AT THE COLLEGES
TUSPM Conducts Office Emergency Workshop

The Office emergency workshop is held for all third year podiatry students at the Temple University School of Podiatric Medicine (TUSPM). It is taught by Temple’s ER doctors and begins with a lecture and is followed by a clinical workshop.

Dr. David Wald (C) sets the scene of a patient going into anaphylactic shock. Students (L-R) Patrick Bik, Gabriel Cathcart, Karan Malani, Walter Payne, and Alex Lakner debate whether the patient requires an EpiPen.

The clinical component includes a combination of high fidelity simulation, additional small group teaching, and case discussions with the use of a standardized patient (SP) to enhance the exercise.

PODIATRISTS IN THE NEWS
Toenail Infections Can Spread if Not Treated: OH Podiatrist

If your toenail is about to fall off, you're probably thinking "Help!" in sheer panic and making the same face as the "scream" emoji. But when it comes to losing one of these little guys, it pays to take a chill pill and wait. It's worth giving your toe a little extra TLC because you definitely don't want an infection to spread to the new nail growing in"Redness/drainage/excessive pain could be signs of infection but not always," says Said Atway, DPM, a podiatrist at The Ohio State University Wexner Medical Center.

Dr. Said Atway

"The consequences of a bacterial infection in the toe are the same as the consequences of any other skin/soft tissue infection in that the infection could spread and lead to further detriment of the surrounding tissue," Dr. Atway says. Obviously, not great—so if you think it could be infected, go get it looked at by a doc.

PROFESSIONAL DISCIPLINE
Medical Board to Review OH Podiatrist after Opioid Allegations in WV, KY

The Ohio State Medical Board is set to consider the license of an OH doctor who was previously under investigation by agencies in West Virginia and Kentucky. According to a letter from the Medical Board to Dr. James Prommersberger, the Board will review his license after learning that his license was put on probation in West Virginia after allegedly misprescribing opiates. The Medical Board claims that the Kentucky Office of the Inspector General found that for a period of one year from 2014-2015, Dr. Prommersberger prescribed over 1,100 prescriptions, nearly 90 percent of which were hydrocodone.

PRACTICE MANAGEMENT TIP OF THE DAY
Overall Tax Strategies for Physicians - Part 3

Now that the tax-filing deadlline is here, the die is cast on last year's revenues, refunds, and amounts owed. However, smart planning throughout the year and effective management of expenses and deductions can leave physicians and medical practices better equipped for next year's filing season. Here are some tips that can help physicians maximize revenues and sail smoothly through tax season into next year.

3. Improve Account Reconciliation - "[Improving reconciliation of accounts receivable] will increase financial control, reduce rework, and ensure accuracy of patient accounts. [This includes] posting electronic fund transfers after confirmation of bank deposits and reconciling accounts receivable to bank statements prior to month end." - Jackie Coult, Healthcare Business Consulting

CODINGLINE CORNER
Query: Palmetto GBA blues: Home Health Certification

We have just been informed that podiatrists are not allowed to certify home healthcare. They say it is Medicare guidelines. Previously, Cahaba paid for home health certification by podiatrists with no problem. Is this true or should we fight it?

Katherine Sharp, Keystone Professional Solutions, Woodbury, TN

Response: I recommend fighting it based on this link from CMS.

RESPONSES/COMMENTS (CLINICAL)
From: Dennis Shavelson, DPM

I’m sure that this almost teenager and her family are desperate for a diagnosis and  treatment that will work. This is known as disappearing nail bed syndrome in the literature (Daniel R, Babaev M, Avner S: An Update on the Disappearing Nail Bed; Skin Appendage Disord. 2017 Mar; 3(1): 15–17) and the etiology is not primary infection. Continuing to diagnose and treat this problem as a fungal or saprophytic problem with terbinafine is reckless at best, especially in a twelve year old.

Notice that her toenail is short and the distal digital soft tissue is bulbous (pathognomonic). You should notice that her hallux is especially long (shoe etiology). I also detect signs of a flexible forefoot foot type that is serving as underpinning etiology needing treatment that need to be confirmed. If the patient has peroneus longus atrophy or a weakness in the lateral compartment of her legs, the diagnosis will be made.

These toenails need debridement in addition to attention to the other factors mentioned above. A referral to a dermatologically-dedicated DPM is an option. The patient and her parents will thank you.

Dennis Shavelson, DPM, NY, NY

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Bret Ribotsky, DPM

I have used Tsheets for years with a few service employees. It geotags them, and lets you track them while on the job. It runs via their smart phones and works directly with Quickbooks payroll.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: John S. Steinberg, DPM

We apologize for what your friend experienced. However, it is unfair to turn this misinformation into an accusation that Georgetown is somehow discriminatory towards podiatric surgery. I am the co-director of the Center for Wound Healing at MedStar Georgetown University Hospital. Our team is composed of podiatric surgeons, plastic surgeons, vascular surgeons, nurse practitioners, and numerous other specialists.

MedStar Health is a system of over 36,000 employees with 10 hospitals, so I cannot speak for every circumstance, but I can tell you that podiatric surgery is well established here and is not in a discriminatory status. I believe it would be best for you and me to speak directly about what happened rather than have this debate on PM News. Please contact me at 202.444.3059 and I would be happy to reach out to your friend to provide assistance.
