The 127th running of the Boston marathon took place on Monday in rainy conditions. The medical team included 50 podiatrists led by Jonathan Kaplan, DPM. Students from both NYCPM and TUSPM participated in treating runners.

MN Podiatrist & Wife Promote Lifestyle Medicine
Next weekend, the Lotus Health Foundation will host its fifth annual Community of Wellness Symposium and Gala, a two-day event centered around lifestyle medicine. This year’s theme for the event, which is open to the public, is self-care, said Mei Liu, founder and president of the Lotus Health Foundation.

Liu and her husband, Rochester Clinic podiatrist Jengyu Lai, DPM, have been promoting lifestyle medicine in Rochester since 2010, and the symposium is an extension of their work. Lifestyle medicine, Lai said, focuses on factors like diet, movement, sleep and other lifestyle conditions to improve patients’ health and treat or prevent chronic medical conditions. “About 80% of chronic conditions are lifestyle related, so in order to reduce healthcare (costs), we really need to look at our lifestyle.”

5 Steps to Start Your Own Independent Practice
The key to launching a successful independent practice is proper planning. Knowing what steps to take and what mistakes to avoid is critical to ensuring a sustainable, profitable practice. Follow these five steps to get started down the right path:
2. Envision the Future - Once your mission is clearly defined, set long-term goals for your practice. You can’t get there in one step, but you can certainly strive for it from Day One. For example, do you want to grow your practice to include ancillary services, such as a sports medicine clinic? Do you want to launch your own line of supplements?
The type of practice you want to build will impact the tasks involved in starting it, such as getting credentialed with insurance companies, locating an initial space with room for future expansion, and hiring individuals with the necessary skills to make your practice successful.

Query: Post-Operative Complication
We had a patient who followed up 26 days post-operatively for an Austin/Akin bunionectomy. During the visit, x-rays revealed that the patient had a dorsally displaced fracture involving the 1st metatarsal head. The screw within the first metatarsal head remains in place. The patient does not recall any injury to the area; he does have diabetic neuropathy. Can we bill for the fracture?

Response: Over the years, I’ve faced this several times when the osteotomy becomes displaced during the post-operative global period following bunion repair. The reason this occurred is not relevant to treatment.
The surgeon must decide if the displacement is clinically relevant to a successful outcome. If the decision is made to continue with normal post-operative care, then there should not be any consideration for further billing. In the event that the dislocation is significant and requires reduction, I recommend using the fracture code that best describes what is done. Additionally, add modifier -78 to indicate this fracture care is occurring during a post-operative global period. Here are the options:
CPT 28470 closed treatment of metatarsal fracture without manipulation
CPT 28475 closed treatment of metatarsal, with manipulation
CPT 28476 percutaneous skeletal fixation of metatarsal fracture with manipulation
CPT 28485 open treatment of metatarsal fracture with or without internal or external fixation
In each case, I suggest adding modifier -78, indicating treatment of a fracture during the post-operative global period.

There is an issue with the way this case is being presented that begs to be addressed. Two issues are being conflated and readers may get the wrong idea about writing for compounded creams. One issue is whether a physician should write prescriptions for creams made by a compounding pharmacy. The other issue is whether the physician convicted of fraud actually saw and evaluated the patients. Both are two distinct issues. In the article, it indicates that the creams were medically unnecessary. Well, they may have been unnecessary not because there was anything wrong with the creams but because the prescribing doctors never saw the patients.
There are many compounded medications that are very helpful and doctors from all medical specialties frequently write for these drugs. If there is a clear indication for them, we should not be dissuaded from appropriately prescribing them but we must base the prescription on clear evidence that they are indicated. This always requires seeing and evaluating the patients.

It has been a month since the summit regarding the board certification in our profession hosted by APMA took place in DC. Zero has been communicated back to the participants other than a summary and the audio recording. Personally, I spent a few thousand dollars to attend this event, as I’m sure others did. I am retired, due to my disability, but still a passionate cheerleader for our profession. I was optimistic and hopeful that something would have begun. One of the consensus items was a swift and speedy progress; not seeing this, it’s time to begin.
So today, I am proposing a step forward. As the two boards have not reported any progress, I’m offering the creation of a new board, the American Board of Podiatry (ABP). This board will offer certification in basic podiatry that should be attainable after three years of residency training. Additional certificates of added qualification in surgery, advanced surgery, wound care, and trauma, etc. will also be created. Specific names and qualifications for these CAQs to be determined. All those with certifications by ABPM and ABFAS will be grandfathered to the category appropriate.
The other certifying boards (ABLES, ABMSP) would also have their members included in some format, but I’m not knowledgeable of their processes in enough detail to lay them out at this time. Who will run this board? - I’d love it to be an amalgamation of the current approved boards, but if they are unwilling to participate, then I’d leave this open for suggestions. Who’s interested?
