PRACTICE MANAGEMENT TIP OF THE DAY
Overall Tax Strategies for Physicians - Part 6

Now that the tax-filing deadlline is here, the die is cast on last year's revenues, refunds, and amounts owed. However, smart planning throughout the year and effective management of expenses and deductions can leave physicians and medical practices better equipped for next year's filing season. Here are some tips that can help physicians maximize revenues and sail smoothly through tax season into next year.

6. Consider the New Tax Law  - "Make sure you discuss with your accountant the changes that the new tax law will bring to 2018. Many of our clients are doctors who always paid the alternative minimum tax, but due to changes in that tax... [they] will no longer pay it in 2018 and their taxes are going down. The biggest decreases we see are for [physicians] making between $250,000 and $500,000...." - Mark Dietrich, CPA, member, National Society of Healthcare Business Consultants.

CODINGLINE CORNER
Query: Hospital Initial visit/consultations and Residents

If one of my residents sees a consultation at the hospital, contacts the attending, in this case it's me at 7 PM on any given day. The very next morning, I go and see the patient some 12 hours later: Am I allowed to bill the consultation from the evening prior?

Additionally, what type of procedures can a resident do on a consultation in the hospital or in a in emergency room when I am not present (remove an ingrown toenail, removal of foreign body, sew a laceration, or apply a cast, etc.)?

Jeffrey Klein, DPM, Waterford, MI

Response: No, you cannot bill for the consult from the prior evening if you did not see the patient that evening. You cannot bill for a hospital encounter if you were not there, even if you spoke with the resident on the phone and advised the resident. And even if you sign the resident’s note the next day.

As far as your second question, what the resident can and cannot do is totally up to state and hospital regulations/bylaws. This can vary from state to state and definitely vary between different hospital systems. This is a question for the medical staff office at your hospital. Also, it's not a bad idea to run this question past the malpractice carrier for both you and your residents.

QUERIES (DME)
Query: Employee Incentive Bonuses for Therapeutic Shoe Program

I have had a diabetic shoe program in my office for years. One of my staff members has been in charge of this program and is well versed on the rules and the paperwork that are necessary to run a successful shoe program. However, our shoe numbers have been down lately and I would like to start a reward program for this employee based on the number of shoes that are dispensed as an incentive to be more productive. Does anyone have suggestions for a bonus system for improving our diabetic shoe program?

RESPONSES/COMMENTS (CLINICAL)
From: Keith Sklar, DPM

I had a previous patient with similar clinical findings. The patient was 12 years old. Both he and his parents were educated on what I believed to be a fungal condition. The patient was treated with PinPointe laser as well as oral terbinafine, and topical anti-fungals. The patient and his parents were reminded about the importance of parent/patient compliance.

Pre- and post-treatment photos

I  rarely remove a nail plate. I’ve improved so many dystrophic nails very simply. As I tell all my patients, it just takes time, patient compliance, and good physician care.
