Flatfeet Most Prone to Damage from Being Barefoot: NJ Podiatrist
"Our home-based lifestyle has caused a significant change in the shoes we wear," says Dana Canuso, DPM, a board-certified podiatrist and podiatric surgeon based in New Jersey. "Many of us have shifted from sneakers and boots to slippers and [being] barefoot, and this change significantly impacts many aspects of foot health."

While not all changes to footwear habits have been negative (i.e. Dr. Canuso notes more people are now inclined to wear sneakers all day so going for walks is more convenient), those wearing nothing but comfy footwear — or no footwear at all — could be building a foundation for future foot problems as a result. "Those with a pronatory or flat foot type are predisposed to more injury from not wearing shoes since they're already lacking the support needed to promote a neutral foot position," says Canuso.

Arch Support Running Shoes for Low and High Arched Feet: NY Podiatrist
Finding running shoes with arch support is especially useful for people with flat feet or high arches since they need extra support to prevent injuries like heel spurs, says podiatrist Brad Schaeffer, DPM, owner of Sole Podiatry in New York City.

Runners with high arches should consider arch supports, because they're more prone to issues like plantar fasciitis, in which the connective tissue that runs between their heels and toe bones becomes inflamed. Those with high arches also tend to oversupinate (a.k.a. excessively roll outward with each step), so a supportive shoe can help keep strides more neutral, Schaeffer says. That said, anyone who pounds pavement may want to consider arch-support running shoes, says Schaeffer.

OH Podiatrist Wants New Investigation into Congressional Baseball Shooting
An Ohio congressman is asking for a new investigation after he says the FBI determined a shooting at a congressional baseball practice was “suicide by cop.” U.S. Rep. Brad Wenstrup, DPM, a Cincinnati Republican, was on the field that day in 2017 when a gunman opened fire while a group of Republican lawmakers was practicing for a charity baseball game.

Dr. Wenstrup said he was later told by FBI agents that the shooting was classified as “a case of the attacker seeking suicide by cop” – a designation the lawmaker says “defies logic.” “Director, you want suicide by cop, you just pull a gun on a cop,” Wenstrup said to FBI Director Christopher Wray during a recent hearing. “It doesn’t take 136 rounds. It takes one bullet.”

Query: Lisfranc Amputation and Revision
On February 11, a patient has a transmetatarsal amputation. The patient is a non-compliant diabetic. The site deteriorates weeks after he leaves the hospital. On March 24, he was re-admitted for an infection at the amputation site. On March 26, the remaining 5 metatarsal stumps are removed, and the wound is kept open. I have searched all over Google. How would you recommend coding for the 2nd surgery? What is the code for removing the 5 remaining metatarsal stumps?

Response: From a purely description standpoint, it appears that you have converted your original transmetatarsal amputation into a Lisfranc amputation. I suggest considering CPT 28800 Amputation, foot; midtarsal (e.g, Chopart type procedure) in this scenario. Since you indicate that the first surgery was meant to be definitive, I would certainly look at adding modifier -78 (Unplanned Return to the Operating/Procedure Room by the Same Physician Following Initial Procedure for a Related Procedure During the Post-operative Period).

A question arose by a reader of PM News regarding a submission that I wrote for the April 23, 2020 edition on Mohs surgery. Currently, such procedures are performed by dermatologists and, in some cases, by plastic surgeons. However, podiatric physicians in all states are licensed to perform foot surgery with virtually no restrictions. I suggest that the manufacturer that makes the materials to perform Mohs surgery be contacted (e.g., Henry Schein Medical). Perhaps they may also have the resources to train or arrange for the training of DPMs interested in becoming proficient in this now commonly performed procedure.
The profession should make whatever arrangements are needed for DPMs to acquire the required skill set for this potentially invaluable procedure.

The Sacramento Regional Fellowship in Foot and Ankle Surgery
We are accepting applications for a two-year lower extremity surgical fellowship starting July 1, 2021 (pending timeline for applicant's California license). The scope of work will include reconstructive surgery and limb salvage techniques. Comprehensive wound care. Lower extremity peripheral nerve surgery and training in vascular medicine.
The fellowship includes the fundamental and advanced AENS peripheral nerve surgery cadaver courses, training in microsurgical techniques at the Mayo Clinic, and qualification for fellowship status in the Association for Extremity Nerve Surgeons upon completion. Applicants will need a California license. And have completed a CPME accredited three-year residency.

I am not sure if Dr. Levy's intention on paraphrasing the article on Mohs surgery was informational only or if he was recommending that podiatrists employ it. Clearly, Dr. Wolf interpreted it as the latter and asks how one could "get the training and qualification" to do it. I am also confused about his point on leaving the nail plate in place, but that point is moot when it comes to excising a melanoma for which surely a preliminary biopsy has been performed, and total excision is now the reason for further surgery. Mohs surgery is not at all new.
The short answer to Dr. Wolf's question involves multiple steps towards getting the training and qualification to perform this surgery. Step one: Graduate from an accredited MD/DO program Step two: Complete a ACGME-approved dermatology, surgery, or plastic surgery residency. Step three: Complete a certified dermatopathology fellowship since Mohs surgery requires...

RE: Don’t Give Ciox Access to Your EHR

Ciox is making it "easy" for podiatrists to submit requested medical records by allowing them remote access to your EHR system. “Out of the goodness of their hearts,” they are providing this service to eliminate the time-consuming burden podiatrists have sending Ciox requested medical records. I truly hope podiatrists do not partake in this.
A simple solution that takes less than 2-3 minutes would be to send Ciox an invoice for your service based on the number of charts requested times the amount per chart you wish to charge. Their chart reviews end up costing Medicare millions of dollars.
