PODIATRISTS AND RESEARCH
CA Podiatrist Receives $500K PhRMA Foundation Award for Research

The PhRMA Foundation has awarded $500,000 grants to David G. Armstrong, DPM, MD, PhD, of the University of Southern California and Nino Isakadze, MD, MHS, of Johns Hopkins University to conduct research using digital health technologies (DHTs) to improve health equity and health outcomes for patients.

Dr. David Armstrong

Drs. Armstrong and Isakadze were selected out of 7 researchers awarded $25,000 planning grants in 2023 by the Foundation to develop comprehensive research proposals to study the use of DHTs for advancing patient health, especially in underserved populations. Armstrong’s team seeks to improve patient outcomes with a new smart boot design that allows for remote monitoring of patient activity and adherence to the treatment.

PODIATRISTS AND YOGA
TX and CA Podiatrists Discuss the Benefits of Toe Yoga

Toe yoga refers to various stretches and exercises that are designed to strengthen, stretch, and improve the flexibility of your toes, feet, and the muscles around them, Nam Tran, DPM, podiatrist and foot surgeon at Dallas Direct Podiatry in Dallas, Texas, told Verywell. This form of yoga also consists of stretching routines that enhance foot strength and the circulation to your feet. Dr. Tran said that toe yoga is “used to promote overall foot health and wellness” through movements like raising the toes or just the big toe, curling or scrunching the toes, and heel raises.

(L-R) Drs. Nam Tran and Elizabeth Reilly

Toe yoga emphasizes the intrinsic muscles of the foot, which are very small muscles that begin and end in the foot, Elizabeth Reilly, DPM, a podiatrist at Stanford Medicine and a clinical assistant professor of orthopedic surgery at Stanford University, told Verywell. According to Drs. Tran and Reilly, performing toe yoga exercises and stretches can potentially help with foot strength, flexibility, and posture, as well as preventing common foot problems.

MEDICARE ADVANTAGE IN THE NEWS
Humana Plans to Leave Some Medicare Advantage Markets in 2025

Humana expects to exit Medicare Advantage (MA) markets in 2025, company executives told investors. The company reported its first quarter earnings April 24. Humana posted $741 million in net income in the first quarter of 2024, beating investor expectations, but pulled its 2025 earnings guidance.

On an April 24 call with investors, Humana executives said it will look to pull back benefits and exit some markets, as CMS continues phasing in risk adjustment changes. CMS published its final MA rate notice for 2025 earlier this month. The agency slightly cut benchmark payments and continued phasing in coding changes. Humana previously said the agency's rates were lower than its expectations. Other payers have signaled they will likely cut benefits to accommodate the rate notice.

PRACTICE MANAGEMENT TIP OF THE DAY
10 Suggestions to be an On-time Physician

Many of these ideas can easily be implemented in any office, with minimal or no expenses and without adding additional staff.

4. Avoid interruptions. There should be no interruptions when doctors are involved in patient care. I suggest creating a written policy of exceptions to this rule. Acceptable interruptions include calls from the emergency room, the intensive care unit, or the operating\recovery room. I suggest not taking calls from your stockbroker, insurance agent, spouse, or children.

Each physician must decide if you will take calls from a colleague or from referring physicians. I recommend that your receptionist tell the doctor who is calling you that you are with a patient, and you will interrupt them if it is an emergency. Otherwise, the doctor will return the call when he is between patients. Most doctors calling to speak to a colleague will accept this response and will not request that you be interrupted.

RESPONSES/COMMENTS (EMR)
From: Lawrence Kosova, DPM

The question you are asking is an excellent one and will be answered by the end users/doctors.

Just so you know, I have beta tested 5 products in the AI scribe world and I am on the medical advisory board for HeidiHealth. This is a free AI scribe with multispecialty language models including podiatry, so you can get your SOAP and H&P notes done efficiently. All the EMR companies I have talked with, including the large players like Epic and Athena, are beta testing solutions now.

These will be profit centers for the EMR companies. So whatever product you are currently using, you should be asking what EMR companies are they currently in discussions with for integration and what are their projected timelines? I am perfectly happy right now being about 98% finished with my chart note when I walk out of the patient room. I then spend a few minutes to review the chart note and...

Editor's note: Dr. Kosova's extended-length can be read here.

RESPONSES/COMMENTS (NON--CLINICAL) PART 1A
From: Jeff Root

Dr. Trantalis knows what many other successful podiatrists know. That is, biomechanics is a key component of having a clinically and financially successful practice. Dr. Trantalis states that the challenge is to educate the public to see a podiatrist knowledgeable in biomechanics to treat foot related pathology that manifests in other parts of the body, specifically in the lower back.

Ironically, a retail arch support company advertises heavily about the benefit of using shoe inserts to treat foot, knee, hip, and back pain. While this does increase public awareness about the potential cause and treatment of lower back pain, it's unfortunate that these "patients" are not being directed to podiatrists who, with knowledge of biomechanics, would be better suited to treat these foot-related conditions and other related or unrelated medical conditions.

RESPONSES/COMMENTS (NON--CLINICAL) PART 1B
FromHoward Bonenberger, DPM

Regarding biomechanics, its less training in podiatry school, and its absence in today's practice: Years ago, I happened upon a live lecture by Dr. Howard Dannenberg on functional hallux limitus (FHL). It was eye-opening and led to an awareness of how this common joint dysfunction contributes to pathology all the way up the kinetic chain.

With a simple, proper Rx orthotic modification, I had patients who found relief from their TMJ syndrome. I emphasized that, while I was not treating the TMJ, I was not surprised. There is a lot of compensatory lower extremity pathology caused by FHL. If you are uninformed about FHL, please do your patients a favor and watch some of Dr. Dannenberg's videos. It may be a "light bulb" moment for you.

RESPONSES COMMENTS (NON-CLINICAL) - PART 2
From: Joe Agostinelli, DPM

I have read with interest on the topic “Future of Podiatry“ and have to say I totally agree with Dr. Jacobs. Today, I read Dr. Saxena’s comment on this subject and have to say I am also impressed with what he has to say. His thesis for his post-graduate work and his podcast bring new concepts that I haven’t thought about before, especially on the length of time in obtaining medical degrees and subsequent residency training time. The lack of enough primary care doctors has resulted in many non-physician providers of care such as PAs and nurse practitioners, who by the way, provide comprehensive primary care and offer many services that doctors with more training time do.

In my military career, I can say I had some of the most complete and thorough annual physical examinations by PAs and flight medicine primary care providers. For military retirees, PAs, and nurse practitioners are vital in providing primary care services due to the military readiness and high operational tempos with the various world situations. Readers of this thread need to investigate Dr. Saxena’s podcast/thesis. You may change your present thinking on this subject. We have to be thinking “out of the box” on the subject of the future of podiatry, and this would be a good place to start thinking that way. I applaud Drs. Jacobs and Saxena for addressing this subject so thoughtfully and proactively!
