US and UK Podiatrists Discuss Functional Rehabilitation
Ben Pearl, DPM discussed functional rehabilitation with UK podiatrist Nick Knight on Pearl’s podcast Fit Foot U. Knight mentioned that white noise interferes with our ability to perform heavy load rehabilitation exercises. Dr. Pearl provided an example of compensation with the iliotibial band, which can interfere with glute strengthening. Knight said patients often tell themselves they cannot lift a heavy weight during a strengthening workout, even though they have done nearly the same weight in everyday life tasks like lifting up children.

Knight and Pearl discussed that even small improvements in range of motion and reduced swelling can trigger the nervous system to decrease pain and help the body get back to normal.

MIS Using a Shannon Burr For HAV Surgery: A Systematic Review
Recently, there has been an increase in minimally invasive surgery (MIS) for the correction of hallux valgus deformity. This systematic review aims to evaluate and present the current literature on MIS hallux valgus correction in studies reporting the use of the Shannon burr with distal metatarsal osteotomies to help establish evidence-based guidelines for surgeons using this technique. A total of 17 studies were reviewed. A total of 911 subjects were included, and 1088 MIS procedures were performed. The average follow-up was 23.8 months.
Conclusion: MIS for the treatment of hallux valgus using the Shannon burr appears to be a safe and effective therapy, with appropriate correction of the hallux valgus deformity, improvement in functional outcomes, high patient satisfaction, low recurrence, and acceptable complication rates.

Physicians Who Accept Medicare, Medicaid at All-time Low of 65%
Reduced Medicare and Medicaid payments are having more physicians considering reducing those patient bases, according to Medscape's "Physician Compensation Report" for 2023. Sixty-five percent of physicians surveyed said they would continue treating current Medicare or Medicaid patients and take on new ones, according to the report. Medscape said it is the lowest percentage it has seen in its annual compensation reports. Five years ago, 71 percent of physicians said they would continue treating current Medicare or Medicaid patients and take on new ones.
For the report, Medscape collected responses from 10,011 physicians across more than 29 specialties. The data was collected between Oct. 7, 2022, and Jan. 17, 2023. Eight percent of physicians surveyed said they would not take on new Medicare patients, and 5 percent said they would not take new Medicaid patients. Four percent said they will stop treating some or all of their current Medicare patients and will not take on new ones, and 3 percent said the same about Medicaid patients. Twenty-two percent said they have not yet decided how they will move forward regarding Medicare and Medicaid patients, according to the report.

5 Steps to Start Your Own Independent Practice
The key to launching a successful independent practice is proper planning. Knowing what steps to take and what mistakes to avoid is critical to ensuring a sustainable, profitable practice. Follow these five steps to get started down the right path:
5. Find an appropriate location and space - The adage is true: location IS everything. Be sure to conduct a thorough assessment of your market to determine if a potential location has the patient population you need to sustain your practice for the long-term.
Review population trends, community growth, and new construction permits along with existing or planned locations of other private practices and hospitals. Don’t choose a location based on nostalgia for a neighborhood or what appears to be a bustling location. Do your research and base your decision on facts. Also spend time researching what kind of space will work best for your practice - and your wallet. Finding a walk-in ready medical space or one that only needs a few cosmetic upgrades is a great way to keep costs down, as a full build-out can be very costly, time-consuming, and expensive.

Query: ABNs and CPT 11750
I have a question concerning Medicare’s rules when billing for repeat CPT 11750. What is the panel’s recommendation for billing these for a regrowth following a previous CPT 11750? In another scenario, on the same subject, what if the procedure needs to be performed on a nail border that is adjacent to one that was already billed? Can and should we use an ABN and upon denial, bill the patient?

Response: My suggestion is to file an appeal which supports a repeat procedure (e.g. opposite border, neoplasm, regrowth despite good technique in the past, trauma, etc.). Even the references used in the Surgical Nails LCD cite a 10% recurrence. Because of this, an ABN is inappropriate as repeat matrixectomies are allowed. They are subject to certain requirements which, in my opinion, most patients would meet.
Now to the exciting news, just the other day, Novitas and First Coast announced that use of the KX modifier in addition to the T modifier, would bypass the edit and allow payment. Perhaps this is the first step in doing away with this egregious edit. However, use of the KX modifier, may lead to a post-payment audit and possible recoupment if your documentation fails to support the need for a repeat matrixectomy.

I have been following this issue from a distance and I can understand both sides of this issue, as it’s apparent both sides have reasonable positions. Please correct me if I am wrong, but this appears to be an access issue to qualify to even take the entry level exam for each board. Couldn’t a reasonable compromise be to allow any podiatrist the opportunity to qualify for certification, regardless of the residency program that they did.
We have to acknowledge that our past is full of uneven opportunities for the same residencies and in some cases, none at all. Just my opinion, but you can either pass the test and the subsequent certification process, or you can’t. Both boards have set term limits and residency requirements to gain entry to the process. Why not just let competence determine one’s success for each board and move away from “rebuilding the wheel” and causing divisions among colleagues?

In response to the coding questions of Dr. Warshaw’s post about “neuroma coding” which is really cutting out a nerve that is entrapped, not a true neuroma. I would like to add that perhaps instead of cutting it out and risking a stump neuroma, perhaps learning and offering something more advanced and likely a better option would be better. Many surgeons falsely believe that the success rate of a neurectomy is over 90% but the literature does NOT support this and the many patients I treat every week from all over the country with stump neuromas would say otherwise.
When given a choice of A= no chance of a stump neuroma vs. B= somewhere between 10-30% of a stump (which is much worse with life-altering pain than the original nerve entrapment), why would anyone choose B? I doubt many of my colleagues are telling patients that the chance of a stump is at least 10% which is not a small number. I am proud to say in the last 20 years, I have never caused but have treated many. If anyone is interested, I and Dr. Steve Barrett on his Pod of Inquiry podcast discuss Morton’s entrapment and our experience which emulates the experience of the many AENS members.
