PODIATRISTS AND HUMANITARIAN EFFORTS
PA Podiatrist Offers to Treat Migrant Children Pro Bono

A plane touched down at Erie International Airport days ago carrying 146 migrant children from the United States-Mexico border crisis. Many of these kids took the majority of this trip on foot, a cause for concern for Erie podiatrist Rick Tomassi, DPM. He specializes in foot injuries, and says it's hard to know now, but the potential injuries these kids are facing could impact them for the rest of their lives.

Dr. Rick Tomassi

"Anything from cuts, bruises, lacerations, sprained ankles - when you walk that far, and you're that young, I'm sure that there are injuries to deal with," says Dr. Tomassi. When Tomassi thinks about these kids, he says he just wants to be able to help.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Ways to Maximize Your Referral Program - Part 4

Traditionally, obtaining referrals consisted of visiting a physician’s office bearing breakfast baskets and new literature, organizing open houses, or hosting referring physician lunches. Digital referrals, however, guarantee you’ll reach the right person at the right time in the right place. Often, digital is cheaper, faster, more efficient, and is preferred by physicians in some cases.

4. Launch Facebook and LinkedIn Advertising

Keen to reach new referring physicians? Facebook comes equipped with a number of detailed targeting options that allow you to identify healthcare professionals in your vicinity and target them with engaging content or customized ads about your practice. LinkedIn, meanwhile, was designed for professional peer-to-peer networking and offers a host of targeting options for its paid ads, including specific industries, roles, and specialisms.

MEDICARE NEWS
2% Medicare Pay Cut for Physicians Delayed Until 2022

The 2 percent Medicare pay cut for physicians has been put on hold through the end of the year, according to a report from the American Medical Association. President Joe Biden signed a bill into law April 15 that extended the moratorium on Medicare pay cuts through Dec. 31. The previous moratorium was set to expire April 1 and was temporarily delayed while Congress voted on the bill.

Later this year, legislators may also take steps to avoid a 4 percent Medicare pay cut scheduled for Jan. 1, 2022, which was enacted to offset the costs of the American Rescue Plan economic relief package, according to the AMA's report.

QUERIES (CLINICAL)
Query: Foot and Ankle Pain Post-COVID Vaccine

I have seen 4 patients who have developed increasing foot and ankle pain post-COVID vaccine. They seem to do worse after the second vaccine dose. Has anyone else had this experience?

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Peter J. Bregman, DPM, Kenneth Meisler, DPM

It sounds like a duck (Morton’s neuroma) so likely it is. If steroid injections did not cure the “bursitis” then maybe it is not that. MRIs are great but not always right, especially with diagnosing MN. If this were my patient, I would say we are going to the OR for a decompression of the nerve and if I see a bursal sac in there, I will remove it. Unless the bursal sac is plantar to the deep transverse ligament, there should not be any walking on a pebble feeling. Maybe there could also be a plantarflexed metatarsal head there as well.

Peter J. Bregman, DPM, Las Vegas, NV

I would refer the patient to someone experienced in ultrasound-guided injections. Injecting directly into the bursa is usually successful.

Kenneth Meisler, DPM, NY, NY

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Robert S. Schwartz, CPed

Overuse, repetitive stress injuries from excessive forces occur during the propulsive phase of gait, particularly in the sagittal plane. Managing motion is an effective way to relieve these forces.

High-toe-box, extra-depth, athletic walking shoe with built-in (L) and added rocker (R).

These photos reflect the use of high-toe-box, extra-depth, athletic walking shoes with built-in rockers. When increased motion control is needed, an additional rocker is added to increase toe-spring (dorsiflexion motion mitigation).

Disclosure: I have no fiduciary relationship with Apex.

RESPONSES/COMMENTS (NEWS STORIES)
From: Michael Loshigian, DPM

Dr. Peacock and Dr. Katzen, if you read my comments again, you will find that my purpose was to draw attention to the fact that we should not be presenting MIS surgery, or any foot surgery for that matter, to the public as a quick fix. To tell patients that minimally invasive techniques result in a faster recovery is simply unfounded. The concept that the patient's perception makes it so is completely unscientific when it comes to soft tissue and bone healing. Perception of course has a great deal to do with patient satisfaction, but that is not the topic of this discussion.

Unless human physiology has changed, it still takes 6-8 weeks for bone to heal to structural stability and up to a year for the entire process to be complete (plus or minus in certain circumstances, of course). This is generally true whether it is a fracture, minimally invasive osteotomy, or...
