AT THE COLLEGES
NYCPM Students Care for Boston Runners
 
On Monday April 18, fifteen members of NYCPM’s Class of 2017 served as part of the medical team for the 120th Boston Marathon, as did Jessica Olson, DPM, a 2014 graduate. Dr. Olson had been part of the medical team during the tragic bombing at the Boston Marathon in 2013.

HOSPITAL PODIATRISTS IN THE NEWS
ND Podiatrist Performs First Ankle Replacement at JRMC
 
Staff and technology have made it possible to perform the first total ankle replacement at Jamestown Regional Medical Center. Dr. Kyle Abben, a foot and ankle specialist at the JRMC Podiatry Clinic, recently performed the first ankle replacement at JRMC The Jamestown woman who received the replacement is in her 40s and has suffered with ankle pain for several years, Abben said. After imaging studies, she decided to proceed once she was declared eligible for the procedure, he said.

Changes in technology and medicine make replacements last longer and possible at younger ages today, Abben said. The implant is made from porous materials that fuse with the bone, he said, and 3-D imaging of the foot, ankle, and lower leg allows engineers to design a replacement joint specifically for that patient’s anatomy. “We have come a long way and have a new outlook on function and pain relief,” Abben said. “It is an emerging field.”

NY Podiatrist Named One of the Top 10 Caribbean-Born Doctors
 
Born in the late 1960s in Haiti, Dr. Patricia Nicholas is the Vice-President of the New York State Podiatric Medical Association and founder of the Kindest Heart Foundation which was founded after a volunteer stint in her homeland after the devastating earthquake of 2016. Dr. Nicholas has a private practice in Long Island, New York and is Past President of the Caribbean American Medical and Scientific Association.

She migrated to the United States in 1982 and graduated from the New York College of Podiatry Medicine in 1993 and completed her residency from Wyckoff Heights Medical Center.

UAE Podiatrist Stresses Importance of Physical Activity 
 
Lifestyle habits begin at a young age, and parents must ingrain the habit of walking through family activities. Dr. Susan Tully, the head of podiatry at Healthpoint hospital in Abu Dhabi, says physical activity is losing out to computer games and mobile phones. “A sedentary habit when young becomes a lifestyle when they are adults," warns Tully.

She says young people should aim to spend at least 60 minutes doing a physical activity, which should be done at school as well as at home. Tully says young people can add to their steps each day by being dropped off farther away from school and walking the extra distance, taking the stairs to their classrooms, and involving friends in the activity.

ACA UPDATE
UnitedHealth Will Drop Out of Most ACA Exchanges
 
UnitedHealth Group CEO Stephen Hemsley said Tuesday the health insurance and services conglomerate will pull out of most of its Affordable Care Act marketplaces. But the company won't bail on the exchanges completely and will sell individual plans in a “handful” of states. “We cannot broadly serve it on an effective and sustained basis,” Hemsley told analysts and investors on a conference call. UnitedHealth has fully or partially exited five states so far—Arkansas, Georgia, Louisiana, Michigan, and Oklahoma, according to various news reports. 
 
The company sold plans in 34 states for this policy year and did not disclose which states it will stay in. Insurers that sell plans through the federal HealthCare.gov portal have until May 11 to file rates for 2017 plans. UnitedHealth recorded an additional $125 million loss on its individual ACA plans, meaning the company's total ACA losses for 2015 and 2016 will exceed $1 billion. UnitedHealth signed up many sicker-than-expected members, ending the first quarter with 795,000 public exchange enrollees, which is only a fraction of the ACA's individual market.

RE: Beware of Unjustified Use of the -25 Modifier

There are a lot of skin and nail paring, debridement, and removal of benign lesions in everyday podiatric practice. There are times when in addition to these procedures, a "separately identifiable" E/M service is performed and can be  can be legitimately billed with the appropriate E/M code along with the -25 modifier.  But the rules for what is and what is not a "separately identifiable" service have to be carefully considered every time the E/M code and modifier are billed. Obviously, these dermatologists who were the targets of a federal investigation and fraud conviction didn't pay enough attention to the rules.

Could a signature at the end of the daily schedule book attesting to the patients who were seen that day by the signing doctor, function as a signature log described by Dr. Brody? If someone is the sole provider, why is a signature required when no other provider could have performed the service? I have the employee put the note created in Dragon in the chart, initial and date it to further validate its authenticity. I feel that this would help prevent any question as to the date the note was created.
