Walking Barefoot at Home Can Cause Foot Pain: ME Podiatrist

"It's important to check your feet daily, especially for diabetic patients," says Dr. Jared Kohlbacher. People often walk around their homes barefoot. As we age, the fat pads under our metatarsals atrophy. Walking on hardwood floors for years can cause foot pain"

Dr. Kohlbacher says sometimes a fix for arch pain can be as simple as an orthotic.

FL Podiatrists Treat Homeless on Holy Thursday

In honor of Holy Thursday, one area hospital is taking a step in the right direction to help the homeless, by holding an event to provide foot care new gym shoes. OSF Saint Anthony joined forces with Carpenter's Place for Holy Thursday, a day that commemorates the last supper. "We decided on Holy Thursday and that we would do a washing of the feet,” said OSF podiatry resident Brittany Wojnicki, DPM.

Dr. Wojnicki says many of these people's feet are their only mode of transportation, so keeping them healthy and in proper shoes is important. “We actually bought 75 pairs of shoes on our own at Payless. People probably thought we were crazy because we were just grabbing shoes off the shelf,” Wojnicki said.

Skip Fish-Y Pedicures: NY Podiatrist

You've heard about salon pedicure fish soaks where the swimmers supposedly eat away dead skin from your soles. The truth is they carry skin cells from client to client. Just say no! Emily Splichal, DPM, a New York City podiatrist, cautions: “Avoid pedicure soaks if you have any foot/leg cuts or sores and after a lower leg wax or shave, which can create small undetectable nicks in the skin that create a portal of entry for bacteria. It's important since there is a rise in staph infections called MRSA (methicillin-resistant Staphylococcus aureus) that are hard to treat and resistant to antibiotics.”

Dr. Emily Splichal

While tubs should always be drained, cleaned, swabbed with disinfectant, and wiped between each customer, sometimes you can't tell. Salons that use disposable plastic footbath liners add a layer of reassurance. “Anyone with compromised circulation or nerve sensation to the feet should always skip the soak and cuticle part of a pedicure and just get a polish change,” Dr. Splichal says.

PRACTICE MANAGEMENT TIP OF THE DAY
Get Paid for Procedures Insurance Won’t Cover - Part 1

Getting paid for procedures continues to become more complicated. Many doctors are adding new procedures to their offerings that patients want and can benefit from, such as Botox to treat migraine headaches, dermal fillers for aesthetic purposes, laser therapies for scar reduction, stem cell and platelet rich plasma (PRP) injections for musculoskeletal disorders, genetic testing, and many more. But insurance companies often won’t pay for them. Here are some ways to get paid?

1. Be transparent about cost. Before anything is done, inform the patient how much they will need to pay and why. If a service is one that needs to be done more than once, or has that potential, let the patient know first. The patient will be upset paying out-of-pocket only to learn that there will be further expenses in the future.

CODINGLINE CORNER
Query: Diabetes Necessary for Functional Orthotics

I am seeing an increasing issue with commercial insurance requiring the presence of diabetes to qualify coverage for biomechanical orthotics (L3000). At first I thought there was confusion with the diabetic-insert codes. Are others seeing this trend? How do I address the suspected misinterpretation?

PM News Subscriber

Response: That varies with each carrier. Many still fall back on the old standard of covered if in a shoe attached to a brace. Some policies will allow an orthotic outside of the A codes for patients such as diabetics, people with severe foot deformities, etc., while others allow for the more common biomechanical issues of plantar fasciitis, etc. Bottom-line, pre-authorize orthotics and be clear as to the the HCPCS code you are requesting and the corresponding ICD-10 codes. In addition, always ask for any exclusions to orthotics as you may get a rookie telephone person who does not check the exclusions.
