PA Podiatrist Discusses Ways to Reduce Heel Pain
Local residents may be relacing their walking or running shoes now that the weather has warmed. But with the fresh air and exercise could come heel pain, according to Manor Township podiatrist John Marty. "Heel pain is the most common foot problem in my practice," Marty said in a press release.

With an abundance of places to walk in the area, Marty said walkers should stretch their feet, buy new shoes and start out slowly before building up endurance. Walking on concrete surfaces can affect foot health, too, he said. "It's an overuse injury," he said. "I see it a ton this time of year."
"Wearing shoes that fit well, wearing the proper shoes for each activity, not wearing shoes with excessive or uneven worn heels or soles and stretching foot and ankle muscles properly before exercising are several main ways to avoid heel pain altogether," Marty said.

Lavery Presents 15th Annual Kaplan Memorial Lecture
One of the highlights of this year’s Great Lakes Conference was the Earl G. Kaplan, DPM Memorial Lecture presented by Lawrence Lavery, DPM. Lavery’s lecture focused on the question, “Does diabetes increase the risk of complications after ankle fractures?”

Great Lakes attendees were also taken down memory lane through a historical video presentation narrated by Irvin Kanat, DPM, which chronicled the lives and times of Dr. Earl Kaplan, the father of modern podiatric surgery. Kaplan was the first podiatrist inducted in the PM Podiatry Hall of Fame. Two of Kaplan’s three sons (all podiatrists), Randy and Gary were on hand for this emotional tribute.
Next year’s Great Lakes Conference will be held April 15-18, 2010.

Court Approves Doctors' Suit Over Ranking Program
Doctors in Massachusetts won round one in their legal fight to knock down a ranking program they say has misled patients while besmirching doctors' reputations. On March 30, a trial court green-lighted a lawsuit brought by the Massachusetts Medical Society and five doctors to halt a physician tiering program mandated in 2006 by the Group Insurance Commission, a state agency that oversees health insurance for public employees. Doctors also are suing two health plans participating in the program, called the Clinical Performance Improvement Initiative. It purports to improve quality and restrain costs by grading doctors' performance based on those factors.
Patients get an incentive, through discounted co-payments, to choose so-called better-performing doctors, who are ranked "Tier 1" out of three tiers. But physicians contend that the methodology used to rate them is faulty and has led to serious errors that have forced patients to pay more for their care and unfairly damaged physicians' credibility.

Query: Acroangiodermatitis (Pseudo Kaposi's) in Compromised Patient
I have a 80 y.o. sedentary IDDM, Hep C, HTN, CAD, chronic kidney dx, antiphospholipid dx, with chronic erosions of the posterior heels, R>L, which hurt upon standing. She has variable LE edema (increases with dependency) especially around the ankles. DP/PT 1/4 B/L. Generally, the skin is subtle, good texture/turgor. The heel has areas of pink to violet, dotted with superficial eschar, nodular, and friable especially the raised nodular areas which are slightly boggy, tender to palpation and bleed upon debridement/abrasion.

Skin biopsy shows "thick walled capillaries proliferate in the superficial dermis, intercellular edema and parakeratosis and epithelial hyperplasia." Diagnosis: stasis dermatitis, acrosyringeal hyperplasia, all consistent with acroangiodermatitis (pseudo Kaposi's). I have attempted edema control without success (patient doesn't tolerate compressive dressings and is non-compliant to elevation), topical applications of Psorcon E, Cultivate, Lactinol, Amerigel, ABX cream, off-loading, referrals, bedside debridements, etc. without resolution. A suggested course of action would be appreciated.
