ACA High Deductible Plans Burdensome on MI Podiatrist

The growth of high-deductible plans has caused John Arsen, DPM, a podiatrist in a two-person practice in the Detroit suburb of Lake Orion, MI, to hire an extra biller and receptionist. Annual deductibles in such plans -- particularly low-premium plans sold under the Affordable Care Act -- can range from $2,500 to up to $4,300.

"[Patients] are frequently upset because they need to pay for that service because of the large deductible," Arsen said. For solo-practicing specialists, gaining referrals can be difficult as well. Hospitals buy practices to solidify their referral base and encourage their newly found physicians to keep referrals "in house."

CA Podiatrist Now on List of Top Democratic Prospects 
 
Dr. Lee Rogers, a Democratic candidate, who the party recently placed on a list of top prospects, said he was confident the national party would be there for him. But he said it faced hard political choices. Rogers has taken the lead in fundraising in the 25th Congressional District race, with more than $500,000 in total contributions, according to Federal Elections Commission finance reports.

“I think they are probably going to focus on the races that are a real possibility and pull back from some of the more fringe races, and a lot of those are drying up in the current climate,” said Rogers, a podiatrist seeking a vacant congressional seat in Southern California. “They don’t make a lot of promises to anybody.”

I applaud Paul Scherer's message in regards to reactive arthritis. I agree, the name  (R.S.) is synonymous with Nazi war crimes, and we should be sensitive to those Holocaust survivors, and their offspring.
 
A special congratulations to Paul who recently completed his year of service as President of the Physicians Fellowship for Israel Society. It was a great honor for a podiatrist to serve in this capacity. I remember fondly when Paul began the volunteer fellowship program for podiatrists in Israel for which I served. He helped to make a great impact on podiatric medicine and surgery in Israel. Thank you Paul for your contributions.

Our 19 providers (16 DPMs and 3 MDs) have used Clinical Works for almost 4 years. We were early adopters having used EMR for 25 years. The system works well and like all of the other certified systems, there is a little more hassle than the old way of using MS WORD in an EMR program. Two thirds of our providers use templates mixed with Dragon dictation, and the others use Dragon almost exclusively.
 
In the past year, we have a few clinical assistants who act as "scribes" and fill in the medical history and drugs, surgeries, etc. They then type in whatever I am saying during my examination: "Range of motion at the ankle of 10 degrees with the knee extended, hallux valgus right, ingrown nail left hallux, etc." They get enough put in that I can quickly fill in the remainder at the same encounter or finalize the record at the end of the day. It usually takes 30 minutes for 35 patients. Like many others, the billing aspect is great, with billing going directly to insurance companies within 12 hours after it has been scribed.
 
I have no disclosure to report or any income or grants received from eCW. BUT, we do get meaningful use $$ for 19 providers.

Yes, the DEA in NY is an outrage, especially when I also pay for a DEA and CDS in NJ too. When I called NY, I told them that I never write for narcotics. They told me that I didn't have to renew with them, my prescriptions would be under my NY license, it would be no problem, and I didn't need the DEA.

Kudos to Dr. Markinson for standing up and taking a public stance on this apparent abusive billing procedure. As an expert witness and reviewer, I get to look into many doctors charts and look at their billing practices. I am amazed at how creative some podiatrists can get. I reviewed one case where the podiatrist charged $5.00 for using disposable gloves to treat the patient. He used a hospital code which the hospital would normally charge for a BOX of gloves when a patient is quarantined. It is shameful that podiatrists stoop to these billing practices. Thanks Bryan for pointing out this abuse. Maybe these docs will be shamed into correcting their ways.

I have been in communication with Lawrence Harkless, DPM for about five years now to develop this type of program for the podiatry profession. I paid $9,000 for the three-day CPEP program, only to discover that the program was essentially for MDs, DOs and PAs. For example, the assessment was a sort of a template, and it was evident that the practice approach was different. The CPEP report improperly stated that I would be classified as an "Infectious Disease Podiatrist." 
 
We know that our profession does not have these distinguished sub-specialties. This was just the tip of the iceberg. The new California program is a welcomed addition for podiatrists. We will need more trained U.S. podiatrists in light of increased demands of the Affordable Care Act.

Wedges are not better than orthotics for cycling shoes! Feet can and do pronate while cycling, on and off the saddle. As for orthotic design, it's important that forefoot varus or valgus is addressed, as they directly affect knee alignment. Many times, I order met pads. I use TL2100, which can be thinner than what I use for gait. I use different orthotic designs, depending on the shoe, and may recommend a different shoe.
 
I agree that pedal "float" is very important. It was first introduced by Look and then picked up by Century (the same Look company made ski bindings.). I only wedge after I have stabilized the foot by placing the orthotic in the shoe, and then doing a bike set-up in a bike shop. Personally, I like "Frogs" (Speedplay clipless pedals) and Diadora road shoes.
