeries and orthotics, that's it. In these times of declining reimbursements and increased bureaucracy, we all need to minimize these headaches as much as possible.
