PODIATRISTS IN THE NEWS
Avoid Prolonged Foot Soaking: MD Podiatrist

Dr. Jacob Wynes

According to Jacob Wynes, DPM, assistant professor in the department of orthopaedics at the University of Maryland School of Medicine & Foot and Ankle Surgery, the simple act of soaking your feet can lead to fluctuations in moisture balance as well as fissuring, which is a known cause of cellulitis or deep-skin infection, especially in patients who are prone to leg swelling. He recommends avoiding prolonged soaking, particularly if you are prone to dry skin.

PODIATRISTS AND FOOTWEAR
NY Podiatrists Discuss Footwear for Medical Professionals

When your profession involves anything from running alongside a gurney to standing for hours in the ER or operating room, the right shoes are a must. “You now see all kinds of medical professionals on hospital floors wearing running shoes that are designed to stand up to a lot of stress and strain,” says Karen Langone, DPM, a podiatrist based in Southampton, New York. Clogs are also a good choice, says Howard Friedman, DPM, a Suffern, New York-based podiatrist. They tend to have a wide toe box, which helps accommodate feet as they naturally expand throughout the day. This is why you often see surgeons and other health professionals wearing them in the operating room, Friedman says.

Drs. Karen Langone and Howard Friedman

To help keep pain at bay, you’ll want to pick up at least two pairs of supportive sneakers or clogs. (Or one of each!) A shoe's cushioning becomes completely compressed after six hours or so and no longer offers support, Friedman cautions. So having two great pairs allows you to change halfway through your shift.

PRACTICE MANAGEMENT TIP OF THE DAY
Overall Tax Strategies for Physicians - Part 5

Now that the tax-filing deadlline is here, the die is cast on last year's revenues, refunds, and amounts owed. However, smart planning throughout the year and effective management of expenses and deductions can leave physicians and medical practices better equipped for next year's filing season. Here are tips that can help physicians maximize revenues and sail smoothly through tax season into next year.

5. Think Long-term - "Don't get carried away about tax savings. Remember, it's about wealth accumulation. For example, you may write off higher amounts sooner on an auto lease, but...does that make financial sense? Your equipment supplier may come to you at the end of the year trying to get you to buy a new piece of equipment to save taxes, but if there's minimal return on investment and it becomes an overpriced doorstop, it is not a good choice."  - Dawn Renner, CPA, member, National Society of Healthcare Business Consultants

RESPONSES/COMMENTS (CLINICAL)

From: Jeffrey Kass, DPM

I would like to thank Dr. Shavelson for sharing with the readers the condition of "disappearing nail bed (DNB)". It was a condition I was previously unaware of. Having said that, I do have concerns regarding some of his statements. 1) He states for this condition that the etiology is not primarily infection. In the study he linked us to, I don't see where it stated that. I did see that 36% of DNB in toenails did indeed have fungus, 17% had nail trauma, and 18% a history of prior surgery.

2) Shavelson continues that "continuing to treat as fungal or saprophytic is 'reckless'." If a nail biopsy or culture reveals fungus or mold, then it would NOT be reckless to include this in your treatment paradigm. 3) Shavelson continues to point out the great toe is especially long and parenthetically mentions shoe etiology. The original posting revealed the child was born with the problem.
