NYS State Senator Bill Perkins Tours Foot Center of New York
Recenlty, State Senator Bill Perkins toured the facilities of the Foot Center of New York (FCNY), the clinical affiliate of the New York College of Podiatric Medicine (NYCPM). Senator Perkins recognizes that the FCNY’s patient base has the highest incidence of diabetes in New York City, and he reinforces often in the community and in Albany the importance of podiatry in combatting this epidemic.

AAPSM Holds Day of Sports Medicine
AAPSM Fellow Dr. Clint Laird organized a great day of sports medicine at Barry University for the current podiatry students on April 25, 2015. Speakers included AAPSM past presidents Drs. Matt Werd and James Losito and current AAPSM Board member Dr. Brian Fullem.

Topics included hallux limitus in the athlete, stress fractures, alternative exercises for the injured athlete, and how to build your sports medicine practice. Dr. Losito is the team podiatrist for the Miami Heat and Dr. Werd works with the Detroit Tigers in Spring training. The AAPSM provides education to several of the podiatry schools each year on a rotating basis.

Dr. Ashleigh E. Korves was recently elected Associate Chief of Podiatry at Anne Arundel Medical Center. Korves is a native Marylander who grew up outside Annapolis and graduated from Wellesley College. She then received her DPM degree from the Ohio College of Podiatric Medicine. While in podiatry school, she was a member of the Pi Delta National Honors Society, and received the William L. Goldfarb Foundation Education Award. She completed a three-year comprehensive foot and ankle surgical residency at Yale New Haven Hospital in Connecticut and received the Dr. Gary P. Jolly Academic Achievement Award.

The 2015 Cherry Blossom Dermatology Seminar took place last weekend at The Westin Hotel in Baltimore, MD. The seminar had over 180 registrants, and a record number of sponsors and exhibitors. The bulk of the lectures were given by both podiatrists and dermatologists. This year, the seminar introduced DERMjunkies - roundtable discussions which gave the audience a new interactive way to learn.

Query: Diabetic Ulcer in Homeless Patient
I need some ideas on materials I can use to off-load a sub 1st plantar ulceration in a homeless patient who is not compliant. Due to lack of insurance, he has no access to wound care or other off-loading devices. I work at a community health center, so there is no time to apply a total contact cast. He has a plantarflexed 1st ray with a 4 x 9 mm Wagner Grade 1 ulceration with no evidence of infection. There is a good granular base to the wound. He tends to wear sandal-like shoes. Any help would be appreciated.

Query: Can Original Records be Shredded?
I always was told never to destroy an original document. Is scanning an original document and shredding that original document such as a signature on file or a paper chart acceptable practice so long as a digital copy of such a scan is placed and stored on a hard drive?

Editor's comments: PM News does not provide legal advice. The rules requiring the maintaining of original paper records were applicable before the digital age in which scanning and storing of documents is now routine. It is still important to note that a copy of scanned records should also be maintained in a secure off-site location. This protects you in the event of a flood, fire, or corruption of the digital files. One popular way of storing files is in the cloud. If you do this, make sure that all records are encrypted and that the storage meets HIPAA compliance standards.

Query: Medicare Penalties
During negotiations of the "doc fix", I thought I saw that they were ending all of the PQRS and meaningful use penalties and replacing them instead with an incentive. Is this true?

Response: There are some changes coming as a result of the SGR bill that passed. Here is a brief summary of what happens to meaningful use (MU) and PQRS:
The provision would create a new incentive payment system while sun setting several existing programs on the last day of 2018: (1) the meaningful use incentive program for certified electronic health record (EHR) technology, (2) the quality reporting incentive program currently called PQRS, and (3) the value–based payment modifier (VM). The Secretary would establish a replacement program, the merit–based incentive payment system (MIPS) that would accomplish the following:
1. Develop a methodology for assessing the total performance of each MIPS eligible professional according to performance standards described below;
2. Using the methodology above, provide for a composite performance score as specified below for each professional for each performance period; and
3. Use the composite performance score of the MIPS eligible professional to make MIPS program incentive payments (as described below) to the professional for the year.
The MIPS program would apply to payments for items and services furnished on or after January 1, 2019. So PQRS, VM and MU will sunset at the end of 2018 and be replaced with the merit-based incentive payment system (MIPS). This program will have winners and losers, so you could in theory earn an incentive or be penalized or be neutral.

Unfortunately Dr. Carver, EMR companies are not required to give you your information in a “Read-only” format or any other useable format. The courts have all ruled in favor of the software companies. Their only obligation is to “give” you your data, usually as a raw data dump which meets their legal requirements but is useless to you.
You can try to have a software IT expert try to extract your data before you cancel your contract, but be prepared to shell out thousands to tens of thousands of...

The current terminology is "focused" shockwave and "radial" soundwave (RSW), as "high" & "low" energy are no longer applicable. Research has shown that the traditional "high energy" devices had inferior results than the "focused" and radial devices. Radial devices can produce similar energy to shockwave devices.
Furthermore, research has shown anesthesia, particularly local anesthesia, decreases the effectiveness of soundwave therapy. Therefore, performing soundwave/shockwave in an OR setting with anesthesia is not only cost-ineffective, but also likely...

The question of where Name Withheld needs to take his practice should be predicated on what the needs of his family will be both in the near and far future. Name Withheld needs to have an honest dialogue with his spouse to accurately assess their future financial needs. A modest retirement nest egg will not take anyone too far. If you have children, then college tuition alone (undergraduate and graduate) will cost multiple hundreds of thousands of dollars. Add in possible weddings and helping with a down payment on a house, and you will quickly realize that a half a million dollars has just evaporated from your savings.
It seems that Name Withheld should maximize the last 10 years of practice to double down on pension and personal savings. Given the direction of medicine, joining a group practice is certainly worth evaluating.
