Limb Preservation Program Saves Over 1,000 Limbs: FL Podiatrist
HCA Florida Aventura Hospital, named by Healthgrades among America’s 50 Best Hospitals for Vascular Surgery and America’s 50 Best Hospitals for Surgical Care, recognized April as Amputation Prevention and Limb Loss Awareness Month by shining a spotlight on its Limb Preservation Program. To date, the program has saved over 1,000 limbs via a collaborative approach which frequently required endovascular revascularization procedures to improve perfusion and avoid amputation.

“Early diagnosis and treatment via coordinated care from a multispecialty team like ours is key to preventing amputations,” shares Marie Williams, DPM podiatrist and chair, department of podiatry at HCA Florida Aventura Hospital. Furthermore, she adds, “Persons with diabetes are at increased risk for major amputations due to foot infections.” Other risk factors for CLTI include smoking, high blood pressure, high cholesterol, obesity, and a family history of PAD.

OH Podiatrist Discusses Preventing Ankle Re-Sprains
Joe Lewis, DPM, a podiatrist at Trinity Health System, recently discussed some treatments for ankle sprains and how to prevent sprains from re-occurring. According to Dr. Lewis, "A lot of it is physical therapy based. We do a lot of what are called balance and proprioceptive exercises that really retrain the muscles and nerves to prevent the abnormal rotation and to stabilize the ankle."

"Overall conditioning is a big part of it as well. Most of the injuries we see in sports come from fatigue. As the game goes on or the practice is prolonged, and the athlete becomes more tired and fatigued, now they’re more prone to break down in their technique, and to become injured. So, the more we can really condition and develop that overall fitness, we can help prevent fatigue and subsequent injuries," said Lewis.

RE: Office Visit Co-Pays: Specialists vs. PCPs (CA Blue Shield)

In an outdated method to financially punish their insureds and place yet another barrier to specialist care, most health insurance companies on many of their plans require higher office visit co-pays when a patient is seen by a specialist. Why should an insured patient be required to pay more out-of-pocket, for the "luxury" of care provided by a specialist doctor, who everyone knows is more qualified to treat their condition, when specialized care is medically indicated and necessary?
Most PPO insurance plans have different office co-pay requirements depending on if the patient is seen by a PCP doctor or a specialist. The office visit co-pay is almost always higher when the patient is seen by a specialist. The co-pay amount, due and paid by the patient, is then subtracted from the contracted allowable amount when the insurance company processes the claim and makes payment to the doctor.
As an example, Aetna, Anthem Blue Cross, Cigna, HealthNet, UnitedHealthCare all consider a podiatry visit as a specialist visit for the purpose of charging and collecting the co-pay amount shown on the ID card or on the company website. However, Blue Shield for some unknown reason continues to process podiatry claims as if the patient was seen by a PCP doctor and not a specialist. Why doesn’t Blue Shield classify podiatrists as specialists when it comes to claims processing, including the required collection of co-pays as a specialist? Podiatrists are certainly not PCPs. How can we fix this?

The increasing cost of wound care, including the employment of skin graft substitutes, is not a podiatry centric problem. Many factors, such as an aging population, the rise in disorders with which wounds are associated (e.g.: diabetes, PAD, venous disease) have continued to drive the need for wound care. The failure of insurers to provide reasonable or in fact any payment for preventive care is a factor. Socioeconomic issues such as patient access, patient education, patient financial concerns are factors.
The expense associated with dressings, skin graft substitutes, de-facto referral of patient referrals to wound care centers, contribute to the problem. With specific reference to skin graft substitutes, Dr. Geistler notes in PM News that in his experience, skin graft substitutes are not required for the management of most wounds. There are over 350 "skin graft substitutes" available world-wide. Many are extraordinarily expensive, with little...
