PODIATRISTS AND DIABETES
Increasing Number of Young People Have Diabetes - TX Podiatrist

During recent years, Luis R. Venegas, DPM said he has been seeing more young patients in the Valley, including some in their 30s and 40s, developing foot wounds related to diabetes. “Since we are having this surprising increase in younger people having complications with their feet, I believe it is time to increase education and awareness, not only among the elderly population, but among the younger population; in colleges, high schools, etc., regarding the importance of diet, exercise and general health awareness,” Venegas said.

Dr. Luis Venegas

Dr. Venegas said that diabetic patients should check their feet every day to look for lesions, ulcers, or any other problems which could lead to damage and dangerous infections of their feet. If foot problems are not properly cared for, diabetic patients run the risk of progressively increasing problems, which could result in amputations.

RELEVANT RESEARCH
Radiologists Freeze Phantom Limb Pain Using Minimally Invasive Procedure

J. David Prologo, MD, delivered new findings from an Emory Interventional Radiology and Image Guided Medicine study at the Society of Interventional Radiology’s 2016 Annual Scientific Meeting. The study examines the use of cryoablation, or extreme cold, to reduce phantom limb pain (PLP) – a condition that causes individuals to perceive chronic pain in amputated limbs. Emory’s study shows that interventional radiologists who applied cold blasts to patients suffering from PLP significantly reduced the patients' level of pain.

The clinical trial entitled, “Percutaneous Image Guided Cryoablation for the Treatment of Refractory Phantom Limb Pain” examines the safety and effectiveness of using cryoablation to treat patients with PLP. According to the Centers for Disease Control and Prevention, nearly 185,000 amputations occur each year in the United States. Prologo says that during the hour-long outpatient procedure, interventional radiologists use image guidance to access hard-to-target areas and shut down problematic nerves. “We insert a cryoablation probe into the nerves causing pain, and drop the temperature to negative 40 degrees centigrade for 25 minutes,” says Prologo.

PRACTICE MANAGEMENT TIP OF THE DAY

8 Last-Minute Tax Tips for Physicians - Part 5

"Under the IRS’ new tangible property regulations, you may be able to expense costs associated with a building you own or lease for your practice. Many expenditures that were previously considered capitalized items that were depreciated over a number of years, now have new guidelines, making them eligible to fall under repair and maintenance rules. This allows you to make the deduction in the current year."

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Is There a Role for Podiatric Physicians in the Current Zika Outbreak?
From: Leonard A. Levy, DPM, MPH

Little if anything appears in the podiatric medical literature about Zika, yet there is a potentially significant role for the profession in this outbreak. First, there is a public health role that the profession should accept regarding the disease. Patients may ask the podiatric physician questions about Zika, and as a health professional, an appropriate response will be expected. However, in 90-95 percent of patients infected with Zika, there is a maculopapular often pruritic eruption that appears within three to five days after the initial infection which, while beginning on the face, spreads to the trunk and the extremities (March 2016, Journal of the American Academy of Dermatology).

This eruption begins to subside within two to three days, resolving completely within about a week and healing with desquamation. While the eruption is not pathognomonic, it should raise the index of suspicion of the podiatric physician that this dermatological manifestation may be due to Zika. In addition, arthralgia also is very common, and it could appear in the feet and ankles, although the majority of Zika-infected patents remain asymptomatic (about 80 percent).

Another finding that may be associated with Zika is peripheral edema, which could affect the feet and ankles. A maculopapular eruption together with arthralgia and peripheral edema should further elevate the index of suspicion of the podiatric physician regarding the possibility of Zika. Also associated with Zika has been a dramatic increase in Gullain-Barre syndrome, which begins with weakness or paresthesia that may begin in the lower extremities.

RESPONSES/COMMENTS (THE FUTURE OF HEALTHCARE)
From: Bryan C. Markinson, DPM

I respect Dr. Hrywnak’s knowledge and research in the areas of practice economics. Dr. Hrywnak writes that the implementing of analytics led to the discovery of practices that were costly or as he described it, “associated with a high dollar amount” necessitating reductions in escalating CPT billings. He goes on to say, “The results we have seen to date (and there are more to come), are systematic quality improvements along with cost reduction, enhancing the physician-patient relationship and clinical risk intervention with population management.”

His grasp of economics aside, he seems to be hook, line, and sinker in agreement that the physician-patient relationship has been enhanced. I am outraged by this sentiment, no matter who would utter it. Hospital staffing is cut to the bare bone. Attention to the computer at each visit is so glaringly disastrous to the physician-patient relationship, that only someone who sees six patients a day could possibly view it as an enhancement. I and everyone else could go on and on. What distresses me most is when physicians themselves get on board in agreement with what is happening. If we were iron workers, brick layers, or transit workers, we would have brought the system to a halt by now until someone demonstrated that they were listening. Understanding that changes need to be made is not the same as blanket acceptance of the decimation of medicine as we knew it.
