ME Podiatrist Provides Foot Health Tips
Nadine Atalla, DPM is a podiatrist and certified wound specialist at Northern Light AR Gould. She says proper foot care begins with daily attention. “First you need to prevent it [wounds] from getting any infection, any cuts, any calluses, any fungus… Check your feet every day to make sure you don’t have any cuts, any sores, anything… If it’s open, make sure you put on it antibiotic ointment, cover it with a Band-Aid.”

She says calluses, a thickened area of skin that develops in response to repeated friction or pressure, are often caused by poor footwear and foot deformities. “Keeping your feet dry is very important… This is the first thing that can promote fungus of the foot… Fungus thrives on three things: heat, wetness, and darkness… Make sure you dry your feet after your shower, especially in between the toes.”

NY Podiatrist Named Chair of Nassau University Medical Center
Former Orange County Health Commissioner Irina Gelman, DPM has been appointed as the board chair of Nassau University Medical Center (NUMC) by County Executive Bruce Blakeman. Dr. Gelman, Nassau County’s Health Commissioner, replaced Matthew Bruderman, who was terminated on Thursday.

Dr. Gelman, the former president of the New York State Association of County Health Officials, was hired by Orange County in 2018 after four years as public health director in upstate Fulton County. She played an integral role in leading Orange through the measles outbreak in 2018 and the COVID-19 pandemic response. She was named Nassau County’s Health Commissioner in July of 2022.

VA Podiatrist Discusses Regenerative Medicine on Pain Podcast
Ben Pearl, DPM was recently featured in the Truth About Pain Management podcast. Dr. Pearl highlighted regenerative medicine, a field that he had first been convinced was not just hype when he injected his own Achilles tendon 15 years ago after suffering a small tear in a soccer game. He used a combination of radial shockwave and then injected his tendon sheath with placenta tissue. He had plateaued with the shockwave treatment and the injection was the last piece of the puzzle to get him back to soccer.

Pearl stated that regenerative medicine can be divided into biologics like growth factors and energy modalities like laser, shockwave, and pulsed electromagnetic fields. He cautioned against using terms like stem cell therapy unless you are actually using stem cells derived from bone marrow aspirate or fat. Finally, he emphasized the need for more robust studies, even though there are some studies that have showed promise, and to set realistic expectations for patients.

Query: Rejected Claims for Medicare DME to Novitas Solutions
Has anyone had recent issues with rejections of claims for DME products (i.e., L4360 CAM walker)? We have been submitting these claims with laterality and -KX modifiers. Now, out of nowhere, they are rejected saying "invalid information as specified in status details." We have tried new modifier combos, just -RT or-LT, just- KX, added -NU... none work. We called them and they said modifiers are incorrect but would not tell us what to use since that info is locked up with the nuclear codes. If something has changed, they never informed us. Anyone else with the same issue or any suggestion appreciated.

Response from thedoctorline.com: This issue was partly due to the confusion of the provider and incorrect information in the claim, both of which resulted in a front end rejection. The provider thought it was "their own" clearinghouse, but in fact it was CEDI=Common Electronic Data Interchange, which is the DME MAC clearinghouse which rejected the claim. Thus it was Medicare's own clearinghouse which was not at fault but actually where the front end rejection took place.
A front-end rejection indicates that Medicare cannot process a claim as is, due to a severe fault with the claim, resulting in the claim not being processable or appealable as submitted. Some examples, incorrect information in the Common Work File vs. what the claim contains. This could have included wrong DOB, name spelling vs. MBI, etc. Others include wrong "other insurance information". Reaching out to CGS ombudsmen, it turned out to be the latter. Dr. Goldstein was contacted and the claims were corrected and resubmitted and currently are processing. Additional coding and billing questions and responses can be found at thedoctorline.com

Diogenes is partially famous because he spent time carrying a lantern looking for an honest man. He is also called Diogenes the Cynic. A good 30 years ago, I shared some uninhibited conversation and a cup of coffee with Leonard Levy, DPM, the Dean of the Des Moines college. I was a couple of classes into pursuing my education doctorate and we talked about the significance of a doctorate in education as a podiatrist in academia. In a moment of astonishing frankness, Leonard said to me, “I could do so much better for myself if I only had an MPH from Columbia and not a DPM accompanying it.”
I had experienced a taste of the three-tiered medical caste system where MDs were on top, DOs made up the second class, then there were the rest of us who made up the third layer. It was impossible to “tier up” with a DPM degree. The graduation stole, as part of our academic...
