The not-for-profit Lower Extremity Amputation Prevention (LEAP) Alliance today announced it has added a new member to its advisory council roster. Alejandro Jimenez-Baeza has been named Podiatry Student Representative. Jimenez-Baeza had been active in exemplary community health care related work before starting his podiatric education. He is currently in his first year at the Scholl College of Podiatric Medicine

Currently, the LEAP advisory council members are helping to develop value-based optimal standards of podiatric care that are expected to be implemented by the time present students begin to practice. They believe giving students access to information about conforming to optimal standards of care early on in their studies will better equip them to help patients and conform to insurance medical necessity requirements.

CA Podiatrist Discusses Bunion Surgery
More than 25% of people worldwide have bunions—and the deformity, which is particularly prevalent among older people, is more likely to affect women than men. But you wouldn’t know how common bunions are based on the level of discourse surrounding them; they’re not exactly considered dinner-party conversation. That should change, experts say. “It’s not talked about a lot…but it is an insecurity that patients have,” says Dana Brems, DPM, a podiatric foot and ankle surgery specialist in Los Angeles. “People say they feel uncomfortable wearing open-toed shoes or sandals because of the appearance of the bunion.”

More discussion about bunions, including how they affect quality of life and what to do about them, could help alleviate that stigma, she adds. People can generally walk immediately after bunion surgery, but they need to wear a special post-surgery shoe, like a boot, for at least a few weeks. “That’s just unavoidable—solid bone takes six to eight weeks to heal,” Dr. Brem says. “You’ll probably be back to full activity after around three months, running and jumping and that sort of thing.”

FTC Votes 3-2 to Ban Non-Compete Agreements, but Legal Challenges Expected
The Federal Trade Commission (FTC) voted 3-2 Tuesday to issue a final rule striking new non-compete agreements for all workers and phasing out existing non-competes for all but senior executives across “most employers.” The ban does not apply to non-profits including many of the country’s healthcare provider organizations due to the limitations of the FTC’s jurisdiction, one of several points of contention that has been raised by hospital industry groups that have opposed the ban.
The final rule will take effect 120 days after its publication in the Federal Register. To be in compliance, impacted employers will need to stop enforcing existing non-competes with workers other than senior executives, inform those who are no longer bound by existing noncompetes and stop initiating new non-competes for all workers going forward, FTC staff said during an open meeting on the final rule held Tuesday afternoon.

10 Suggestions to be an On-time Physician
Many of these ideas can easily be implemented in any office, with minimal or no expenses and without adding additional staff.
3. The doctor must start on time. Although this is consistent with adopting the philosophy in point number 1, this is the implementation process and having the doctor walk the talk. If the patients are to be seen at 9:00, the doctor must arrive before that start time and be ready to commence with patient care at 9:00. There is a better time to look at emails or return phone calls. If the doctor doesn’t arrive until 9:30, you can be sure delays and interruptions will occur.
As a result, patients who have later appointments will be seen 45-60 minutes after their designated appointment times. This leads to anxiety and frustration among the staff and lowers patient satisfaction and the doctor’s online reputation. You might also inform patients that you are trying to be on time and request that they arrive 5-10 minutes before their appointment. BTW, this only works if the doctor is on time.

I recently started using a very competent remote scribe and I am very thankful and grateful after making this decision. I feel like I can continue working solo, see more patients, and not bother hiring another doc - or get sidetracked with “burnout”! I am wondering what other pods are doing regarding current EMR.

The commentary of Dr. Kornfeld with regard to the need for profitability in maintaining practice survival is appreciated. He has long represented a particular view on non-insurance based patient care which has been successful for himself and others. Medicine, including podiatry, differs from other "ordinary businesses". The practice of medicine is a calling, with a duty to care for patients as the foundation upon which all other business concerns emanate.
The practice of medicine is based upon the four cornerstones of medical ethics: beneficence, non-maleficence, autonomy, and justice. "For-profit first" thinking is the reason we are burdened with pre-authorizations, insurance payment reductions for services rendered, and denials for services. "Money first" thinking denies access to...

Dr. Jacobs points out a great website Kevin MD and changes needed in U.S. medical education in general. My MPH thesis was on redesign of U.S. medical education which I have posted on my website, and I was asked to do a podcast on Kevin MD on "Bias in Medicine". The cost of U.S. medical school contrasted with primary care salaries is a good reason to consider incentives to take on primary care which only 20% of U.S. MD/DOs choose compared to 50% of foreign doctors. More access to primary care improves many economic metrics including GDP.
Currently, almost 1/3 of U.S. medical schools are "accelerated", either three years of medical school or 6-7 years of both undergrad and medical school. With NPs and PTs now receiving doctorates in seven years, I fear the motivation to shortening MD/DO education may be threatened. The AMA holds the length of their training as one of the reasons for them to be called physicians and surgeons but not others. The book The Social Transformation of American Medicineby Paul Starr gave me huge insight...podiatry was not mentioned once in the 500+ page book.

RE: Educating the Medical and Lay Communities About Biomechanics

After practicing for 43 year and having a busy sports medicine practice in Federal Way, WA as well as later on working for a back surgeon in Florida, I learned that the importance of biomechanics is not just the foot and ankle, but includes the lower back. At the University of Washington, I got to experience the complete patient not just the foot and ankles. The difficulty is getting these patients to walk into a podiatry office.
Also, how do you teach back surgeons in medical school not to do surgery on the lower back. After all, that is what they are taught.
For example, limb-length difference with or without pelvic rotation can be acquired as one becomes older. I tried talking to a few medical schools about this and received no interest. Biomechanics can provide a window into many functional pathologies to the lower extremities. The challenge is how to educate the public to see a podiatrist knowledgeable in biomechanics.
