PODIATRISTS IN THE COMMUNITY |
CT Father-Son Podiatrists See Big Steps Over 59 Years
The careers of Dr. James H. Blume and his son, Dr. Peter A. Blume, span the evolution of podiatry from a practice that involved only minor surgery to one that includes reconstruction of feet and ankles and preventing amputation. Recently, both Blumes received honors — Peter was inducted April 8 as president of the Connecticut Podiatric Medical Association. He then presented his father with a lifetime achievement award.

Drs. James and Peter Blume |
James Blume, founder of what is now Affiliated Foot and Ankle Surgeons has practiced for almost 59 years, starting back when “we did minor elective surgery but we didn’t go into the structural corrective surgery” that his son does on a regular basis, he said. Over the years, as the elder Blume helped expand the laws to allow for more complex surgical procedures, the profession became more advanced.

Hosted by Bryan Markinson DPM, Chris Adigun MD, and Justin Lewis, DPM, this hands-on, 4-hour demonstration offered one-of-a-kind learning centered around shave biopsy, skinplasty for cancerous lesions, and tangential nail matrix biopsy to rule out suspicious lesions including melanoma.

Signature Log Can Be the Key to Surviving an Audit
All medical records need to be signed, whether they are paper or electronic. A common reason for failing an audit is unsigned medical records. When using your EHR system, please be sure to sign your notes in a timely manner. Each time you sign your record electronically, a time/date stamp is placed into your record that memorializes the time and date you signed the record. A large gap between the date of service and date of the signature can result in questions regarding the authenticity and accuracy of the signed record.
Paper records also require signatures. With a paper record, there are times when a signature may not be legible. For paper records, a signature log is very important and should be kept. This can be especially useful should a provider leave the practice, since that provider will no longer be available to authenticate his/her signature. A signature log is a typed listing of the provider(s), identifying their name(s) with a corresponding handwritten signature. This may be an individual log or a group log. A signature log may be used to establish signature identity as needed throughout the medical record documentation.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A |
RE: 2006 DERMfoot Meeting
From: Daniel Chaskin, DPM
The DERMFoot organization presented, in my opinion an unparalleled informative seminar greatly enhancing the ability of any attendee to acquire the knowledge needed to address dermatologic conditions of the feet. Every podiatrist practicing should give serious consideration to attending this seminar next year. I just finished my 3rd day here and looking forward to my 4th and last day of this intensive wealth of knowledge. Thank you DERMfoot for bringing to us such brilliant speakers.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B |
RE: 2006 DERMfoot Meeting
From: Elliot Udell, DPM
This past weekend, I had the honor of presenting at and attending the annual DermFoot seminar. Kudos to Drs. Morse, Joyce, and Schultz for taking hundreds of hours from their busy practices to make this seminar happen. To quote Bryan Markinson, "it was mesmerizing." Some of the leading clinical academic dermatologists in the world came from parts of the globe to speak at this seminar. They gave the audience the latest pearls of wisdom in evaluating and treating dermatological conditions affecting the lower extremity.
Another thing that the organizers were aware of and addressed is the reality that in any intense medical seminar, the volume of information always exceeds the ability of most attendees to absorb it all. This problem affects all medical seminars and what the DERMfoot 2016 did was to make videos of all lectures, which will be converted to webinars and made available for review by attendees and members of the organization. I look forward to attending this program again next year which will be held in a much larger venue in Annapolis, Maryland.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: George Jacobson, DPM, Lawrence M. Rubin, DPM
Dr. Secord has it exactly right. It was said that this was a "bridge" to single payer healthcare. Unfortunately, it is also a bridge to early retirement.
George Jacobson, DPM, Hollywood, FL
Times of turbulent social and economic change create new opportunities. Where each podiatrist goes in the future can largely depend on recognizing that what he or she does right now in preparation for the future will shape that future. There is an epidemic of diabetes, pre-diabetes, and peripheral artery disease affecting tens of millions of people in the U.S. There is no healthcare provider in each podiatrist's locale more capable than him or her in preventing the human misery and tremendous economic burden to the ACA of diabetic and PAD foot ulcers and their resulting amputations, disabilities, and even deaths.
Podiatrists who reach out to their communities (ideally together with other podiatrists as a network) to market their special focus on diabetes and PAD amputation prevention care will enjoy the practice satisfaction of saving limbs and lives. When networked with other podiatrists, the dollars and cents value of their much needed services can be negotiated with healthcare payers. What's more, in a system where elective surgery is destined to decline, preventive surgical care that follows defined medical necessity standards for the elimination of potentially hazardous foot problems is not considered to be elective. I suppose how we look at where we are going in the future is kind of like whether we see the glass half-filled or half empty.
Lawrence M. Rubin, DPM, Las Vegas, NV
