VA Podiatrist Takes on Mt. Everest
As a podiatric surgeon, Pamela Jensen-Stanley, DPM, knows a lot about feet. In climbing Mt. Everest, Jensen-Stanley applied one old but important lesson: that of making progress by continuing to put one foot in front of the other at the earth’s highest mountain above sea level, standing more than 29,000 feet high. Jensen-Stanley didn’t go up quite that far — she made a trek to the base camp. That’s still a skyward journey of 5,364 meters, or 17,598 feet — more than three miles.

Dr. Pamela Jensen-Stanley |
“The mental challenge was probably the hardest part,” she said. “Physically, it was reasonable to do for anyone reasonably athletically inclined, but you have to test yourself for the long haul, to tolerate the cold, to be worn out and keep going. This has been a goal of mine for over 20 years,” said Dr. Jensen-Stanley, 52. “I promised myself this trip when I completed podiatry school.”

NY Podiatrist Deals With Different Kind of Stitches
In Park Slope, Brooklyn, a podiatrist has changed careers to offer a different kind of healing. "As a little girl, I did not say, I want to grow up and own a yarn shop. That was not how that worked at all. I actually wanted to grow up and be a doctor, and I was. I am. I'm a podiatrist, and I practiced for about 10 years," says Felicia Eve, DPM.

|
Dr. Felicia Eve |
"Having been a doctor for 10 years, you develop a fondness and attachment to people. And sometimes I feel like I should put a co-pay jar on the counter, because people come in and it sometimes turns into a full-on therapy session. Or they're like, Oh my God, you're a podiatrist? And the shoe comes off, and I'm like, we should really go in the back yard and talk about that," says Dr. Eve.

UK Podiatrist Dropped from NHS For Charging Patient to Cut Toenails
A Lancashire healthcare worker has been fired after she charged a patient for treatment before lying to the trust. Ceri Eccles, who worked as a podiatrist at the Morecambe Bay NHS Hospitals Trust, has been told she can no longer practice after she took £15 from a patient for services that should have been free. The hospital worker was found guilty of privately charging a patient for nail cutting services before lying on the NHS booking system to retrospectively cover her tracks back in 2018.

Ways to Play the Prior Authorization Game - Part 4
Prior authorizations (PAs) are a pain for physicians. Here are a few things you can do to improve your odds of winning.
4. Learn what is needed to get an approval.
One example is that with certain insurance companies, every time I order an MRI of the lower back, they want the patient to have had a plain x-ray first. Why? There is no evidence that x-rays are a good test to diagnose back problems. In fact, if I am looking for a herniated disc, a plain x-ray will not show it but rather an MRI is needed. I know this but sometimes the only way to get the patient to get the test is to do it anyway.

Query: Strep Agalactiae Nail Infection
A 61 year old male presented with symptoms starting last fall. The condition started as swelling around the nail. The nail becomes loose, with white purulence present. The skin sheds and has weeping. The remaining nail is very soft. In those affected digits, he is having significant arthritic symptoms. This started in a toe, then a finger, then a toe, etc. (There are 7 digits affected).

| Strep Agalactiae Nail Infection |
He has tried: oral and topical anti-fungal, antibiotics, soaking in apple cider vinegar, and in dilute bleach, and has tried oral and topical steroids. All treatments have alleviated some of the symptoms, but it is still progressing. Uric acid 6.7, ESR: 16, RF <10, CRP 3.1, CBC normal, ANA negative, fungal culture negative, cultured Strep agalactiae. Suggestions?

| RESPONSES/COMMENTS (RELEVANT RESEARCH) | |
From: Clifford Wolf, DPM
While Dr. Srivastava and her colleagues make this look easy, there are significant issues which need to be recognized before a team can start employing this technique. Training and experience of the Mohs micrographic surgery (MMS) staff in processing the nail unit tissue is paramount to produce the highest quality sections. Because of the unique anatomy of the nail unit, it is easy to create sections with either false negative or false positive findings. Additionally, for optimal histopathologic analysis, the operating surgeon would benefit from training and experience in leaving the nail plate in place, as opposed to avulsing it.
When at all possible, functional surgery for nail unit melanoma in situ is preferable over amputation to preserve digit functionality. The recurrence rate observed in this study was non-inferior to other published studies. How do podiatrists get the training and qualification to perform treatment and management of ungual melanoma in situ?
Clifford Wolf, DPM, Oceanside, CA
