MI Podiatrist Discusses Freiberg's Infraction
According to Conway McLean, DPM, "avascular necrosis (AVN) is a term describing the loss of blood supply to some bone, which is especially common in the hip bone or knee cap but not infrequently seen in the foot. This strange ailment occurs most commonly in the head of the second metatarsal bone. AVN causes the head of this bone to soften. Consequently, the stresses and pressures of ambulation and weight-bearing lead to deformation of the surface of the metatarsal head."

"This condition is known as Freiberg's infraction, although technically it's an "infarction" which is defined as the disruption in blood supply to a bone, which this is. Dr. Freiberg, the physician who initially wrote about it, believed it to be a broken bone problem, aka a fracture. He had no inkling of the disruption of blood flow, which is correctly referred to as an infarction, not a fracture. But this condition isn’t called Freiberg’s infarction since the old name was maintained despite its inaccuracy.

Bill Would Move Guam Podiatrist From Allied Health to Medical Board
Lawmakers on Tuesday reviewed and heard testimony on Bill 56-38, a measure proposing to transfer the licensing and oversight of podiatrists from the Guam Board of Allied Health Examiners to the Guam Board of Medical Examiners (GBME). The bill, introduced by Sen. Therese Terlaje, would also create a designated podiatrist seat on the medical board. The legislation is looking to update regulatory structures that have remained largely unchanged since 1998, aligning local policy with models used in other U.S. jurisdictions.

Steve Kim, DPM, a podiatrist and member GMBE himself, told lawmakers of the growing, critical need for ethics training and clearer licensing standards. Dr. Kim, who practices podiatry and serves on the Allied Health board, thanked senators for introducing the measure. Kim said placing podiatrists under GBME aligns with practices in other states and reflects the limitations of Guam’s healthcare workforce.

I have been in practice for 33 years. I do work in my private office as well as on call at our hospital and see patients in our hospital wound care center. I went over my statistics for last year and I averaged seeing 32 ulcers a week for 48 weeks, not individual patients but ulcers. I have found it the gross exception to have to use a graft to heal anything. I certainly use grafts but when I went over my numbers, I used a graft on 4.8% of my patients last year.
Most people who have ulcers are going to heal with using the basic paradigms; such as, off-loading, establishing circulation, taking care of systemic illnesses or contributing morbidities, surgically removing...

Interesting that in a recent article in Becker's Newsletter providing a detailed rationale for the cut in UHC stock price. Three Issues: The government cutting funding to CMS. This requires many lengthy discussions. The government is changing the risk model analysis, which while initially embraced by UHC in 2023, UHC is now rejecting. Many have been ranting about this for some time! Last, but not what needs to be addressed today, and here is the clincher, INCREASED UTILIZATION BY MEDICARE BENEFICIARIES.
In other words, their beneficiary population is getting older and using more services. So now it's the patient's fault that they are getting older and using more services? Did I read that right? They have the chutzpah (for those of you not familiar with the term that's the utmost in nerve) to blame it on the patients! You have got to be kidding! That many carriers thought this would never...
