NYCPM Prof Presents Research on Empathy in Podiatric Medical Students

Peter Barbosa, PhD, Director of Biochemistry at NYCPM, presented preliminary analysis of his research work examining empathy in podiatric medical students at the Education Research Day 2015 of the Icahn School of Medicine at Mount Sinai in New York City. The team’s research findings demonstrate strong correlations between empathy, emotional intelligence, and personality traits in DPM students. Kurtis Bertram (NYCPM second year student research team member) will be presenting additional finings at the 2015 APMA Annual Scientific Meeting (The National) at the Orlando World Center Marriott in Orlando.

Walkathons Present Foot Health Challenges for Sedentary Adults
"Most adults do far less walking and standing for long periods of time, despite what they might think," says Thanh Dinh, DPM, FACFAS, a Boston-based foot and ankle surgeon and Fellow member of the American College of Foot and Ankle Surgeons. "There is potential for real harm, especially for individuals who may have existing structural foot deformities including bunions or other problems such as diabetes. Hence, it is especially important to prepare for the event by gradually increasing the intensity and duration of activity."

But conditioning programs that help individuals prepare for a distance walk are limited—unlike the numerous training programs aimed at helping non-runners get ready to run even short distances—and most walkers don't recognize that they need to prepare at all. Adults who engage in these activities also must be educated to know when to take a break, and not just "work through the pain," continues Dr. Dinh. "Knowing the signs of minor injury can help prevent a major injury."

Paperwork Dreaded by UK Podiatrist
Michael Gillies, a podiatrist from the Brechin Foot and Ankle Clinic was welcomed back to the April meeting of Laurencekirk and District Inner Wheel. Speaking passionately about his subject, Michael gave an illustrated talk and presentation which proved very interesting and informative. Podiatry, it seems dates back as far as the time of King Louis XVI who had a “Corn Doctor”, called Laurence Leforest.

Upon graduation with a BSc degree, a podiatrist can specialize in many fields, including dermatology, diabetes, forensics, vascular, rheumatology, and research lecturing. Before providing a guide for healthy feet, Michael outlined some of the tasks he would cover in a normal day in his 100 patient (per day) practice. These might include anything from routine foot care, acupuncture, steroid injections, vascular assessments, sports injuries, and minor surgery to, “The dreaded paperwork!”

I'm responding to the question about how to off-load a sub 1st met. head ulcer in a sandal- wearing homeless patient. I've tried every kind of off-loading in shoe padding for this type of ulcer and have been finding more luck lately with a metatarsal bar applied externally to shoes. This helped a diabetic patient with diabetic shoes off-load a chronic 1st met. head callus.
There is a small local shoe repair store in my town that assists me in this for about $20. It's about 1/4 inch of rubber 2 or 3 inches square just proximal to the met. heads. I don't know how to do it myself. Maybe other DPMs have advice on how to do this in our offices? Orthaheel sandals seem to help plantar fasciitis patients with the built-in orthotic foot bed. If the homeless patient has pes planus and is pronating off of the 1st met. head, then a sandal with an orthotic foot bed would benefit too.

1. "The Secretary of Health and Human Services will solicit recommended measures from professional organizations and others to develop additional measures.” Perhaps, we should have a national discussion among podiatrists – perhaps here on PM News – about what we really want, then have the APMA advocate it for us. We got blind-sided by the meaningful use/ PQRS nonsense. This is our chance to get ahead of the wave instead of blindly reacting to it. We need to help CMS set up performance standards that apply to podiatry. Since they emphasize prevention, perhaps this is also the opportunity to try to get rid of the restrictions on “routine foot care”.
2. “Professionals will receive credit for improvement from one year to the next in the quality and resource use performance categories in order to incentivize improved performance.” The penalties and incentives start at 4% the first year and grow to 9% 3 years later. Perhaps, it would be best to intentionally score bad the first year, so that you show improvement the following years? Take a 4% penalty hit the first year to try to get the 9% bonus?

According to most insurance companies, ESWT is "experimental", which is their way of not having to cover a service. Having done hundreds of like treatments, both high and low voltage, I agree with your assessment. The other difficulty we have is that the unit has to be used in a hospital setting and brought in from another city. Because of these factors, I have been performing a procedure using Tenex ultrasonic probes. I have been doing this for chronic plantar fasciitis and Achilles tendinitis for the past 6 months and have been very pleased with the results, especially on the Achilles. In fact, some of the results have been amazing. I have no long-term patients yet, but hopefully the improvement seen in these patients will be permanent.
