NY Podiatrist Discusses Home Treatment of Calluses
 
Vaseline is a super affordable way to soften callused skin. "It’s a sealant," explains Emily Splichal, DPM, a New York City-based podiatrist. "When you take a bath or shower, your skin absorbs all the water and moisture. Vaseline helps lock all that in so it stays within the skin." Apply it to a callus immediately after bathing, then cover the area with plastic wrap overnight to seal in moisture and nix rough skin.

To treat calluses and massage your feet at the same time, try using a pumice stone. "These are better than the blades they use in nail salons because the chance of cutting yourself is much lower," explains Dr. Splichal. In addition to applying topical treatments like creams or ointments nightly, Dr. Splichal encourages patients to use "mechanical debridement" tools like the Ped Egg foot file on calluses one to two times per week to remove coarse skin on the foot.

TN Podiatrist Joins Bone and Joint Clinic
 
There have been two important developments at West Tennessee Bone and Joint Clinic recently. Orthopedic surgeon Kyle Stephens is now performing hip arthroscopy in Paris and Martin, and podiatric surgeon David Long, DPM is now treating patients in Paris, Martin and Union City.

The right shoe will provide a comfortable haven for tired, achy, and swollen feet.

So what should you look for? Bruce Pinker, DPM, a podiatrist and foot surgeon with Progressive Foot Care, says an ideal pair will firmly cup the heel and offer sturdy arch support. The toe box should provide ample room, and the shank, or middle part of the shoe, should be somewhat flexible—not too rigid, but not too flimsy.

For one thing, at that time, the data about asset and loan balances is readily on hand. For another thing, look at your tax return as the income statement of your year and the net worth as your balance sheet. Is your net worth keeping pace the way your income has? Query: Billing a Post-operative Complication
 
I have a patient about 12 weeks post-bunionectomy. She had a deep suture that was irritating her. I anesthetized the area locally, scrubbed it and went in via a small incision and removed the suture. This was done in the office procedure room. She did well and I didn’t bill thinking it should be in the global period. I know that “return to operating room” for a complication should be covered but my office procedure room is not an OR so I did not bill. My front office is in disagreement with that decision.
 
Could this have been billed as a retained foreign body? If so, since it was actually a suture, what code would it be billed as? Then can it be billed as removal of foreign body? I figured this was just one of those things where you do what is right and don’t worry about billing. Am I wrong?

Response: The return procedure needs to go to a similar setting as the surgery, so in your case, the complication would have to go back to an OR. But then, is it “medically necessary and reasonable” to do so vs an in-office procedure? To me, this is just a post-op complication that you handled well and I would not have billed it either.

I'd like to expand a bit on the response provided by Dr. White. The issue of anti-kickback and Stark rules and regulations are far too complex to summarize into a few small paragraphs. There are attorneys who specialize in this area of law who can best provide a practice with a way to safely navigate these rules and regulations. Some of these rules may at first seemingly only apply to Medicare patients. When one digs deeper, you will find that many of these regulations universally apply to all patients no matter the revenue stream (i.e. third party payers vs. self-pay patients).
 
Any practice wishing to financially (or otherwise) incentivize an employee (professional or ancillary) directly related to income generated from services provided by the practice, needs to be extremely careful about how this is crafted. I am only beginning to scratch the surface of these rules and regulations. It is very apparent to me that the average healthcare attorney may not possess sufficient knowledge on these issues, which the experts in Stark and anti-kickback regulations may possess.
 
It is also important to remember that investigations of these violations are very costly and the potential financial fines for violating these rules are staggering. The very employee you are providing financially bonuses to, may actually be the star witness against you. It is thus absolutely essential that you seek out those who are best knowledgeable on these matters prior to incorporating any bonus system in their written or verbal contracts.

I am responding to the letter from Bill Beaton DPM which appeared on April 26, 2018 saying I recommended Crocs for medical professionals in a recent article in Prevention.com. In fact, I recommended Dansko clogs which have an APMA seal of approval. I did not mention Crocs in that interview. The article specifically says "Clogs are also a good choice, says Howard Friedman, DPM, a Suffern, New York-based podiatrist. They tend to have a wide toe box, which helps accommodate feet as they naturally expand throughout the day."
