NY Podiatrist Promotes National Bunion Day
 
To celebrate National Bunion Day, podiatric surgeon Bradley Schaeffer, DPM is on a mission to keep people well-informed about foot health while also dismantling all the misconceptions surrounding bunions and what causes the painful condition. Dr. Schaeffer is also a well-known expert from TLC’s show My Feet are Killing Me and owns Central Park SOLE in New York City, where he cares for patients dealing with ankle and foot problems.

Schaeffer graduated from the Temple University School of Podiatric Medicine, and then completed a foot and ankle reconstructive surgical residency program at Hoboken University Medical Center. UAB Introduces Advanced Podiatric Limb Salvage and Reconstruction Fellowship
 
The University of Alabama at Birmingham (UAB) Division of Vascular Surgery and Endovascular Therapy is expanding its podiatric surgical services and introducing a new podiatric fellowship – the first of its kind in Alabama. The Advanced Limb Preservation Clinic at UAB will play a vital role in improving the lives of patients with complex, chronic conditions.

Clinical Associate Professor Kyle Mauk, DPM, who specializes in podiatric foot and ankle surgery, spoke on a recent episode of UAB MedCast about the future of podiatric care and training at UAB. “Now is a great time to be a podiatrist in Alabama,” Dr. Mauk said. “We can really treat almost every condition of the foot and ankle. I think that provides a whole lot of expansive options for patients when they’re looking to receive foot and ankle care.”

"Ozempic feet" Side-Effect of the Weight-Loss Drug Reported      
 
Injectible weight-loss drugs such as Ozempic can be life-changing for patients who have struggled to lose weight, yet some people have reported side-effects. According to Dr. Ross Perry,  medical director of Cosmedics Skin Clinics, now “Ozempic feet” is “emerging as another potential side-effect gaining attention. “Much like the face, the feet can experience sagging, wrinkling, and a loss of the usual taut, smooth appearance due to a significant reduction in body fat. In more extreme cases, particularly in individuals who were previously overweight, this can even result in excess skin or folds developing around the feet,” he said.
 
Taking Ozempic has even been linked with a change in shoe size, “either because of the reduced fat volume or shifting bone structure once the cushioning is lost,” Dr. Tijion Esho, an aesthetic doctor and the founder of Cultskin, told The Standard. “Shoes that once fit snugly now feel loose or uncomfortable, which can further impact posture and gait.” According to Esho, Ozempic can cause changes in the feet due to weight loss, leading to a “sudden loss of fat padding in the soles”. The fat padding in our feet cushions and absorbs impact when we walk. “When that volume suddenly disappears, it can feel like you’re walking on bone,” he said. “Patients report soreness, burning, and even difficulty standing for long periods.”

NJ Podiatrist Discusses What to Look for in a Flip-Flop
 
“A good flip-flop should have a cushioned footbed, adequate arch support, and a secure strap system,” says Elizabeth Bonarigo, DPM, a board-certified podiatrist specializing in foot biomechanics, based in New Jersey. “The original flip-flop design, with its tong toe strap and flat rubber footbed, lacks arch support. Luckily, newer designs and technologies offer some options with more support.”

A contoured footbed can help stabilize the foot, improving alignment and reducing strain. Dr. Bonarigo suggests looking for flip-flops with a heel cup, which secures the heel and prevents excessive movement while walking.

Query: Annual Wellness Visit for Diabetic Feet
 
Am I entitled to bill for a wellness annual diabetic foot visit?
 
Thedoctorline.com response: Internal medicine and family practices can get reimbursed for an annual well exam, with specific CPT coding. However, podiatry does not get reimbursed for an annual diabetic foot exam via a specific CPT code other than under the Loss of Protective Sensation (LOPS coding). The inherent issue with LOPS coding is that patients do not qualify for this if they have seen any healthcare provider who billed for a foot-related issue in the past six months.
 
The good news is that most insurance covers illness, injury, or symptom related to an E&M visit. A common diabetic issue is Type 2 diabetes with polyneuropathy, ICD-10-CM E11.42. These patients have a chronic illness that affects the feet and legs and should be evaluated and medically managed when necessary. But remember, it is both E (Evaluation) and M (Management). You must be actively managing the patient’s condition.

RE: LCD Changes for Wound Care

Here are two recent articles from NIH. The first article states that the U.S. spends $150 billion annually in wound care. We are in the number one slot, with China in a distant second. The second article shows Medicare spending between $28-97 billion on wound care, with diabetic ulcers being the mainstay of the cost outlaid annually.
 
Whether podiatry is specifically mentioned or not is inconsequential. The cost of wound care has skyrocketed for some time. The recent LCD changes that have now been delayed until Jan. 2026 are necessary and should be implemented to not only quell the costs, but to also limit fraud waste and abuse.

Dr. Kesselman has offered a concern and has advised precaution in referrals to interventional radiologists, interventional cardiologists, and vascular surgeons. My experience over the last 2 years would support his warning. I have had the occasion to review multiple litigation scenarios in which a podiatrist has been named in a suit, rightfully or wrongly, for alleged inappropriate referral to interventionalists.
 
In some circumstances, podiatrists have entered into co-investment agreements with free-standing cath labs, somewhat similar in structure to that typically constructed with surgical centers. At least one national chain of such cath labs, offering stock and investment opportunity for the podiatrist, has come under federal investigation.
