The American Socierty of Foot and Ankle Dermatology (ASFAD) recently presented its President's Award to Joel Morse, DPM, honoring his over 11 years of dedicated service to podiatric dermatology education as a distinguished leader of the former American Society of Podiatric Dermatology (ASPD) and the new ASFAD.

Dr. Morse reflected, "This dermatolgy conference started in my living room over 11 years ago. Chuck Perry, DPM: When treating a sub 1st metatarsal, I have been removing sesamoids to get that area to heal. Is it better to just do a gastroc resection and leave the ulcer alone?

J. Monroe Laborde, MD: Yes, I have found that by removing the sesamoid under the 1st ray, the hole gets deeper and it takes longer to heal. I have found that even in the cases where the sesamoids are osteomyletic, as long as the bone is not fragmented on x-ray, there is no need to remove them. Just do the tendon balancing procedure and healing will occur. For all ulcers sub 1st metatarsal, I do a gastric resection and a lengthening of the perional longus tendon.

If Blue Cross pays you 150 percent of Medicare, they are going to have language relating to a fee schedule, David Wofford, senior manager for ECG Management Consultants in San Diego notes. "Is it 150 percent of the unadjusted rate? Or of what the provider gets based upon their performance? Then you have to determine which year they are talking about."
 
There is probably going to be a two-year lag between performance and the actual penalty or reward from Medicare. If you have a commercial contract tied to that percentage of Medicare, which time period are we looking at? It is going to require adjustment of some of that language, he adds.

Residents want to know what their salaries and benefits from a prospective employer should be. Prospective solo and small group podiatric practice employers want to know to what degree an applicant will "pay his or her own way" with anticipated future revenue generation. In my humble opinion, it is important that those training residents themselves understand and make sure residents also understand that aside from podiatric medical and surgical competence -- even excellence -- the new practitioners they will become if hired in most independent solo or group practices will have to ultimately reach revenue and profit goals. 
 
I hope residency directors are providing residents with the practice management information and training that will make it easier for their residents to secure a position in an existing independent solo or group podiatric practice. If a previous resident has knowledge of how to gain patient confidence by developing an affable doctor-patient relationship, and how to present foot care treatment options that are in the patient's best interests, how to accurately and compliantly document their services to maximize insurance reimbursement, how to reach out to other physicians in the area and successfully gain referrals, and how to adapt to the evolving value-based healthcare payment system -- I believe the resident will be in the best possible negotiating position with an existing practice to be hired and earn a very satisfactory salary and benefit package.

My first reaction is to ask who is posting this anonymous question and why? Is this yet another furtive attempt to curtail expectations for a fair and reasonable salary? The graduating podiatrist will have endured 7 years of long, hard training. Now burdened with student loans, to the absolute breaking point, the job seeking graduate despairs with lowly paid job offers, or sometimes no salary at all! A doctor of podiatric medicine and surgery ought to have a reasonable expectation that all the hard work, training, and expense has some merit, over and 
above a salary befitting a low to middle management position in Costco. Nurses are regularly paid six figures. But the doctor should not entertain such a lofty expectation? 
 
I respectfully ask only this: if there is no vacancy in your office, with a commensurate work load to provide an associate with a fair salary, then why are you...

I have followed Stephen Curry’s career since the time he took unheralded Davidson to the Final Four Championship. I have never examined his feet, nor have I ever even seen a weight-bearing radiograph. These top-tier athletes are very closed lipped when it comes to their injuries, so that makes my opinion/conjecture on par with Dr. Kirby’s. Curry has actually had two surgeries on his right foot, neither of which (based upon his limited recovery period) appears to have involved serious osseous procedures.
 
My conjecture is that he has a rear foot varus deformity, due to his easily inverted ankle sprains which plagued him before last season. He has performed incredibly throughout this regular season; therefore his movement, angulation, acceleration and deceleration indicate that remnant scar tissue was not an issue. Without knowing what his baseline diagnosis is, or what procedures were performed, Dr. Kirby’s conjecture falls flat...including his comment that it was the coach's decision.
