Risks Outweigh Benefits of Walking Barefoot: MD Podiatrist
What do foot doctors think about the barefoot lifestyle? “I’ll just be honest, my feet don’t hit the floor barefoot, even in the house,” John Roy Senatore, DPM, a podiatrist at MedStar Health in Baltimore, Maryland, tells TODAY.com. “Because all I see are injuries, I am a big proponent of good shoes and supportive ones, too.”

“During COVID, when people weren’t working and they were at home … I saw more issues with plantar fasciitis, muscle strains on the foot than any time in my practice,” Dr. Senatore recalls. “Just because people were at home not wearing shoes.” People with diabetes should never walk barefoot, even around the house, because of diabetic neuropathy, a complication that can cause a loss of sensation in their feet, Senatore warns.

Cushioned Running Shoes Aren't for Everyone: CA Podiatrist
“It goes without saying we need shoes to protect our feet but how much cushioning do we need? It really depends on the individual,” functional podiatrist Tea Nguyen, DPM told The Epoch Times. A study published in Scientific Reports found that cushioned shoes increase leg stiffness and amplify impact loading in runners. Faster runners, in particular, tended to have a stiffer leg landing compared to runners in conventional shoes.

Dr. Nguyen said minimal shoes aren’t for everyone—but you wouldn’t know that from marketing, which tends to push the extremes: either ultra-cushioned shoes or barefoot ones with zero support. Most people have a cushioning preference and know what works for their body’s comfort best, said Nguyen. However, being proactive about strengthening your foot muscles is a good approach even if you don’t intend to give up cushiony shoes.

According to Holly Spohn-Gross, DPM, podiatrist and president of the California Podiatric Medical Association, the best shoes to manage plantar fasciitis are ones that will support the arch of the foot and reduce pressure on the inflamed tendon — granted you know which arch morphology, or type, you have.

“If the patient has a low arch or flat foot, I usually recommend a motion control or stabilizing shoe,” Dr. Spohn-Gross said. “If the patient has an average arch, then I lean towards a neutral shoe. Finally, if the patient has a high arch, then I will recommend a neutral or cushioned shoe.”

Malnutrition-Related Diabetes Officially Recognized as ‘Type 5’
Malnutrition-related diabetes has been officially recognized as 'type 5 diabetes' by the International Diabetes Federation during the World Diabetes Congress in Bangkok. This condition, primarily affecting young men in low- and middle-income countries, is often misdiagnosed as type 1 diabetes. Unlike type 1, patients do not develop ketonuria or ketosis, despite high blood glucose levels.
Recent studies, including one involving 73 Asian Indian men, reveal that type 5 diabetes is characterized by a significant defect in insulin secretion rather than insulin resistance. This recognition aims to improve diagnosis and treatment, as excessive insulin can be fatal. Current suggestions include minimal insulin use with oral agents and dietary adjustments. A working group is developing formal diagnostic and therapeutic guidelines for diagnosis and treatment over the next 2 years. The Takeaway: Malnutrition-related diabetes, now classified as type 5, requires distinct treatment to avoid harm from insulin overuse.

I agree with Dr. Geistler. I think grafts can be very helpful, but see them being used in place of standard, well-supported wound care treatments when they are probably not necessary.

I too am a seasoned provider and all too often encounter some of the issues Dr. Geistler mentions. I attribute failed care for foot ulcers primarily to non-compliance from the patient, improper/insufficient off-loading, vascular and then metabolic problems. I’m not aware of many in my area that use these grafts much, but we all have patients that might benefit from them. The current LCD should clean up some of the problems. It is quite clear on the matter and defines the indication and use rules distinctly. It is evident that CMS felt it necessary to implement.
Every practitioner practices differently. Not everyone was trained the same way. For instance, the quality of a residency can be a significant influence. If you are in a surgically focused program, you learn to solve problems with an arsenal of procedures you have seen work. But if you’ve never seen or done them, then you are at an unbeknownst disadvantage because...

Dr. Ronald "Ronnie" Carroll, 68, passed away on March 15, 2025. Dr. Carroll received his Doctor of Podiatric Medicine degree from the Ohio College of Podiatric Medicine in Cleveland, Ohio.
Dr. Carroll completed his post-graduate training in Podiatric Surgery at Lawndale Community Hospital in Philadelphia where he served as chief resident. Dr. Carroll is Board Certified by the American Board of Podiatric Medicine.
