MD Podiatrist Discusses DFUs
Dr. Brian Belgin says, “One to two percent of diabetics generally will have an ulceration. In some studies, between 20 and 40 percent of diabetics at some point in time will have an ulcer.” A diabetic foot ulcer is an open sore, usually located on the bottom of the foot. Belgin, a podiatrist with the GBMC Wound Care and Hyperbaric Medicine Center says foot ulcers often occur in diabetics because of the loss of sensation in their feet.

“They don't feel anything so they can pick skin off their feet, they can step on something, they can have something in their shoe and they don't know it and it rubs the skin off their foot and it develops an ulceration,” says Dr. Belgin. Be aware of redness or swelling and pain in the area among other symptoms. “Fever, chills, shortness of breath, spike in the blood sugar. Sometimes they just don’t feel well,” says Dr. Belgin.

FL Podiatrist Discusses Ankle Replacements and Fusions
Dr. Michael Black, a podiatrist with Lee Health, says the ankle joint is supposed to be the primary shock absorber for the foot during walking. “When we lose the shock absorber, we get the same amount of stress that then gets pushed forward underneath and above the ankle. That stress can cause ankle arthritis. “Ankle arthritis is a term that means inflammation of the ankle joint. The treatment historically for end-stage ankle arthritis has been ankle fusion. That’s a procedure in which the diseased cartilage and bone is removed from the ankle and screws are placed in the ankle to allow the leg bone to grow to the ankle bone,” said Dr. Black.

Patients typically need ankle fusions every five years. Another treatment option is ankle replacement surgery, which will treat the arthritis and preserve the motion of the ankle for an estimated ten years. “There has been some newer advancement in not only the surgical instrumentation, but in the design implants. That being said, we can position the ankle implant in line with the leg and make very accurate bone cuts that are reproducible,” said Dr. Black.

KY Podiatrist Appointed to State Podiatry Board
Gov. Matt Bevin has appointed Dr. William R. Adams II of Paducah to the State Board of Podiatry. He and Dr. Bradford Martin Fine of Lexington will serve on the board through June 30, 2020. The State Board of Podiatry consists of five gubernatorial appointees. The Board regulates and controls the licensing and conduct of podiatrists within Kentucky.

Dr. Adams earned his Doctor of Podiatric Medicine degree from Scholl College of Podiatric Medicine in Chicago. He continued his training with a Harris County Podiatric Surgical Residency at West Houston Medical Center in Texas. He is on staff at Western Baptist Hospital, Jackson Purchase Medical Center and Surgicare.

CA Podiatrist Wins Forty Under 40 Award
Kenneth Lopez, DPM has been awarded a Forty Under 40 Award. Dr. Lopez is the Assistant Physician-in-Chief of the Operating Rooms at Kaiser Santa Rosa. He is the Chief of the department of Foot and Ankle Surgery and the Department Technology Lead for the Foot and Ankle Surgery.

UK Footcare Initiative Scoops Top NHS Award
A diabetes footcare initiative in Exeter has won a top NHS award for lowering amputation rates below the national average. The Integrated Diabetic Footcare Faculty (IDFF), which was set up in 2012, scooped the NHS Health Education England Innovation Champion Star Award in March.

Sarah Clauson, who is a complex care and rheumatology podiatrist at the Royal Devon and Exeter Foundation Trust, said: “The reason given at the award ceremony was that we are a voluntary organization thought to have contributed to lowering the amputation rate in East Devon from 1.4 in 2012 to 0.6 in 2017, which is lower than the national average of 0.8.”

Healthcare Fraud 101: Under False Claims Act - Part 1
When you learn of a possible investigation, the first step is always to try to find out what exactly the government is investigating. Is it a coding practice? Billing for medically unnecessary procedures? The receipt of possible kickbacks for using a medical device or drug? Hiring outside counsel is critical; this is not the time to try to navigate the situation yourself. Using outside counsel will protect the confidentiality of many communications with employees and management. Most important, outside counsel can effectively contact the government and begin a discussion about the scope of the investigation.
You will also examine the government's request for documents carefully for clues about what is being investigated. For example, if the government asks about all of the lunches, dinners, or travel you have accepted from a specific medical device company, it is likely that kickbacks are at issue. Or if the government asks for the names of all patients who have visited your practice with a specific medical complaint and the codes for which you billed them, it is likely that some sort of billing practice is of concern.

Query: Coding for Trauma Wound with Hematoma
I have an 80 year old female non-diabetic patient with normal peripheral vascular status who was cleaning her oven and bumped into the open door causing bilateral skin tears of both legs. She also had an organized hematoma of the right leg at the margins of the wound. The only code for “hematoma” that I can find states “with skin intact”. The ICD-10 Alphabetic Index then suggests to look at “Contusion” by location. Even there, I could not find a code mentioning “hematoma” that did not mention “with skin intact”. Also, these wounds are traumatic not pressure or diabetic and they are not lacerations. Am I missing something or is it just ICD-10 overload and we just take our best guess?

Response: I recommend looking at “laceration” ICD-10 coding. You said “bilateral skin tears of both legs. She also had an organized hematoma of the right leg.” So it seems this was the initial encounter for the acute episode of care and you would select for both legs:
S81.811A laceration without foreign body, right lower leg
S81.812A laceration without foreign body, left lower leg
Additionally, you stated this is a hematoma which is the same as a contusion.

You are using injury codes, so make sure you document the date she injured her legs. It would be called the date of injury in your billing software.
