NY Podiatrist Discusses Athlete's Foot

Athlete’s foot is a common contagious skin infection caused by a fungal infection that produces an itchy rash on the toes and feet. According to Johanna Youner, DPM, a board-certified podiatric surgeon and a Fellow of the American College of Foot and Ankle Surgeons, "The root cause of athlete’s foot is moisture, so the most important step in preventing it is keeping both feet and shoes dry. Always completely dry your feet after bathing, and be sure to get rid of moisture between toes. Powders such as corn starch, baking soda, and antifungals all are effective at keeping shoes and feet dry. Vary the shoes you wear, and let them dry completely before you wear them again."

Dr. Johanna Youner

"To treat an athlete’s foot infection, apply Lamisil, Lotrimin, or miconazole to the affected areas twice a day for up to two weeks. Homemade remedies include soaking your feet in the astringent Domeboro to dry the feet and prevent fungal infections…and applying essential oils such as eucalyptus, lavender, or tea tree oil to eliminate an active infection," says Dr. Youner.

PODIATRISTS IN THE COMMUNITY - PART 1
NY Podiatrist Opens Mobile Foot Clinic

Pamela Sparks, DPM  has had her shoe in the podiatry field for more than a decade, but she's stepping out on her own to tackle a new feat. The Syracuse woman has started her own mobile foot care concierge, SPARKS Podiatry, serving clients in Cayuga and Onondaga counties. She has always wanted to start her own business, and this endeavor, she said, is bringing back the old days when physicians made house calls.

Dr. Sparks is certified in foot surgery through the American Board of Foot and Ankle Surgery and is a 2003 graduate of the New York College of Podiatric Medicine. Prior to starting SPARKS Podiatry, she worked at the Auburn branch of Podiatry Services CNY. That position ended in November, she said, giving her the opportunity to dip her toe into a new kind of client pool.

PRACTICE MANAGEMENT TIP OF THE DAY
Overall Tax Strategies for Physicians - Part 4

Now that the tax-filing deadlline is here, the die is cast on last year's revenues, refunds, and amounts owed. However, smart planning throughout the year and effective management of expenses and deductions can leave physicians and medical practices better equipped for next year's filing season. Here are some tips that can help physicians maximize revenues and sail smoothly through tax season into next year.

4. Maximize Deductions - "Although higher-income doctors cannot benefit from the new qualified income deduction available to S-Corporation owners, partners in partnerships or limited liabilility corporations, sole proprietors, married doctors with incomes under $415,000 and especially those under $315,00, can benefit. The levels are lower for singles. You want to start planning now if you are eligible. - Mark Dietrich, CPA, member, National Society of Healthcare Business Consultants

RESPONSES/COMMENTS (CLINICAL)
From: Paul T Slowik, DPM

Referral to a dermatologist would be my first course of action. Pending that consultation result, I would consider a total simple avulsion with regrowth expected. Although we are generally taught to the contrary, I've seen many nails grow back much improved without any medicinal intervention.

RESPONSES/COMMENTS (EHR)
From: Stefan Feldman, DPM, Arden Smith, DPM

We switched from Dox about 6 years ago and recommend against using them for a variety of reasons. Much better experience with Traknet.

Stefan Feldman, DPM, Westlake Village, CA

I have used DOX EMR for about 6 years now. This system is podiatry-specific. It has every diagnosis and treatment we use in podiatry pre-programmed into the system from the start. Drs./users do not need to build any templates – building templates and entering details we need in podiatry can be frustrating and very time-consuming. None of that is required with DOX – they have done all that work for you. The system is ready for use from day one. It is easy to learn with a few nuances, but very intuitive, very user-friendly, and very little typing is involved.

As with any new system, there is a learning curve but it should be a relatively short time before you are able to go through a complete note very quickly. DOX is extremely good in their handling of Meaningful Use/ MIPS – with only a little extra work selecting PQRS measures. The training is done by the owner, a podiatrist, Bart Ripperger, himself, and he and his tech support team are always available and respond very quickly. Dr. Ripperger personally guided my staff through our Meaningful Use, PQRS, MACRA, MIPS submissions. It is a very good system and competitive in price to the other full-service and specific systems – I highly recommend it.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Dennis Shavelson, DPM

Greg Sands, OP, & Ortho-Rite have been the sole fabrication subcontractor for FootHelpers Lab for decades. He accepts the cost and high maintenance standards needed to produce foot centering orthotics. That enables me to offer an alternative to subtalar joint neutral cast devices where needed. During the recent shutdown, I made the mistake of sending casts to two other labs that assured me they could produce RestorThotics to my standards. The 75%+ return rate that ensued proved more destructive than if I had shut down and waited for Greg to rebuild.

My devices dispense by conforming to two tests. They must “integrate” to optimal casting position and they must activate the 1st ray rockers when underfoot. The replacement devices were too wide, too long, under-vaulted, and poorly posted. LLD lifts were not applied professionally and many made 1st ray activation worse, not better.

Although some patients and clients accepted the makeshift devices without complaint, I have re-dispensed more than half of the back-up devices with an Ortho-Rite product in order to re-establish my reputation. I have stated many times that Greg is the best production administrator of a custom foot orthotic lab. This was a pricey way to find out. I'm so glad to have you back.
