NY Podiatrist Explains Why Shoes Cause Odors
You slipped them out of your shoes and got a rather unfortunate whiff: Your feet smell bad. So you put your shoes in the closet, backed away slowly, and hoped that tomorrow the situation might be better. "The odor in feet is caused by sweat or moisture 'fermenting,'" Jacqueline Sutera, DPM, board-certified podiatrist and foot surgeon in New York City and New Jersey, tells LIVESTRONG.com. Stink is the result of "any combination of bacteria, fungus, mold, and/or yeast," she says.
|
|
Dr. Jacqueline Sutera |
"Odor-causing germs thrive in dark, damp spaces, which makes your socks and shoes ideal environments," Dr. Sutera says. One specific type of bacteria, brevibacterium, is responsible for foot odor caused by wearing tight or constricting shoes. And guess what? It makes your feet smell like cheese.

Shocking Images of UK NH Residents' Feet As Podiatrists Locked Out For Over a Year
A picture of a Scots pensioner's feet in a ‘disgraceful’ condition has sparked reaction over Covid care home restrictions. Care Home Relatives Scotland, a group launched to bring together people who have loved ones in nursing and residential homes, went on social media by sharing the image and claiming podiatrists have not been inside for more than a year.

Monica Lennon and nursing home resident's foot |
The graphic image of an unknown resident's feet, at an unknown care home, claims to show the results of the elderly going without the service. Monica Lennon MSP, the Scottish Labour Secretary for Economy, Jobs and Fair Work re-tweeted the picture and said it shows "too many residents are not getting the right care." Lennon wrote: “Behind the closed doors of care homes, there are still too many residents who are not getting the right care."

CVC Applauds Bipartisan Bill to Prevent Amputations and Increase PAD Screening
The CardioVascular Coalition (CVC) – a coalition of physicians, care providers, advocates, and manufacturers working to improve awareness and prevention of peripheral artery disease (PAD) – today applauded lawmakers in Congress for re-introducing the Amputation Reduction and Compassion (ARC) Act (H.R. 2631), bipartisan legislation to improve peripheral artery disease (PAD) education, increase access to PAD screening, and reduce lower limb amputations.
The bill would provide coverage of PAD screening for at-risk beneficiaries under the Medicare and Medicaid programs without the imposition of cost-sharing requirements, thereby ensuring early detection and intervention to prevent amputation.

Clinical Factors Impacting Clear Margins of Primary Melanoma in Situ
In this retrospective cohort study, the overall clearance rate was 87.2% for initial excision among 313 patients with melanoma in situ (MIS) treated with primary surgical excision. The clearance rate varied by lesion site, with head and neck MIS achieving a lower clearance rate than trunk and extremity (80.0% vs 95.3%; P < .001). On multivariate analysis, head and neck MIS were 73% less likely to have clear margins. Unsurprisingly, excisions with narrow margins due to anatomic constraints had lower clearance rates, and larger MIS lesions were associated with decreased clearance rates. For every 1-cm increase in lesion diameter, the rate of clear margins decreased by 33% (P = .037).
Head and neck location, decreased surgical margin size, and increased lesion diameter were associated with a lower likelihood of negative margins following primary excision of MIS.

Ways to Play the Prior Authorization Game - Part 3
Prior authorizations (PAs) are a pain for physicians. Here are a few things you can do to improve your odds of winning.
3. If something is abnormal, stop and document.
Jot a quick note to yourself or type in the abnormal exam into the computer. It only takes a moment and improves the likelihood that you will accurately capture the exam.

RE: Treatment For Ungual Melanoma in Situ
From: Leonard A. Levy, DPM, MPH
In American Academy of Dermatology Insight and Inquiries, Adam Rubin, MD reports (April 21, 2021) about a recent dermatologic surgery manuscript “Nail Unit Melanoma In Situ Treated with Mohs Micrographic Surgery” by Dr. Divya Srivastava and her colleagues at the University of Texas Southwestern Medical Center who demonstrated the effectiveness of the combination of Mohs micrographic surgery (MMS) and the application of MART-1 staining in the treatment of ungual melanoma in situ (MIS). Given lower recurrence rates and high patient satisfaction with digit sparing surgery, it is hoped that additional data will support the further use of MMS for nail unit MIS.
|
|
Ungual Melanoma in Situ |
The surgical treatment for nail unit melanoma in situ (MIS) has evolved over the last several decades with a trend towards tissue sparing and conservative management. While nail unit melanoma was traditionally treated with distal or proximal amputation, studies show that digit- sparing surgery with either en bloc excision or Mohs micrographic surgery (MMS) yields lower recurrence rates.

Query: What do Podiatrists Keep in Crash Carts?
I am curious about what podiatrists keep in their clinical “crash cart.” I also wonder how many podiatrists have AEDs in their clinics. What would be a sensible approach to this type of emergency care and preparation?
