PODIATRISTS IN THE NEWS - PART 1
Insurance Likely to Cover Non-Cosmetic Bunion Surgery: NY Podiatrist

You love those high-heeled shoes, but your feet don't. Now you have a bunion—a painful bony bump on the joint of your big toe. This is a foot deformity.

Dr. Josef Geldwert

According to Josef J. Geldwert, DPM, a sports medicine doctor and podiatrist in New York City, "more health insurance companies will cover a bunion procedure than you may think. It all depends on the circumstances, namely, if your bunion is causing pain and impacting your functioning," he says.

Vinegar Soaks May Help Prevent Onychomycosis: CO Podiatrist

Vinegar is an old-time remedy that people still swear by, citing beauty benefits and a purported ability to aid weight loss and skin concerns. And it can possibly aid the treatment of toenail fungus. "Be sure to dry well between the toes and apply moisturizer," says Cara Wylie, DPM, primary podiatric medicine specialist with UCHealth Foot and Ankle Center-Central in Denver, who has seen positive outcomes when her patients soak three times a week for a year.

Dr. Cara Wylie

"It takes nine months to a year for the nails to grow out, so this is why you have to do this for so long a period. There are other soaks, but I find this to be safest and best," says Dr. Wylie.

INTERNATIONAL PODIATRISTS IN THE COMMUNITY
Unprecedented Demand for Podiatry Services: Aussie Podiatrist

Unprecedented demand for podiatry services and a shortage of new graduates has resulted in Shepparton’s Nixon Podiatry feeling the squeeze as they work hard to accommodate all their patients. “I have been in practice for 13 years and have always managed to recruit a new graduate to commence at the start of a new year,” podiatrist and director of Nixon Podiatry, Sally Howes said.

Sally Howes

The registration body, AHPRA, was delayed with new graduate registrations, which thus delayed podiatrist recruitment, a knock-on effect from COVID-19.

PRACTICE MANAGEMENT TIP OF THE DAY
Ways to Play the Prior Authorization Game - Part 8

Prior authorizations (PAs) are a pain for physicians. Here are a few things you can do to improve your odds of winning.

8. Ask for the medical director of the insurance company.

Most of them were practicing medicine at some point and understand our struggles on the frontlines. If we present our medical reasoning with them, they often are able to authorize a test. Not always.

QUERIES (DME)
Query: Shoes for Diabetic Amputee

I have a diabetic patient with a BKA. He has a Medicare managed care plan. The family has found a shoe dispenser who accepts his insurance. According to the family, the dispenser wants an Rx from me for a pair of shoes. It sounds weird to me. Is this the usual practice to prescribe a pair of shoes for a patient with one foot?

QUERIES (NON-CLINICAL)
Query: Formula for Valuing a Practice

What is the current method in 2021 to value a practice for sale? Is it still based on one year’s net or a multiple or percentage of that? Is there any difference between a C-Corp vs. solo practitioner?

Mark Ross, DPM, Yardley, PA

Editor's Comment: Note that any formula would just be a potential starting point for the valuation of a practice. There are multiple factors which go into any valuation including, but not limited to, type of practice, age of equipment, lease terms, accounts receivable, type of insurances accpted, and location and demographics of the area.

RESPONSES/COMMENTS (RELEVANT RESEARCH) - PART 1A
RE: Treatment for Ungual Melanoma in Situ (Leonard A. Levy, DPM, MPH)
From: Bryan C. Markinson, DPM

Dr. Levy’s persistence on the issue of podiatrists performing Mohs surgery on the foot is perplexing. He is supporting his opinion based on advances podiatry as a profession has made over decades regarding residency training and practice act changes. Dr. Levy asks, "Are we or are we not physicians?" Personal feelings aside, including mine, the fact is that this question is still a raging debate closer to question than consensus, and has nothing to do with performing Mohs surgery.

He states, "We should not be denied training to perform any kind of surgery on the foot and ankle." Really? Sorry Dr. Levy, when it comes to many kinds of surgery involving the foot, others may be more endowed with the requisite knowledge, training, and experience. Would Dr. Levy support removal of a malignant nerve sheath tumor by a ...

RESPONSES/COMMENTS (RELEVANT RESEARCH) - PART 1B
From: Steven Finer, DPM

A few years ago, I had the procedure done on the tip of my nose. It was done at a major teaching hospital in Philadelphia. The surgeon had the MD degree, dermatology residency, and a fellowship in dermatopathology. He also had the resources of the institution, including all the complicated machines and stains necessary to do the procedure. It is self-evident, as pointed out by Dr. Markinson, that a podiatrist must have the same.
