Raymond R. Diprimio, DPM
 
Dr. Raymond R. DiPrimio, 85, of Washington Township, a South Philadelphia podiatrist and one of the foremost lower-extremity anatomists in the country, died on April 14 of heart failure. Dr. DiPrimio, a former professor at Temple University and one of the founders of the Pennsylvania College of Podiatric Medicine, taught more than 4,000 students over 50 years.

Over the years, many of his classes recognized him as "professor of the year." Dr. John Mattiacci, dean of Temple's podiatric school and one of Dr. DiPrimio's students, called him an outstanding teacher who was beloved by faculty and became like a grandfather to his students. "To know him was to love him," said Mattiacci, a personal friend.

CA Podiatrist Comments on Star Basketball Player's Ankle Injury
 
Golden State Warriors’ guard Stephen Curry has been dominating sports headlines this week – or rather, his right ankle has. After helping his team set the record for regular-season wins, the NBA’s reigning Most Valuable Player twisted his ankle in the first game of the playoffs – a win against the Houston Rockets. He was pulled from the second half of that game and – to the surprise of many – every contest since. Dr. Kevin Kirby, a Sacramento podiatrist who has been treating foot and ankle injuries in athletes for more than 30 years, said this is “not an unusual situation.”

“We’d call it a tweak,” he said of Curry’s problem. He could have pinched the joint capsule or tore some scar tissue from the prior surgery. In those cases it’s not a major sprain, but it can take a week or two (to heal).” Curry underwent an MRI Tuesday that did not show anything concerning, according to ESPN. Kirby said coaches are making the right decision by keeping the player out and giving the ankle time to heal.

Nearly 1,000 clinicians from throughout the region joined an international faculty to "wage war on wounds". Dr. David G. Armstrong, Professor of Surgery and Director of the University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA), provided one of the keynote lectures for the event.

"When one in four people on the Arabian peninsula have diabetes and the most common reason they will end up hospitalized is for a foot infection -- one that is often painless and silent initially -- it's time to sound the alarm," noted Armstrong.

Pilot Program Results Show Promise for New CMS Payment Model
 
The CMS last week announced it is expanding the four-year pilot launched in October 2012. The federal government will engage public and private payers and about 20,000 providers in 20 regions. Just as before, providers will be encouraged to identify and work with patients who had multiple conditions that led to frequent doctor visits and hospitalizations. The clinics will be given monthly care-management fees for each beneficiary in addition to regular fee-for-service payments. The monthly amount will come whether or not a patient interacted with the practice that month.
 
The practices will need to revamp their delivery care models to focus on five areas: access to and continuity of care; planned care for preventive and chronic needs; risk-stratified care management; engagement of patients and their caregivers; and coordination of care with patients' other providers. In a New England Journal of Medicine article, Mathematica Policy Research evaluated the initiative's first two years and found modest but impressive improvements. The pilot has not yet shown net cost savings, but reduced spending enough to almost cover the monthly care-management fees. These results should be interpreted with caution but are “promising and more favorable than might be expected,” according to the report.

Four Consequences That Will Derive From MIPS - Part 3

3. Physician compensation and service agreements will need to evolve. It is expected that MIPS will be driven by outcomes-oriented measures. Wofford explains that if you look at the care process that needs to occur in order to influence those metrics, there is interplay between many different entities. For instance, there is discharge planning, care coordination, and making the clinical data available across inpatient and outpatient settings. All of that activity is what is going to keep people out of the hospital and keep your re-admission rates low, but how much of that is actually performed by the doctor?
 
"Doctors will tell you they have very little to do with that," says David Wofford, senior manager for ECG Management Consultants in San Diego. "So you have to figure out to what degree you want to place them at risk for these things. Do you put them at risk for the outcome? Or do you try to distill it down to the specific behavior they can influence that plays a part in all of that? That is going to be a real challenge to figure out."

We are wondering what residents are being told by their residency directors and residency attendings concerning an average starting salary out of residency in private practice. Is there a specific hourly rate/yearly salary that they are being told to expect? Is it also the standard to provide health insurance? What are they getting in reality when they get out and start working? Any feedback would be appreciated.

There must be some way to inform these applicants about podiatric medicine. I think we could increase the number of applicants to our schools if the dental applicants could be informed about what our profession has to offer!
