CA Podiatrist Takes Helm at WesternU
Western University of Health Sciences’ College of Podiatric Medicine is trying something unprecedented in podiatric medicine – having its students sit for the same licensing exam as osteopathic (DO) and allopathic (MD) students. To provide stability during this critical juncture, WesternU has tapped CPM Associate Dean V. Kathleen Satterfield, DPM as the College’s new dean, effective May 1, 2019.

Satterfield has worked at WesternU since September 2010 and provides leadership continuity as she succeeds Founding Dean Lawrence Harkless, DPM, who retired in June 2017, and Executive Associate Dean Lester J. Jones, DPM, MSEd, who has served as interim dean and will retire at the end of April. “Our college is going through some growing pains,” Satterfield said. “I was afraid if I stopped now maybe somebody with a different goal would come in. We’re trying something very ambitious here in California and that is a change in our license, in a way.”

Sever’s Disease in Heel on Rise: CT Podiatrist
"One of the most common soccer injuries we have seen is the large number of young soccer players complaining of heel pain. This problem is often caused by a condition known as apophysitis of the calcaneus or Sever’s disease. This condition consists of an inflammation of the growth area of the heel bone which has not completely matured, or closed together, and has developed in two parts. It is most commonly seen in boys and girls between the ages of 10-15. The pain is usually present in the back of the heel and is more pronounced in running and jumping sports," says Robert Weiss, DPM.

"Treatment consists of x-ray evaluation to make a proper diagnosis and to rule out any bone fractures. An orthotic insert is often needed to correct the biomechanical imbalances which may be causing a jamming effect on the heel plate; additionally, the elimination of any cleated shoes with less than four cleats in the heel area. In summary, watch for any warning signals; when a child complains of heel pain, there is usually a need for examination," says Dr. Weiss.

TX Podiatrist Discusses Ingrown Toenails
Ingrown toenails are caused when the border of the nail grows into the adjacent skin fold. "Over-aggressive pedicures, trimming toenails too short, and trying to trim the edges of toenails at the corners too aggressively can all cause the nail to get infected and grow further into the fold," warns D. Sean Sweeney, DPM, of Sweeney Foot & Ankle.

To correct an ingrown toenail is a simple in-office procedure, where the toe is numbed and the ingrown portion of the nail is removed. Phenol is then applied to kill the nail root, causing the nail to grow back properly. Relief is immediate.

Single Use Extremities Kit Cuts OR Time 33%: Study
A new, 14-month multi-center study comparing the use of single use extremities instrument kits to standard, traditional implants and instruments, is reporting that physicians may be able to reduce intraoperative time by as much as 33% and realize the attendant, significant savings. The study’s lead investigator was John Levin, DPM, with Orthopedic Center of Palm Beach, Florida. The study looked at first metatarsal phalangeal joint fusion in 71 patients using sterile instrument kits supplied by Flower Orthopedics, a Horsham, Pennsylvania-based foot and ankle surgical implant and instrument company.

The specific kit was the Midfoot FlowerCube, a single-use, sterile implant and instrument system. Dr. Levin, and his study colleagues at the other sites, utilized the kits over a 14-month period from February 2017 to April 2018 in both hospital and ambulatory surgical center settings. The results of the study were consistent with outcomes reported in prior literature. At eight weeks post-surgery, 84.51% of study patients experienced union of joints. By 12 weeks after surgery, 98.59% of patients experienced union of the joints. The study concluded that there was a nearly 33% time savings for each case that used the FlowerCube compared to procedures that used traditional implants.

Get Paid for Procedures Insurance Won’t Cover - Part 6
Getting paid for procedures continues to become more complicated. Many doctors are adding new procedures to their offerings that patients want and can benefit from, such as Botox to treat migraine headaches, dermal fillers for aesthetic purposes, laser therapies for scar reduction, stem cell and platelet rich plasma (PRP) injections for musculoskeletal disorders, genetic testing, and many more. But insurance companies often won’t pay for them. Here are some ways to get paid.
6. Make sure your staff knows what is covered and what isn’t. A patient doesn’t want to schedule an appointment and be told the insurance will pay for it and then be disappointed. Also, the patient will get angry if they have the procedure done thinking it was a covered service only to get a bill later because someone made a mistake.

Query: First MPJ Fracture Dislocation
I am looking for some insight in treatment/surgical planning for this 51 year old male who sustained a first MPJ fracture/dislocation 2 months ago. He was never treated and came to me with pain plantarly at the first metatarsal head and sesamoids with ambulation and palpation, as well as the hallux dorsally displaced 1.5 to 2cm. from the WB surface. The hallux IPJ is mildly contracted dorsally, but currently not painful with shoe pressure.

Radiographically, the sesamoid fractures are apparent but they are not widely separated; therefore the intersesamoidal ligament is likely intact. The hallux looks only dorsally subluxed vs. dislocated. MRI confirmed that the flexor hallucis brevis is intact, but thinned at its distal insertion. Closed reduction failed, as expected. I have considered a plantar approach to excise the distal sesamoid fragments and attempt to repair the flexor with a Mitek anchor, or a combination of a complete removal of both sesamoids and fragments with a hallux IPJ arthrodesis with or without a total first MPJ Silastic implant to allow better reduction. All suggestions are welcome.

Query: Are the Meet the Masters Podcasts Still Available?
I am a 2nd year student at CSPM. Recently, I noticed the Meet The Masters (Podiatric Success) website was down and I was unable to go back to listen to the old podcasts from other DPMs. I have listened to most of the episodes and it has allowed me to learn so much about the profession and how much progress has been made within the field.
In fact, one episode about the California's Physician and Surgeon Task Force is what convinced me to enroll in podiatry school and stay in California. It was also one of reasons which motivated me to run for the CPMA Student Representative position and be involved with the profession early on. My classmates have also benefitted from this podcast and has helped us realize the different paths that are available in this field and the work that needs to be done to help this field grow, such as student recruitment. Please advise.
