MI Podiatrist Discusses Chronic Wound Care
 
According to Conway McLean, DPM, "Wound Care has entered the space age and now requires more than simply applying a dressing. From ultra-sonically charged mist to the application of fish skin, from specialized lasers to the use of lab-grown skin, healing of these potentially deadly conditions has improved incredibly. Approximately 75% of all chronic wounds are venous ulcers, nearly always occurring below the knee. Another sizable category of non-healing skin ulcers occur to people with diabetes. These are typically found on the bottom or sides of the foot."

"Wounds that won’t heal because of inadequate arterial blood flow are called ischemic ulcers and are primarily found on the toes, foot, or lower leg. Closing these unhealing skin lesions means a drastically reduced risk of infection. And that means limbs are saved. If you have a skin injury that isn’t healing, or a family member with one, don’t minimize the potential consequences. Get specialized care and get it healed," says Dr. McLean.

ON THE INTERNATIONAL LECTURE CIRCUIT
OH Podiatrist Lectures at the South African Limb & Reconstruction Meeting 
 
Mark Mendeszoon, DPM lectured at the SALARM orthopedic surgery meeting that took place April 20-22, 2023 in Cape Town, South Africa. As the only international podiatrist on the panel, he presenting on a lecture on limb salvage and trauma surgical techniques. The meeting had 120 attendees from 40 nations.

Dr. Mendeszoon has also been appointed as an associate, visiting professor at Iowa State University in the biomedical science and kinesiology departments. In addition, he also has been appointed as a board member at Miami University’s Mallory-Wilson Center of Health Education and a faculty professor at the new Miami University Physician Assistants post-graduate program. Both appointments are the first at both institutions for a DPM.

Dungeons & Dragons Brings MT Podiatrist and Family Together
 
Jarret Isles is as instructor of all things Dungeons & Dragons. Erik Ploot, DPM is a podiatrist when not adventuring as a barbarian, into Isles’ classroom. He saw Dungeons & Dragons as a way to spend time with his adult children, all of them “Lord of the Rings” fans, he said. “I know they would totally get into it,” Ploot said after an early session.

Although he played in high school and middle school and dabbled in college, it has been a while, he admitted. He heard about the class from his wife and it seemed a way to ease back into it, Ploot said. Both she and his children have been encouraging, he said. "I’m glad she did, otherwise it’s so hard to even think about jumping into a game,” he said.

Eight Gifts that Violate the Anti-kickback Statute
 
Like the False Claims Act, the Anti-Kickback Statute (AKS) remains a frequent tool used by the Department of Justice to investigate the healthcare industry. Unlike the False Claims Act, the AKS imposes criminal penalties on violators. The leaders of a physicians' practice may be held liable for what others in the practice do, even if the leader did not know precisely what was going on. It has been called the "crime of doing nothing."
 
1. Providing free dinners or lunches to physicians
2. Travel expenses paid to physicians
3. Entering into consulting or research agreements with physicianns under which payments are made but minimal (or zero) work is done in return
4. Other gifts, such as electronics or tickets to sporting events

As you know, MIS bunion surgery is a hot topic, game-changing, and no longer the mark of an "inferior surgeon." You might particularly appreciate the parallels I draw between new MIS bunion repair to the history of laparoscopic surgery, which was once considered an "unethical procedure" and now is the global gold standard.

A total of 911 subjects were included, and 1088 MIS procedures were performed. The average follow-up was 23.8 months. All used the Shannon burr and performed distal metatarsal osteotomies which needed screw fixation. This review found a 16.6% complication rate with MIS hallux valgus correction, which is higher than that reported in the literature for open procedures of 7% to 8%. Many reasons for this increase in complication rate with MIS hallux valgus surgery can be considered. The hardware removal rate accounted for almost 40% of all complications as is likely due to screw prominence after swelling resolves given the entry point on the medial border of the first ray.

I graduated NYCPM in 1980. Back then, there was a huge divide between “closed” and “open” surgeons. I did lots of MIS bunionectomies with a Shannon #44 burr in those years. There was no question that the x-rays weren’t always pretty but the patients were happy. But applying MIS to certain levels of deformity was not satisfying. While I spent most of my surgical career doing mainly traditional open surgery, I always found MIS preferable for certain cases.
 
To be honest, this many decades later, it is surprising to me that there are still “evaluations” of MIS being reported. It has been around for a really long time. It is being performed all over the world. I say let each surgeon choose the technique that is most satisfying in his or her hands. If it did not produce positive outcomes, it would no longer be a surgical option for thousands of surgeons around the world.

At any rate, I will not be renewing my DEA license and will save the $1,000 which I will donate to a podiatry cause.
