For the past thirty-one years, it has been our privilege to provide the podiatric community with a daily newsletter containing news, coding and billing information, as well as an interactive forum for a discussion of issues important to this profession. In short, we have attempted to keep everyone informed and thoughtful. We are proud to be the longest running periodical of this type serving any profession.

Foot Clinic of NY/NYCPM Awarded Hepatitis B Foundation Grant for 2025
The Foot Center of New York (FCNY), affiliated with New York College of Podiatric Medicine (NYCPM) at Touro University, was awarded the Hepatitis B Foundation’s Hep B United Mini-Grant for 2025. This CDC funded program focuses on enhancing the capacity of Hep B United coalition partners to conduct hepatitis B education, testing, and linkage to care activities. The principal investigator (PI) of this project is Paramita Basu, PhD, MS, associate professor of infectious diseases at NYCPM and the Co-PI is Eunice Ramsey-Parker, DPM, Director of Corporate Compliance and Regulatory Affairs, FCNY.

Hepatitis B remains a significant public health concern in New York City, with particular implications for communities like Harlem. Medical evidence has also established a bi-directional relationship between hepatitis infections and diabetes mellitus. "This grant will help FCNY and NYCPM address the critical public health gap in Hepatitis B awareness, screening, and prevention, especially in high-risk populations, within the podiatric care settings of FCNY, with a particular focus on serving susceptible communities like Harlem, which is a diabetic "red-zone" with patients from diverse racial ethnic and economic backgrounds," according to Dr. Basu.

The team, made up of undergraduate and postgraduate podiatry students from the University’s School of Education, Sport and Health Science, will join healthcare professionals inside the medical tents at this year’s TCS London Marathon on April 27th, providing expert foot and lower limb care to thousands of runners.

Query: Injection Codes for Orthobiologics
Is billing small joint and large joint injection codes permitted for orthobiologics as per a recent webinar I attended?
thedoctorline.com response: Joint injection CPT Codes are not covered when you inject a material that is considered experimental and investigational. If the patient's specific health plan has a policy, check both their medical policy and reimbursement policy websites.
Also, consider having your staff call with the specific biological you plan to inject to determine if they cover it. Be careful about thinking that it is automatically covered because an injection CPT code like 20600 or 20605 is generally a covered service.
Remember, it has to be paired with a J code usually detailing what was injected. Just attending a webinar or a colleague saying "yes, it is covered to bill orthobiologics," is never a reason to start billing something that is new and/or controversial. Do your due diligence, like starting in a forum with experts. By no means stop with this Q&A though. Confirm. Confirm. Confirm. Medicare has determined that some orthobiologics are not covered and has been clawing back payments they made several years ago, so buyer beware!

RE: Practice Dissatisfaction

Over the past 38 years, I have worked with many doctors in the field of functional medicine and regenerative medicine. And we are a very happy and satisfied group of doctors. About 2 1/2 years ago, when I became very active on LinkedIn, I became aware of the stark reality of most doctors out there. Unhappiness, malcontent, frustration, disappointment, anxiety, depression, burnout, anger and high levels of stress have caused most to be really miserable in practice. And I'm not only speaking about podiatrists. It's universal among all doctors.
And when they share why, that is universal too. It is a combination of the low fees, unpredictable payments, administrative burdens, enormously high volume of patients they have to treat, or unscrupulous and demanding employers who put them under constant stress and don't pay them fairly. Yet, for some reason, it is the pink elephant in the room. Does the next new surgical approach or instrument really matter if you are miserable? Is the next new modality really...

RE: A Fishy Story

In 1976, I was one year out of my one-year podiatry residency. I was on staff at New York Downtown Hospital with residents from NYCPM. The director of the podiatry department was Gary Sherman, DPM. He was also my favorite attending during my residency at Maimonides Medical Center. Although I have had a lot of contact with Dr. Sherman over the years, one experience stands out. I had a patient scheduled for surgery with severe hallux rigidus for a cheilectomy or 1st MPJ fusion. I told the patient I was going to decide which procedure to do during the surgery. Because I hadn’t done any first MPJ fusions on my own, I asked Dr. Sherman if he would mind scrubbing with me.

During the surgery, he was watching me operate, but as I was about halfway through the surgery, I noticed he kept looking up at the clock. I said “am I operating too slowly?” He said “TIME AND TIDE WAITS FOR NO MAN! At noon, I am going fishing; that is when the tide will be right and the fish will be biting. Hurry up and you can go fishing with me.” I picked up my pace during the surgery and he convinced me to cancel my afternoon patients and join him. At noon, we were catching striped bass and bluefish near the Statue of Liberty. Gary will be retiring in about a month after 50 years of being the director of podiatry at various hospitals and teaching hundreds of residents. I know all of his former residents, myself included, wish him well. He has fished all over the world and now he will have even more time to do it.
