INTERNATIONAL PODIATRISTS IN THE NEWS
UK Podiatrist to Run London Marathon for Charity

Podiatrist Emma Godson is preparing to put her best feet forward in this year's London Marathon - to raise funds for Cheltenham charity Heidi's Heroes. Paul Wood of Cheltenham launched Heidi's Heroes as a tribute to his late wife Heidi, a former world championship swimmer who died of cancer, aged 51, in 2012.

Self-employed Emma, a 35-year-old mother-of-two from Tewkesbury, swore she would never again tackle the gruelling 26 mile course around England's capital after completing the marathon in 2014. But when she gained a ballot place, she invested in a new pair of running shoes and is now a regular sight pounding the streets around her Walton Cardiff home. Emma, who part owns ProPodiatry in Cheltenham, said: "I had planned to take part last year, but I got injured so deferred my place until this year. I wanted to raise money for a smaller, local charity this time, and my husband Mark suggested Heidi's Heroes."

VALUE-BASED REIMBURSEMENTS

Four Consequences That Will Derive From MIPS- Part 2

2. Understanding the relationship between Medicare Parts A and B will become more complicated.

Some incentives will target cost of care for episodes based on diagnoses says David Wofford, senior manager for ECG Management Consultants in San Diego. So, if you are a health system employing your doctors, there are going to be very sophisticated analytics you would want to do to understand how performance under Part B — particularly the resource utilization incentive — will affect reimbursement under Part A. "For instance, he adds, "does the penalty outweigh the revenue they get from having a re-admission? What is the interplay of all these things? Most organizations are going to be struggling to figure that out."

QUERIES (CLINICAL)
Query: Oral Anti-fungal in 15 year-old with Kidney Disease

I have a 15 year-old student athlete with symptomatic subungual onychomycosis confirmed by biopsy/PAS staining. Her pediatrician had her use topical ciclopirox lacquer for the past several months, with little improvement. Her medical history includes IgA nephropathy. Like many of my patients, she can't use Jublia or Kerydin due to cost. Any recommendations for use of an oral anti-fungal (terbinafine, diflucan) in this patient?

CODINGLINE CORNER
Query: Removal of Benign Cyst

What CPT code is most appropriate for removal of a benign lesion/cyst such as a ganglion cyst?

Monica Link, DPM, Houston, TX

Response: The location of the cyst would determine which CPT code is best.

A cyst excised from the toe would be coded using CPT 28092.
A cyst excised from the foot would be coded using CPT 28090.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Veterans Deserve Better — and You Can Help
From: Patrick A. DeHeer, DPM

American veterans are getting a raw deal when it comes to foot and ankle care. Current law hampers the U.S. Veterans Health Administration’s ability to recruit and retain the best qualified foot and ankle care providers—podiatrists like yourself—and as a result, our vets must deal with unnecessary and burdensome delays to their healthcare. They deserve better.

How can you help? Sign this online petition demanding action, and then share the petition on Facebook and Twitter. When you share the petition, tell your friends, family, and patients why it is imperative for veterans to receive care by a podiatrist. Share the petition in your practice, as well, and encourage your patients to share it on their own social media feeds. To have the biggest impact on your senators, all APMA members are encouraged to follow up by sending a pre-written message via APMA’s eAdvocacy website.

RESPONSES/COMMENTS (E-HEALTH NEWS)
RE: Signature Log Can Be the Key to Surviving an Audit (George Jacobson, DPM)
From: Michael L. Brody, DPM, Simon Young, DPM

It is a Medicare requirement that every note be signed. A signature log is not a substitute for signing your notes. A signature log is a 'glossary' where you have the name of the provider in a human readable format next to the signature, which may be in chicken scratch format.

If you are the only provider in an office and the only signature on the notes is yours, such a glossary would not be necessary. When you have multiple providers in the office and a pen is used to place signatures, the signatures may not be legible, so a signature log can be extremely beneficial when trying to determine the author of notes that are older, especially if there is turnover in your office.

Michael L. Brody, DPM, Commack, NY

Simple answer. It is a CMS REQUIREMENT... period. Heed Dr. Brody's advice. Common sense need not rule, and per the OJ trial, "if it's not signed, it's not done."

Simon Young, DPM, NY, NY
