Bunions Aren't Just an Aesthetic Thing: NY Podiatrist
"A bunion develops because of a pressure imbalance in your foot, which makes your toe joint unstable," explains Yolanda Ragland, DPM, podiatrist and founder of Fix Your Feet. "The bones of your big toe begin to shift and angle toward your second toe. Constant pressure causes the head of your metatarsal (the bone at the base of your toe) to become irritated, and it gradually enlarges, forming a bump."

Bunions aren't just an aesthetic thing; they can also be uncomfortable and even super painful. "You may experience pain, swelling, and redness around the affected joint," says Dr. Ragland. "The skin may thicken and become callused, and your big toe may angle inward, which can bully the lesser toes, affecting them as well. The big toe may even overlap or tuck under your other toes, resulting in corns or calluses."

NY Podiatrist Discusses Benefits of Walking Barefoot
In general, wearing shoes is a good thing because they provide protection and support. But if you've been loving barefoot life, there is good news: it does have some health perks. "Without support from footwear, your feet work harder to maintain balance and stability, which essentially gives them a greater workout," says Bruce Pinker, DPM, a New York-based board-certified podiatrist and foot surgeon.

What's more, going barefoot outdoors — called "earthing" or "grounding" — in particular can even be used as a cathartic form of mindfulness, as it forces you to slow down and be more aware of your environment. "Many people will walk barefoot to be more connected to Mother Nature, and this connectedness can be therapeutic," says Dr. Pinker. Even science backs it up: Research has found that simply having direct contact with the Earth (via your feet, for example) can reduce the risk of heart problems, pain, and stress.

Simple Foot Test Detects Heart Rhythm Disorder in Diabetics
Atrial fibrillation can be detected during annual foot assessments in patients with diabetes, according to research presented at EHRA 2021, an online scientific congress of the European Society of Cardiology (ESC). "In our study, one in six patients with diabetes had previously undiagnosed atrial fibrillation," said study author Dr. Ilias Kanellos of the European University of Cyprus, Nicosia. "This presents an opportunity to provide treatment to prevent subsequent strokes."
Dr. Kanellos said: "The yearly check-up includes palpating the arteries in the feet to examine whether there is healthy blood flow. During this examination, we noticed that some patients had an irregular rhythm and decided to investigate its usefulness for diagnosing atrial fibrillation." Two podiatrists and six podologists were trained to spot rhythm abnormalities during pulse palpation of the foot arteries. They were also shown how to confirm their findings using a hand-held Doppler ultrasound device.

They should be contacting their insurance company as well. They will be talking to member services and sometimes they find a sympathetic ear who helps them get coverage or reveals the holy grail of coverage determination to them. They are often successful when we are not.

Having read the back/forth on the “melanoma/Mohs surgery emails, Dr. Markinson has “hit the nail on the head.” Too many times, as DPMs, we think of obtaining clinical or surgical skills “to do a new surgery” as purely a “technical skill” without the requisite base knowledge accumulated by standard training. I looked up the requirements for Mohs post-graduate training which is one of two years post dermatology/plastic surgery residency. As DPMs, we can’t matriculate to these fellowships because of our degree! Dr. Markinson is absolutely correct in that there is a pathway - MD/DO, dermatology/plastic surgery residency/Mohs fellowship.
Just obtaining the training is not a viable way to perform this technique. This is an issue of referring cases benefiting from Mohs surgery to the trained physician able to perform it, not DPMs!

Dr. Markinson might be confused about leaving the nail plate in place. Dr. Divya Srivastava and her colleagues use this technique for primary biopsy. MART-1 immunoperoxidase staining was used to define the melanocyte density in the specimens. I agree with Dr. Markinson on the multiple steps he describes towards getting the training and qualification to perform this surgery.
For me, the message is that just because the pathology involves the foot and ankle, we (most of us) are not qualified to perform the standard MMS technique. The implications determining the tumor is malignant are horrific. Identify a team capable of doing this surgery in your area and use it as a resource for referral.
