Diabetic Wounds Require Protection from Sun and Sand: NY Podiatrist 
 
“I can’t tell you the number of near disasters I see in my office every summer from people who say, ‘Well, I just slid my shoes off for a few minutes so I could play beach volleyball with my kids,’ ” says Harvey Strauss, DPM, a podiatrist at the Hospital for Special Surgery in New York. At the shore, wear a closed-toe beach shoe, ideally one that you can also wear into the water.

Cover open wounds with bandages, which will block UV rays as well as prevent harmful bacteria from entering your skin and causing infection. If you have more extensive wounds or ulcers, you may need to wear a protective boot or even use crutches, especially if the ulcer is on the bottom of your foot. “Both help reduce pressure and irritation to the area, which allows it to heal faster,” explains Strauss. Until your doctor clears you, stay out of the pool or ocean, both of which harbor germs that can cause a sore to get infected.

UK Podiatrist Discusses Blister Treatment
 
Mike O’Neill, a consultant podiatrist in Windsor says, "Ideally, it (a blister) should be left to heal naturally, because breaking the skin could lead to an infection or delay healing. If it’s smaller than a 5p (a U.S. dime), leave it and cover it with a blister plaster such as Compeed.

If the blister is big, or it happens in the middle of a big walk, it might be better to pop it. Use a sterile needle – run it through a flame on a match or lighter – then gently pop the blister and drain the fluid before covering it with a plaster (Band-Aid)."

CMS Proposed Rule on EHRs Seeks to Reduce Provider Burden
 
The Centers for Medicare and Medicaid Services this week followed through on its promise to institute an overhaul of the Meaningful Use program designed to reduce the burdens on providers in terms of time and cost, while also increasing the programs focus on interoperability. The policy changes to the Meaningful Use program—which has been renamed the “Promoting Interoperability” program includes the use of 90-day electronic health record reporting periods in 2019 and 2020.
 
Further, the proposed rule includes the requirement for providers to use the 2015 Edition certified EHR technology in 2019 to qualify for incentive payments and avoid reductions to Medicare payments. In announcing the proposed rule, CMS Administrator Seema Verma said the aim is to remove unnecessary, redundant, and process-driven quality measures from a number of reporting and pay-for-performance programs.

Query: Horizon NJ Direct Medicare Advantage Plan L3000 Denials
 
We’ve been having a problem getting L3000 paid for our retired patients who are now covered with Horizon NJ Direct Medicare Advantage (they were previously in the state plan Horizon NJ Direct before retirement). Has anyone had this same issue? If so, how did you resolve/appeal it? We also heard that the coverage from the previous policy is supposed to be grandfathered into the new Medicare advantage policy. We’d really appreciate any guidance on this.

Response: This situation has been discussed at the Board of Trustees of the New Jersey Podiatric Medical Society (NJPMS) in the past. We have been attempting to work with Horizon on a resolution. I suggest, after contacting the insurance plan and trying to resolve the claim(s) on your own, contacting the insurance committee of NJPMS and submitting your documentation. They may be able to assist with the appeal of your claim(s).

Whether HIV or meds that are used to treat HIV cause a lack of resistance to the virus that causes warts is a good question. Unfortunately, asking that question to most readers of PM News might not give us the answers we are looking for. Why? Many people will not tell a podiatrist or dentist in private practice that they have AIDS. Only on rare occasions do patients tell me that they are being treated for AIDS. I suspect that many more have the virus but don't want to divulge that aspect of their medical history.  
 
The same holds true for other sexually transmitted diseases. In the last 20 years, not a single new patient has told me about any medical history of having being treated for a sexually transmitted illness. Am I to think that these diseases do not exist in my community? Podiatrists working in hospital settings where "the truth" cannot be hidden might be in a better position to answer Dr. Sullivan's question.

RE: Suggestion for Recruiting Podiatric Students

I have two cousins who became dentists in the 1970s. My younger cousin was admitted to dental school with only two years of college. I believe podiatric medical schools should recruit very well qualified high school students and guarantee them a place in class if they complete the appropriate science courses, even if they do so before completing a 4 year (college) degree. I believe some students could do this in just two years. It would be possible for a well-qualified student to become a fully trained podiatric surgeon in 9 years after high school. This student would be actually only paying for 6 years of school as the last 3 years of training would be accomplished as a resident.
 
Podiatric medical students are now virtually guaranteed a residency position when they receive their DPM degree. This cannot be said for dental students or even medical students who are trying to go into a residency that they truly want.
