CA Podiatrist Explains How He Named the Cinderella Procedure

When Dr. Ali Sadrieh, a podiatrist, started Evo Advanced Foot Surgery in Beverly Hills, CA, 13 years ago, he thought it seemed a little vain for women to ask for surgery because their feet hurt wearing fashionable shoes. “Patients would bring in shoes they dreamed of wearing,” he said over tea recently at the St. Regis New York, where he was staying to see New York patients. “On the surface, it looked shallow. But I came to see she needs these shoes to project confidence, they are part of her outside skin. That’s the real world.”

For Dr. Sadrieh (who was wearing made-to-order Gucci brogues), foot surgery is a fusion of medicine and fairy tale. At his practice, you don’t have a bunionectomy; you have a Cinderella procedure. “I had never met a patient who asked for a hallux valgus correction with osteotomy and screw fixation,” he said. “So I decided to create a name that captures the result of the procedure, without all the Latin. The point of the Cinderella: being able to put a shoe on that didn’t fit comfortably before.”

AT THE COLLEGES
Canadian Podiatric Medical Students Complete Clinical Rotation at NYCPM
 
Eight senior podiatric medical students from the Université du Québec à Trois-Rivières (UQTR) recently completed a four-month clinical rotation at the New York College of Podiatric Medicine’s (NYCPM) clinical affiliate, Foot Center of New York, as well as in three different departments at hospitals affiliated with the college.

The clinical rotation is part of UQTR’s program in podiatric medicine, which was founded in late 2003 through the efforts of an NYCPM graduate, Dr. François Allart, in cooperation with the faculty and administration at NYCPM. The UQTR program draws on a number of resources at NYCPM, and is the first DPM program in Canada, as well as the first French-speaking DPM program in the world.

RIPMA Spearheads Sock Collection Drive for the Needy
 
Podiatrists throughout the state, along with YMCAs, the Pawtucket Red Sox, and The Salvation Army, are taking part in Socks for the Needy, an effort to collect new socks for needy children and adults. Collection bins are set up in the lobbies at YMCAs throughout Rhode Island, as well as at the podiatry offices of members of the Rhode Island Podiatric Medical Association (RIPMA) through May 30.

"Podiatrists have long recognized the high incidence of foot conditions among the needy and homeless as their feet are often wet and they lack dry clean socks to wear," said Dr. David Ruggiero, president of the RIPMA. "Our association thought a community outreach campaign like this was the perfect fit for podiatrists as we enter our anniversary year providing foot health for Rhode Islanders for over a hundred years. We give particular thanks to the YMCAs in Rhode Island, the Salvation Army, and the Pawtucket Red Sox for participating as partners in our community service project."

UK Podiatrist Denies Sexually Assaulting Patient
 
A podiatrist accused of sexually assaulting a patient by touching her thighs and hips, while cutting her toenails, has denied doing anything wrong. Yousef Kalami told a jury that his examination of the woman had been medical and not sexual in any way. The 59-year-old, of Chandlers Reach, Llantwit Fardre, said he had trained to degree-level at UWIC in Cardiff and the University of Glamorgan – and his treatments were “all in the manual”. The patient, who is in her 60s and from Pontypridd, alleged she was told to lie down when she usually sat in a chair for the chiropodist.

QUERIES (CLINICAL)
Query: Raynaud's Disease Treatment
 
My patient is a 57 year old Caucasian female who related an acute onset of pain on 2/25/14 in the second and fourth toes of her left foot. There is no history of trauma and no underlying medical condition other than having been diagnosed with Raynaud's disease. Both toes are distally mildly bluish-red with no sign of infection. Radiographs are negative for any bony abnormalities. Any thoughts about Raynaud's treatments that may be effective? A few women patients of mine responded to Diabetiderm, but I have no other thoughts since quinine is no longer available.

Here's  another way to  deal with a patient's complaint that "Your Fees are Too High"
 
First, clarify what the patient is saying. “Your fees are too high” could mean any of the following: It’s not worth it. It’s more than my neighbor paid for the same thing. It’s more than I can afford. It’s more than I thought it would be.
 
Each of these interpretations requires an entirely different response. All that may be needed to clarify the situation is a  simple question such as: “What makes you say that?  or “Why do you say that?”

RE: Payment for Diabetic Therapeutic Shoes (Paul Kesselman, DPM)

Coverage for therapeutic shoes for eligible diabetic Medicare recipients was enacted by Congress in 1998. It is the law of the land. Failure of Medicare carriers to comply violates that law, plain and simple. Of course, we realize that agencies charged with creating regulations and compliance with such laws have an enormous amount of latitude, largely extra-Constitutional, in the application of laws. Dr. Kesselman, as always, has performed an exemplary analysis of the issues concerning the current state of diabetic shoe coverage in his recent post.
 
Agency regulations that involve a de facto reversal of a law duly passed by both houses of Congress and signed into law by the President require a vigorous legal challenge. I am not sure where APMA is on this issue, but such a challenge should be performed with the assistance of various interested parties such as the American Diabetes Association.

The Helping Ensure Life- and Limb-Saving Access to Podiatric Physicians (HELLPP) Act, which we have repeatedly asked our members to support by contacting their Congressman and Senators, includes provisions to clarify and reduce some of the burdensome requirements of the therapeutic shoe program.

Paul Kesselman has asked great questions. First, let me give you the most important piece of information regarding the express agreement included here. 
 
I sent CMS a copy of the agreement and asked CMS to cite any Medicare rules or regulations preventing use of the agreement. If the folks at CMS could have cited a rule prohibiting use of the agreement, CMS would have. That didn’t happen. Instead, CMS sent me a long list of citations to the Social Security Act (SSA) and the Code of Federal Regulations (CFR). I did the attorney thing and very carefully read every one of the referenced sections. My legal opinion: There isn’t..
