Most Foot Problems Result from Functional and Structure Imbalances: VA Podiatrist
 
According to Mitchell R. Waskin, DPM from The Foot & Ankle Center, the majority of people's foot pain is functional. "You're born with a foot structure that not quite exactly normal, so you get minor muscle imbalances that cause stress and strains that don't belong there. This causes problems like chronic heel pain (plantar fasciitis), bunions, hammertoes, or calluses.

"Unfortunately, many people do not seek treatment because they mistakenly believe that foot pain is normal. Fortunately, there are treatments for these conditions. Most, with the exception of foot surgery, are not painful," says Dr. Waskin.

ABFAS Announces New and Expanded Case Review Requirements
 
The American Board of Foot and Ankle Surgery Board of Directors has approved a set of changes to case review requirements that reflects current practice patterns and evaluates patient outcomes. Effective for the 2017 exam cycle is the expansion of eligible case subcategories used for the Part II certification case review process.
 
The podiatry profession continues to evolve with more podiatrists performing rearfoot reconstruction, and trauma and limb salvage procedures. In response to the many changes in the profession and the need to expand case review categories, ABFAS took action to ensure that skilled podiatric surgeons who commonly perform sophisticated non-bunion forefoot and/or rearfoot/ankle surgery and reconstruction could participate in the case review process in a timely fashion. The expanded case subcategories and other details can be found here.

David G. Armstrong, DPM, MD, PhD, Professor of Surgery and Director of the University of Arizona’s Southern Arizona Limb Salvage Alliance (SALSA) delivered the first “UCSF Limb Preservation Lecture” at the annual UCSF Vascular Surgery Symposium.

CA  Podiatrist Sentenced to Prison for Healthcare Fraud
 
A Roseville podiatrist who admitted to carrying out a $1 million healthcare fraud scheme has been sentenced to three years in federal prison. Neil Van Dyck, 64, was sentenced Friday in Sacramento by U.S. District Judge Garland E. Burrell Jr., who also ordered Van Dyck to pay a $10,000 fine, according to a U.S. Attorney’s Office news release. Van Dyck pleaded guilty in October to committing healthcare fraud. According to court documents, Van Dyck was a California-licensed podiatrist who operated a podiatry practice in Roseville called Placer Podiatry. He offered “spa-like” treatments and performed routine foot care at his practice.

Modified Maggots Could Help Human Wound Healing
 
In a proof-of-concept study, NC State University researchers show that genetically engineered green bottle fly (Lucilia sericata) larvae can produce and secrete a human growth factor – a molecule that helps promote cell growth and wound healing. Sterile, lab-raised green bottle fly larvae are used for maggot debridement therapy (MDT), in which maggots are applied to non-healing wounds, especially diabetic foot ulcers, to promote healing. Maggots clean the wound, remove dead tissue, and secrete anti-microbial factors. The treatment is cost-effective and approved by the Food and Drug Administration. However, there is no evidence from randomized clinical trials that MDT shortens wound healing times.
 
With the goal of making a strain of maggots with enhanced wound-healing activity, NC State researchers genetically engineered maggots to produce and then secrete human platelet derived growth factor-BB (PDGF-BB), which is known to aid the healing process by stimulating cell growth and survival.

FDA Clarifies Compounding Rules
 
The Food and Drug Administration issued three draft guidance documents on Friday that answer providers' and pharmacists' burning questions about drug compounding standards. The proposed rules clarify the amount of drugs that providers can compound without a prescription, set requirements for health system compounding pharmacies, and clarify how large-scale compounding sites known as “outsourcing facilities” may be set up. This follows two reports from the Pew Charitable Trusts that found compounding oversight is largely left up to the states and varies widely.
 
Added in 1997, section 503A of the Food, Drug and Cosmetic Act allows pharmacists or physicians to compound a “limited quantity” of drugs to keep on hand in anticipation of an established patient's needs. Friday's guidance explains that providers will meet this requirement if they compound only what they think they would need for a typical 30-day period, based on patterns with actual patients.

"Physicians who are employed by a group practice and/or hospital and who individually receive miscellaneous income in the form of 1099-MISC from pharmaceutical companies, attorneys for court testimony, or other sources, should consider reporting such income on a schedule C. This income could be reduced by expenses incurred for generating certain income such as travel, accommodations, meals, and supplies. You could also make contributions to a SEP to lower or eliminate this type of income, even if you participate in a 401(k) with your employer."

Query: Post-Op ORIF Care for Another Surgeon
 
I saw a new patient yesterday who had open reduction internal fixation (ORIF), left ankle, 2 weeks ago while on vacation up north. She was referred by her primary care physician to me for "post-op care." She was a new patient to my practice, and required x-rays, removal of sutures, and a walking cast application. She will need follow-up and subsequent office visits, cast changes, x-rays, and other post-op care. Needless to say, I am assuming care. 
 
Do I bill an office visit, cast application, x-ray study, or do I use the ORIF procedure code with modifier "-55" (post-op management only)? What do I bill for subsequent care if I use the ORIF code?

While I have the greatest respect for Dr. Secord as a podiatrist, his post is just opinion with a lot of guesses. Maybe I see it from a different point of view because I have not participated in Medicare or Medicaid for nearly 30 years. When the PPACA was first proposed, and I heard that the insurance companies jumped on board, I was very suspicious as to why. It sure wasn't to lose money, and I really doubt that they are actually really losing. What I do know for sure is that I have a number of patients with a PPACA policy, with varying high deductibles, who sought treatments because they had insurance for the possibility of a major injury or illness, so they were not panicked by having to pay out-of-pocket for routine care, including orthotics. 
 
What I think has really colored Dr. Secord's thinking, was Faux News :-)
