OBITUARIES |
John W Pagliano, DPM
On April 20, 2012, Dr. John W Pagliano sucumbed to cancer. Pagliano was a past president of the American Academy of Podiatric Sports Medicine and adjunct instructor at the Califirnia College of Podiatric Medicine. He was the author of numerous articles in publications such as The Physician in Sports Medicine, Ameican Medical Joggers Association, Medicine and Sport Science, Runner's World, and Podiatry Management.

Pagliano was an accomplished runner, having competed in over 500 races including 86 marathons and six 50-mile races. He was the USA-AAU National champion at 50 miles and the STC Athlete of the Year in 1969.

Barry Professor Elected President of AAWC
Barry University’s School of Podiatric Medicine professor Dr. Robert J. Snyder, a podiatric physician and surgeon for more than 30 years, was elected president of the Association for the Advancement of Wound Care (AAWC) Friday, April 20. Snyder, a leading wound care specialist and professor and director of clinical research at Barry will serve as president of the AAWC for the 2012-2014 term.

Snyder has a private practice at University Hospital in Tamarac, Florida, where he is director of the Wound Healing Center, serves on the editorial advisory boards of Ostomy Wound Management, WOUNDS, and Podiatry Manangement, and reviews articles for the New England Journal of Medicine.

LA Podiatrist a Featured Speaker at Chinese Foot & Ankle Society Meeting
Dr. Rita Rae Fontenot was the featured speaker at the annual meeting of the Chinese Foot & Ankle Society. This year, the meeting took place in Uremqi, a large, modern city on the Old Silk Road in the far northwest part of China. Dr Fontenot presented her paper on "Autograph Replacement of the Tibialis Anterior Tendon- A Case Study" which was published in the Chinese Journal of Orthopedics.

Dr. Fontenot was the only non-Chinese at the meeting of over 300 orthopedists. She has been invited to speak to the Foot & Ankle Society numerous times since she first visited China in 1989. She has watched Chinese medicine intergrate Western theory and techniques over the years, and has personally witnessed the modernization of their hospitals and equipment, especially in the discipline of foot surgery.

AZ Podiatrist Keynotes Canadian Pedorthics Symposium
Dr. David G. Armstrong, Professor of Surgery and Director of the Southern Arizona Limb Salvage Alliance (SALSA) at the University of Arizona, was this year's keynote speaker at the annual Pedorthics Canada Symposium in Whistler, British Columbia.

"Joining 300 colleagues that share our passion for interdisciplinary care is an inspiration," noted Armstrong. The event was punctuated by the group's release of their clinical practice guidelines, a culmination of the work of more than 100 people across several years.

SD Podiatrist Wins Governor's Vision Award for Inventing OTC Orthotics
One-fourth of all people have flat feet, and Spearfish (SD) podiatrist Dr. Lorri Riley has invented a product that, she says, can provide them with prescription-quality orthotic support at the price of an over-the-counter product. Riley and her product won second place last week in a statewide entrepreneurship contest, the Governor’s Giant Vision Award. She will put her prize money of $5,000 toward marketing her J1 Insoles, which are already available in Rapid City at The Runner’s Shop, in Spearfish at Walgreens and Sole Perfection, and in Belle Fourche at Lynn’s Dakotamart.

Riley said she developed the insoles after feeling frustrated that she wasn’t really helping her patients when she wrote them prescriptions for orthotics knowing they couldn’t afford the $300 to $800 the shoe inserts normally cost. With fewer insurance companies covering the cost of orthotics, Riley looked to develop a product that would support the typical flat foot, but could be sold over-the-counter.

Douglas Stoker: I don’t think you can make an absolute standard statement like that. There is a whole range of diabetic patients. Some are much more brittle than others. There are others that can go 3 to 4 months between visits. The maintenance of the at-risk types of patients is regulated by Medicare guidelines, which state that you can’t see them any more frequently and be paid and more frequently than every 60 days.

My office received a fax from a company identifying itself as Healthcare Alliance. The fax message asks for permission to send their prescription discount cards to the office to be dispensed to patients who have difficulty paying for their prescriptions. I surmise that the patient would then fill out the cards providing their protected healthcare information and mail them to this company. Does anyone have experience with Healthcare Alliance?

RE: Sterilizing Bits Between Debridements (Elliot Udell, DPM)

With all do respect to Dr. Udell's protocol of not burring nails after debridement, he brings up an excellent point concerning the hazards that apply to patients, assistants, and doctors concerning the development of pulmonary problems associated with the nail burring. In addition, he talks about OSHA regulations. To the best of my knowledge, there are no OSHA regulations pertaining to the inhalation of nail dust by podiatric assistants.
We as physicians are forced to implement strict OSHA regulations, some of which are just plain silly, but when it comes to protecting our staff from breathing in nail dust, I have not seen any OSHA regulations that require assistants to even wear a mask. I have used nail dust extractors in my office for over 26 years and am proud to report no respiratory problems with myself or my staff. I have had the same two assistants for over 17 years without their having any respiratory medical history. Using an effective cold sterile solution for the burrs (I use Control 3) and a nail dust extractor, your patients and staff will be happy and healthy.

RE: Article by Jeffrey Toobin (Bret Ribotsky, DPM)

Dr. Ribotsky made a reference in his recent letter about Jeffrey Toobin, (a writer for The New Yorker Magazine), who was interviewing a New York City attorney. In that rather nasty article was the discussion of various illegal shenanigans some podiatrists were being charged with. It is sad that we have podiatrists who get into trouble. But even that attorney may someday have a parent in a nursing home who needs a podiatrist and instead ends up with a third-world aide (who speaks no English) who snips his Mom’s toe in half while trying to trim her nails.
That article stands in my mind as the most vicious one I have ever read about us. That said, I would ask the simple question: “Who is calling the kettle black?”

RE: OH Podiatrist Discusses Celebrity's Neuroma

What's missing from the story is that, in a recent interview, Steven Tyler revealed the crossover toe deformity was actually the result of a neuroma removal surgery performed by his orthopedic surgeon, Brian McKeon. From Medical News: What's Wrong with Steven Tyler's feet (Besides the toenail polish), "In 2008, Tyler underwent surgery on his feet 'to correct long-time foot injuries resulting from his trademark athletic performance onstage.' He had been diagnosed with a condition called Morton's neuroma, and was told by his orthopedic surgeon, Dr. Brian McKeon, that it would require a number of surgeries to repair...The 'foot repair' pain was intense, greater than I'd anticipated. The months of rehabilitative care and the painful strain of physical therapy were traumatic. I really needed a safe environment to recuperate where I could shut off my phone and get back on my feet."
