Paralyzed Midwestern Student Receives Encouragement from Lance Armstrong 
 
As cyclist Nate Aikele approached the final turn at The Driveway Series race on April 10, he looked over his shoulder to gauge his first-place lead. It wasn't the first time he was slated to win, but some fear this might have been his last. In an instant, Aikele veered off course, crashing into a tree. After extensive spinal reconstructive surgery, the doctor’s prognosis is a complete spinal cord tear resulting in paralysis from the middle of his chest down.

No one wants to hear that kind of news, especially an athletic 30-year-old who is seven weeks away from receiving his doctor of podiatric medicine. The returned missionary for The Church of Jesus Christ of Latter-day Saints doesn’t know if he can graduate this May from Midwestern University, or if he can complete his podiatric medicine and surgery residency at North Colorado Medical Center. Recently, he received a surprise visit from Lance Armstrong. Having defeated cancer, Armstrong offered advice: “In any situation — cancer, spinal injury, MS — I’m always an optimist. I always encourage these folks in these situations to just be positive, to think their ‘cure’ is around the corner — it’s just a matter of timing.”

WA Podiatrist Discusses Relationship of Poor Circulation to Neuropathy 
 
One problem many older adults have with their feet is poor circulation, but that’s a problem that can affect anyone at any age. “When you don’t have good blood flow in your feet, you might experience tingling, numbness, cramping, and discoloration of your skin and toenails. Several factors can restrict blood flow in your feet, including being outdoors in the cold or having your feet in cold water, shoes or stockings that are too tight, sitting too long with your legs crossed, smoking, and drinking too many caffeinated beverages," says Philip Yearian, DPM, a board-certified podiatric surgeon at Franciscan Orthopedic Associates.

People who suffer from diabetes need to be particularly sensitive to the circulation in their feet. “Reduced circulation can lead to a loss of feeling in the feet, a condition known as neuropathy. This can prevent diabetics from feeling small aches and pains that let us know we have been cut or bruised. Left untreated, these small problems can develop into serious conditions," says Dr. Yearian.

Recently, Dr. Bradley Lamm was invited to speak at the American Academy of Orthopedic Surgeons (AAOS ) meeting on ankle distraction for preservation of the ankle joint.

AZ Podiatrist Uses Low Level Laser for Plantar Fasciitis
 
Erchonia has announced that the U.S. Food and Drug Administration (FDA) has granted the company 510 (k) clearance to market Allay, its new low level laser for the relief of chronic heel pain from plantar fasciitis. Erchonia received FDA clearance based on a double-blind, randomized, multi-site and placebo-controlled clinical trial. In order to participate in the study, patients had to have self-reported pain of greater than 50 on a visual analog scale (VAS) of 0 to 100 and be unresponsive to conservative measures.

HIPAA UPDATE

Three Laptops Stolen from NY Podiatry Office

Nearly 6,500 patients of Sims and Associates Podiatry may have had personal information – including Social Security numbers – compromised after three laptops containing the patient data were stolen from the New York office. The theft occurred sometime between Jan. 10 and Jan. 12 and was discovered on Jan. 12 at around 9:30 a.m. The data for patients undergoing vascular testing was taken from 2007 to Jan. 2010. The data for patients who were prescribed orthotics was taken from mid-2012 to Jan. 2010. The data for patients who had x-rays taken was from 2004 to Jan. 2010.

“There has been no evidence of misuse of any affected person's information, and a review of procedures to enhance security and theft protection is ongoing,” according to the notification posted to the podiatry office’s website.

Query: Mobile Credit Card Apps and HIPAA

I would like to know the readers' experiences using apps such as four square for mobile credit card processing. Specifically, how can the use of such apps can be made to adhere to HIPAA privacy standards?

Response: PM News does not provide legal advice. The Office of Civil Rights — the federal agency that does most of the enforcing for HIPAA — stated in January of 2013 that HIPAA does not come into play when we charge clients’ credit cards for healthcare services. The HIPAA problem only arises if you were to text or e-mail a credit card receipt. Therefore, the safest thing to do is to supply a paper receipt.

I wanted to report the passing of Dr. Marvin L. Kravitz.  I took over for him in Havre de Grace, MD back in July of 2000.  He passed away on April 20, 2014 and was 83 years old.  He started his podiatry practice in 1956.

In Syracuse, NY, we treat Raynaud's patients from November through March. 
1. No tobacco products, including second hand smoke.
2. Hydrate and limit caffeinated products.
3. Closed, warm shoes or boots. Synthetic or wool socks, no cotton. Warm hat and gloves.
4. Prescription for Nitro-Bid ointment; apply 2 square cm to top of foot or over tarsal tunnel area, whichever works better for the patient. 
5. If no response, refer back to PCP for Nifedipine and/or vascular consult. 
 
Nine times out of ten, I can get the patients comfortable with #1-4.  I will occasionally write a prescription for Nifedipine, but if they are already on meds for hypertension, I refer them back to the PCP for that. I have tried nerve blocks with very limited success. I have recommended contrast baths which have helped a couple of patients. A few have gotten good relief by getting some cardiovascular exercise daily.

I've used oral Nefedipine, 10 mg t.i.d. or 30 mg extended release, and there are other calcium channel blockers that would have similar effect. I have used Nitroglycerin ointment, although it can have a systemic effect as well. And there are various formulations by compounding pharmacies that use Nifedipine 1-10%, Pentoxyphilline 8%, among other ingredients. There are other more involved, expensive, and invasive remedies that I have no experience with. Of course, I tell them to not smoke under any circumstance, to wear socks in cooler weather, and ultimately, if all else fails, move close to the equator, if necessary.
