OH Podiatrists Discuss the 4 Stages of Charcot Foot
Although every patient with Charcot Foot has neuropathy, not every patient with neuropathy gets Charcot Foot, and it's not clear why some get the disease. Samantha Figas, DPM who practices in the Cleveland Clinic system and Craig Frey, DPM, a podiatrist with University Hospitals who practices in Solon, check the feet of diabetics. Dr. Frey said he finds that about one-tenth of his diabetic patients develop Charcot foot. Sometimes, patients suffer an injury, but not all.

Although many sources point to three stages for the disease, Drs. Figas and Frey say there are actually four. Both include Stage 0," where the foot is red, warm and swollen, but x-rays show no damage to the bones. Stage 1, called the "acute" stage, includes the first three symptoms, but features the first signs of damage, such as joint dislocations or breaks. In Stage 2, the "subacute" stage, swelling goes down, and redness and warmth reduce. Imaging may show early signs of bones starting to heal. In Stage 3, the "chronic" stage, is one of healing and stabilization. There is no longer any redness or swelling. Although bones have "healed" and are hardened, the foot may not look as it once did.

NY Podiatrists Lecture at the Atlantic Regional Osteopathic Conference
The New Jersey Podiatric Medical Society (NJPMS) joined with the New Jersey Association of Osteopathic Physicians and Surgeons to present the Atlantic Regional Osteopathic Conference held April 23-26, 2025, in Atlantic City, New Jersey. Joseph C. D’Amico DPM, DSc, Professor of Orthopedics and Pediatrics and Samantha Landau DPM, MPH, Assistant Professor at the New York College of Podiatric Medicine/Touro University in the Division of Orthopedics were featured presenters.

In his lecture titled, “Prescribing Sport Specific Custom Foot Orthoses,” Dr. D’Amico discussed concepts to consider and factors to evaluate based on biomechanical analysis not only of the patient but also of the unique, severe, and often prolonged forces occurring in various sports. He emphasized that one type of device may not be suitable for every sport. The more thorough the evaluation, the more questions that are answered, the more time that is spent with the patient, the more accurate the prescription, the more successful the outcome will be. Dr. Landau presented, “The Truth About the Twisted Plate Theory”. She reviewed and critiqued the literature regarding this theory as well as presented the results of a recent paper discussing the results of different cast techniques on orthotic form and function, and concluded that excessive loading of the lateral column in an attempt to affect a valgus twist negatively impacts custom foot orthotic form and function.

Query: New Vs. Established Patient After Practice Merger
I am merging my podiatry practice into a large Ortho practice; can I bill my prior patients as new vs established?
Response from thedoctorline.com: In the context of your practice’s merger and the new Tax Identification Number (TIN), it’s important to understand how Medicare defines “new” and “established” patients for billing purposes. According to the Centers for Medicare & Medicaid Services (CMS), a new patient is one who has not received any professional services—such as Evaluation and Management (E/M) services or other face-to-face services—from the physician or another physician of the same specialty within the same group practice in the past three years.
This definition hinges on the National Provider Identifier (NPI) rather than the TIN. Therefore, even though your practice is adopting a new TIN due to the merger, patients previously seen by you or another podiatrist in your group within the last three years are considered established patients. Billing them as new patients solely based on the new TIN would not align with Medicare’s guidelines.
However, if these patients are now consulting with an orthopedic specialist in your merged practice and have not seen an orthopedic specialist within the same group in the past three years, they may be classified as new patients for that specialty. Any prospective candidate for podiatry should be shown the results of PM News’ recent poll regarding the rendering of palliative care. In past issues, palliative care has been described as podiatry's little secret. It's really no secret to those who are in practice, and the amount a practitioner will need to do should not be kept from people who are interested in the profession.

In my area, there are a fair number of providers outside of podiatry and the wound center of doctors performing wound care. Orthopedic doctors, general surgeons, and family medicine doctors over the last couple of years are wound care specialists now and billing Medicare.
Many of these applications are now being applied at home and being billed by doctors who never made home visits before. And yes, Ortho is doing wound care in my area. I can't help but feel that much of this is money driven.

I agree with every last word that Dr. Kornfeld writes in his post. The two questions he asks that I feel are most compelling are...Why is it that this problem is not front and center of every discussion? And why is there not a complete commitment on the part of this profession to address the misery, exploitation, and abuse that most podiatrists experience? Yes, why is that? Every stakeholder in medicine should be approaching this issue as if we were fighting for our last breath and nothing else, because I think we just might be. In fact, we may be at the point of no return.
In addition to all the physician issues mentioned by Dr. Kornfeld, I looked up how various hospitals are doing financially here in Cleveland. Four major Northeast Ohio health systems ended 2024 with budget shortfalls in the millions. One major hospital reported a loss of $142 million in 2024. That came after operating losses of $256 million in 2023 and $302 million in 2022. Lately, I have been paying more attention to what I am getting reimbursed by insurance companies and I am utterly shocked. None of this is sustainable for anyone in the medical field. I really don't understand why providers of medical care are not part of the solution for this country's financial healthcare problems, while insurance companies and their investors continue to get rich. If you keep having to bail water out of a ship, the ship is going to eventually sink.
