TUSPM Lymphedema Program Enhances Patient Treatment
The lymphedema program created by the Temple University School of Podiatric Medicine (TUSPM) last year is enhancing patient treatment and sparking a substantial increase in patient volume – and alums can now replicate this program in their practices as well.
|
|
Dr. Marc Karpo treating a patient in the lymphedema and diabetic foot clinic |
Under the direction of Dean John Mattiacci, DPM, TUSPM’s podiatrist-run lymphedema and diabetic ulcer clinic – the first of its kind in Philadelphia – is utilizing medical device technology provided by corporate partner ReMarx Medical Services to improve the quality of life for patients living with chronic venous insufficiencies, lymphedema, and venous ulcers. TUSPM has also seen a rise in patient volume after embarking on a co-branded lymphedema awareness campaign in local media outlets.

NY Podiatrist Authors Book, to Host Book Signing Event
A surgical podiatrist working at the Veterans Affairs clinic on Marine Corps Logistics Base-Albany has a new book out, and is hosting a reception and book signing to help get the word out on the book’s publication and message. Dr. Simone Josey, author of More Than Medicine: What They Don’t Teach You in Medical, MA, Nursing, or PA School.
|
|
Dr. Simone Josey |
Josey, an Ohio native raised in Jamaica who also holds a master’s degree in Public Health, wrote the book primarily with healthcare providers in mind to encourage them to remember that practicing medicine is more than achieving a diagnosis. “(The message) was birthed out of a need in the marketplace,” she said. “After talking to patients day in and day out … they were still (concerned) because they were not getting a good bedside manner. There has been a breakdown in the customer service aspect. You are not taught that in medical school.

New Research Confirms Dangers of High Heels: TX Podiatrist
New research shows that over time, stylish stilettos could cause you to walk like you're older than you really are. "It's really difficult to get women to change the kind of shoes they want to wear," Sanford podiatrist Dr. Adam Nichols said. "Unfortunately, at times, I have to accept that." Nichols says high heels can not only create a high amount of pain, they can also change the way you walk and make you look older than you are. "There are some studies that link foot pain to wearing high-heeled shoes," Nichols said.
|
|
Dr. Adam Nichols |
High heeled shoes can also cause added stress on your knees and hips. "They have been implicated in osteoarthritis or positional changes that can cause pain," Nichols said.

Panel Upholds Late-Diagnosis Award in PA Malpractice Case
An oncologist's expert testimony in a medical malpractice case against a podiatrist did not compromise the validity of the plaintiff's verdict, the state Superior Court has ruled, upholding a $750,000 award. The court affirmed a ruling of the Philadelphia Court of Common Pleas on April 16 in Wilson v. King, a medical malpractice case involving cancer of the foot. "Because a podiatrist, Dr. Gorman, testified that Dr. King deviated from a podiatric standard of care, and the basis for that opinion predated a review of Dr. Schneider's report, which corroborated Dr. Gorman's initial report, we cannot conclude Dr. Schneider's testimony affected the verdict and led to an incorrect result," Senior Judge James J. Fitzgerald III wrote in a 13-page nonprecedential opinion.
Dr. Jack Gorman, a podiatrist, and Dr. Andrew Schneider, an oncologist, testified as experts for the plaintiff. According to Fitzgerald's opinion, both doctors testified that the delay in diagnosing Wilson's cancer caused the need for amputation of her foot. "In this case, Dr. Gorman, a podiatrist, testified that Dr. King 'deviated from the standard of care of a reasonable prudent podiatrist' by failing to timely biopsy Wilson's nonhealing ulcer to ascertain the existence of cancer," Fitzgerald said. "We add that Dr. Gorman arrived at his opinion prior to reviewing Dr. Schneider's expert report."

| HEALTHCARE LEGISLATION | |
Republican Budget Compromise Could Undo Obamacare
Republican negotiators on Monday reached a compromise on a joint budget between the House and the Senate, the first common congressional budget in a decade. The deal would increase military spending and take aim at President Obama’s signature healthcare law. It would also cut education and entitlement programs, like Medicare, but negotiators dropped a proposal by Representative Paul D. Ryan, Republican of Wisconsin and a former chairman of the House Budget Committee, to turn Medicare into a largely private voucher program.
Under the deal, Congress would use a procedural process known as reconciliation to repeal, or at least begin to undo, the Affordable Care Act. With reconciliation, Senate Republicans would need only a simple majority vote to push through legislation to repeal the healthcare law because it would not be subject to a probable Democratic filibuster.

RE: OIG Meaningful Use Audits
From: Michael M. Rosenblatt, DPM
It is unlikely that OIG will be approaching podiatrists directly for any audit data and asking for "voluntary information." That said, if they do, I most strongly recommend you NOT offer any data of any kind to OIG unless it is subpoenaed. And if it is, contact a healthcare attorney before you comply.
One of the characteristics about subpoenas is that you are ONLY required to turn over the material they ask for, and their request must be specific and detailed. Never turn over any extra materials, even if you feel it will "help" your situation. OIG is not trustworthy. They are not your "friends." Never send originals. You'll only see them again if they do harm to you.
Some years ago, OIG went to APMA and requested support from APMA to help obtain data from podiatrists. Not only did OIG lie to APMA, but also OIG reneged on their promises in every possible way. APMA was extremely supportive and helpful to me in my efforts to fight OIG on their so-called "routine foot care audit" of podiatrists. OIG's work was sloppy, incomplete, flawed, and junk science. A high school student could have done a better job than OIG. Technology audits are undoubtedly a "new" element for OIG. I doubt that they have the knowledge or experience to do them properly, or even honestly. Stay as far away from OIG as you can.
Michael M. Rosenblatt, DPM, (retired) San Jose, CA

| RESPONSES/COMMENTS (NON-CLINICAL) | |
From: Robert Scott Steinberg, DPM
I agree with Dr. Rubin. High energy ESWT works. When using the Dornier EPOS Ultra, according to protocol, and doing an ankle block, and a small amount of local on the lateral side of the foot (where the soundwave would exit the foot), I too had a 90% success rate. Early on, I had to repeat two cases, but I then realized that if the abnormal thickness of the plantar fascia extended more than 3 cm, "bathing" the full length of the abnormal plantar fascia with pulses and increasing the total number of pluses to 4800 did away with the necessity for a second treatment.
As for the effect of local anesthesia on the results, if an ankle block is performed, I can't see how it would negatively affect the results! Also, even with the ankle block, patients will feel pain at the beginning of the procedure, if the shockwave gets too close to the calcaneus. "Teasing" the origin will eventually enable higher energy pulses. Yes, absolutely, high energy ESWT should be covered.
Robert Scott Steinberg, DPM, Schaumburg, IL

| RESPONSES/COMMENTS (NEWS STORIES) | |
From: Leonard A. Levy, DPM, MPH
In the April 29 issue of PM News, it was reported that Senator Bill Perkins recognizes that the Foot Clinic of NY’s (FCNY) patient base has the highest incidence of diabetes in New York City, and he reinforces often in the community and in Albany the importance of podiatry in combating this epidemic. I assume that the correct word is “prevalence.”
Incidence refers to the number of new cases in a particular time period while prevalence refers to the total number of cases that exists at any point in time. For example, the incidence of diabetes may be 30 cases each year in a particular location, but the prevalence during any point in time may be a total of 300 cases at that location).
