Orthotics Can Prevent Knee Replacement in Some Cases of Flatfoot: MI Podiatrist
According to Conway McLean, DPM, "One of the more common consequences of the flatter foot type is the performance of many knee replacement procedures, secondary to walking with a collapsed arch. Experts believe most of these operations could have been prevented if, years earlier, these individuals had utilized prescription foot supports (e.g. foot orthotics) which help to maintain a more efficient, less deviated position of the many joints, bones, tendons, and ligaments, which function inappropriately in the presence of a flatfoot deformity."
|
Dr. Conway McLean |
"Clearly, there can be significant consequences to the flat foot type. Although many Americans have no real problems living with a lower arch, who can predict the occurrence of these many complications? Familial history provides only tendencies. A physician trained in the nuances of lower extremity mechanics is a good place to start. With a thorough and multi-faceted evaluation, many of these painful conditions can be prevented," says Dr. McLean.

PODIATRISTS AND SPORTS MEDICINE |
OH Podiatrist Stresses Importance of Keeping Athletic Shoes Clean and Dry
Anyone who has ever put their nose inside a gym shoe knows that our shoes can get a little foul after heavy wear. That’s because of the biology of our feet, and the nature of footwear, says podiatrist Mark Mendeszoon. DPM. “Your feet have the most sweat glands of any part of the body."
|
Dr. Mark Mendeszoon |
Learning how to clean sneakers isn’t only about keeping them smelling—and looking—fresh, says Dr. Mendeszoon. “It prolongs the longevity of the shoe.” Dr. Mendeszoon says if you work out in the same sweaty shoes every single day without taking care of them, you could potentially start to develop rashes or other skin problems within just a week or two. If your feet get particularly drenched when you work out, Dr. Mendeszoon suggests pulling out the insoles and powdering them with cornstarch, baby powder, foot balm, or Gold Bond — whatever you want to use to promote a dry environment, he says.

PRACTICE MANAGEMENT TIP OF THE DAY |
10 Suggestions to be an On-time Physician
Many of these ideas can easily be implemented in any office, with minimal or no expenses and without adding additional staff.
2. Have everything you need ready. This includes equipment, supplies, pathology, lab reports, and imaging studies before the doctor sees the patient. Doctors can’t be efficient if they don’t have everything they need at the point of contact with the patient. We all know about the delays that can take place if the doctor plans to do a procedure and doesn’t have the right instruments to accomplish the procedure. The same applies if the doctor needs the results of a CT scan or a pathology report to discuss with the patient regarding their medical care.
All these issues can be resolved using checklists placed on the chart or in the EMR. These checklists can be reviewed the day before the patients’ appointments, and any missing reports or equipment can be obtained before the patient is placed in the exam room. There is no downtime when the doctor is eyeball-to-eyeball with the patient.

RESPONSES.COMMENTS (NON-CLINICAL) - PART 1A |
From: Elliot Udell, DPM
Thank you Dr. Jacobs for bringing us highlights from an article in KevinMD. One of the important questions that you raised is why won't our national associations or local societies advertise to the public and promote podiatry. When I served on the board of trustees of our county society, that question was strongly asked by all of the board members. The answer we got is that while it seems like a good idea, effective advertising on radio and television is more costly than meets the eye. It would cost millions of dollars to do justice to it.
The next problem is where would this money be spent? Should it be in big cities and hence alienate dues-paying members in suburban and country locations? One possible solution advanced to us was to consider forming a group with local podiatrists, collect funds from each, and then promote advertising at the local geographic area. It was a good idea but in my locality, the idea never got off of the ground. Has this been successful in any other part of the country?

RESPONSES.COMMENTS (NON-CLINICAL) - PART 1B |
From: David Secord, DPM
In an arena of increasing knowledge base and technocracy, the expansion of the knowledge base expected by tomorrow’s patient also increases as does that patient’s expectation that you are not simply seeing a wallet to pick clean upon their visit. As allopathic physicians and surgeons, I believe that our destiny is to either follow the osteopathic school of medicine (adopt the allopathic standards of education and testing for licensure) or fade into oblivion.
I’m unclear as to how you trim a year off medical school and keep the same level of education in the world of expanding knowledge and expectations. As such, I believe that our future is a four-year degree, followed by a year of internal medicine internship and five years of surgical exposure to hone skills and knowledge and a...

RESPONSES COMMENTS (NON-CLINICAL) - PART 1C |
From: Robert Kornfeld, DPM
While I agree with almost everything Dr. Jacobs stated in his post regarding the future of podiatry, there are some issues that I think need to be re-visited. No doubt, the APMA and affiliated state societies have done a very poor job getting the public to understand what it is we do and how well trained we are to do what we do. I also believe they completely missed the boat in advocating for podiatrists. In a HUGE way. And I say this after 42 years in my own private practice (the past few decades as a non-member).
The “business” of medicine should not rely on insurance issues. Those issues have been created by insurance companies in order to exploit and abuse doctors for their gain. Likewise, private equity corporations employ doctors and still, like insurance companies, have a profit motive, not a healthcare motive. They, too, will exploit and abuse their...
