According to Conway McLean, DPM, "Complex regional pain syndrome (CRPS) causes severe and enduring pain and results from some seemingly insignificant injury. Surgical procedures can be such a cause or even a minor sprain. Pain develops far out of proportion to the trauma which becomes entrenched and intractable with time, untouchable with the strongest opioid pain relievers. As with many neurologic conditions, there must be some mechanism, but we simply don’t understand the “why’s and how’s” of CRPS."

"Pain is a complex, multifactorial experience that comprises multiple elements. The perception that something hurts includes sensory features like its intensity, quality, and location. But chronic pain is a whole other animal and leads to distress and disability. It can impair basic functional activities, like the all-important activities of daily life. Objective measures, reproducible person to person, are lacking. This deficit makes the study of pain inexact, imprecise, and prone to guesswork. We simply don’t know enough about pain, but the same can be generally said for the field of neurology. We need better solutions for chronic pain, new directions like encouraging healing rather than addressing symptoms," says Dr. McLean.

UK Podiatrist Discusses Post-Marathon Trauma to Feet
 
Unsurprisingly, considering you’ll have taken more than 50,000 steps, your feet and ankles will be exhausted. “It is only natural that your feet will feel like they have been in the wars,” says podiatrist Heena Patel. Patel says blisters, caused by friction, will very likely be present post-marathon. “Never attempt to pop them at home as it may cause an infection. Instead soak the feet in warm, salty water; and this will eventually draw the fluid out and cause the blister to drain.”

Due to a build-up of lactic acid, you may experience pins and needles, burning, cramps, or weakness in your feet (and legs). “This is perfectly normal and a sign that your muscles are recovering,” Patel explains. “Treat your feet to a foot massage and show them some love,” Patel continues. “The gentle manipulation and massage will make them feel more human again and untighten the muscles in the feet, stimulating blood flow to the feet, which can aid recovery.”

IL Podiatrist Reviews Shift from ASCs to Hospitals
 
According to Patrick McEneaney, DPM, Owner and CEO of Northern Illinois Foot & Ankle Specialists, "Since 2019, there has been a large shift of doctors moving from private practice to working for hospital systems. This number is nearing 80% nationally. This shift has led to these hospital employed surgeons performing their surgeries at the hospitals instead of surgery centers and there can be an unwritten pressure from the hospital for these doctors to do so."

"Because of this, recruiting is having to focus on the remaining independent surgeons. Additionally, we are seeing our patients sicker than previous years with more medical co-morbidities. New Zealand Podiatrist Prescribers Will Increase Access to Medicines
 
Regulations that will soon allow podiatrists to prescribe certain medicines have been approved, giving patients with painful foot and leg conditions better access to healthcare, Health Minister Simeon Brown has announced.
 
“This means podiatrists will join other health professions that have designated prescriber rights, including specially trained nurses, dieticians, and pharmacists. Medicines will include some oral painkillers and oral antibiotics, and some topical antifungals, topical steroids, and local anesthetics, Brown said.

Query: DME, Stark and Designated Health Services (DHS)
 
As part of my review, I have seen that DME is particularly problematic as there are certain Stark laws regulating incentivizing certain "Designated Health Services". What do I do to stay compliant with these issues and still be able to offer bonuses for performance?
 
Response from thedoctorline.com: We suggest you contact an attorney well versed in Stark and contract law.  Stark provides certain exceptions including ancillary services provided by the doctor. This includes DMEPOS familiar to DPMs including surgical dressings, therapeutic shoes, AFOs, etc. But most ancillary services must be provided by the DPM if they are to stand up to the Stark exceptions and particularly those designated with Stark as Designated Health Services (DHS). Shoes and surgical dressings are not designated as DHS and so their provisions in terms of delivery, who at the facility dispenses, and how bonus incentives can be structured are different from those which are designated as DHS. 
 
The incentivizing and providing bonus structures for DHS is much more complicated. This sounds complicated because it is, and you should engage a healthcare attorney well versed in these matters. Additional coding and billing questions and responses can be found at thedoctorline.com

Dr. Balaj's post on the cost of wound care through Medicare is interesting, but an undermining theme that's not clearly brought out is what has been described in many posts through this newsletter by large numbers of podiatrists. Aging population aside, a main factor in driving up increasing costs for wound care is the overuse advanced wound care products. William Marston, MD, well known vascular surgeon, specializing in wound care wrote an interesting paper published in 1999, which is still valid  today. Looking at approximately 200 patients that had healed their venous ulcers with a simple therapy of ambulatory compression therapy with basic standard wound care, his conclusion back then is current today. Only a small number of patients require advanced products - most required nothing more than standard quality wound care.
 
A caution to the young practitioner is that Medicare computerized services can monitor what we do as practitioners more accurately every day. That can be used against you and your practice if you are labeled as an over-user of these products. Be cautious. As in everything else, Medicare is a business and it looks for the best return for the dollar. 
 
Don't be foolish. Don't jump at the advanced collagen and other more costly products as first choice as a standard routine. Use these products selectively when something more than standard care is required.
