GA Podiatrist Discusses Surgery for Club Feet
 
According to Jennifer Monaco, DPM, "Surgery for club foot is an incredibly complex and delicate process. When performed by a skilled surgeon, it can be remarkably successful with little complication. The surgery typically involves repairing misaligned tendons and ligaments, as well as shortening them to improve the overall flexibility of the foot. After surgery, the patient must go through intensive physiotherapy to help with flexibility and movement of their newly adjusted foot."

"Club foot surgery is a major undertaking, but with the right post-operative care and treatment, recovery can be successful and expeditious. In most cases, mobility returns within the first few weeks after surgery, allowing patients to resume normal activities once cleared by their physician. We also advise patients that bandaging will reduce swelling and minimize pain during recovery, as well as remind them not to overexert themselves and take frequent breaks throughout their day. NY Podiatrist Provides Tips on Buying Shoes if You Have a Bad Back 
 
When determining what shoes are best for back pain, podiatrists say it is particularly important to avoid a rigid design, or shoes that do not bend easily in the forefoot. "If the forefoot cannot extend properly, this causes excessive contracture of the calf muscles, which will overly flex the knees and unlock the pelvis," says Robert Kornfeld, DPM, of Manhattan's Chronic Foot Pain Center. Because an unstable pelvis leads to tightened lower-back muscles and pain, you'll want to be mindful of the rigidity of a shoe when making a selection.

Orthopedic shoes are designed to fit the foot of the wearer and may eliminate "tight spots" and compression forces on the foot, which can lead to compensations in gait that can cause back pain. If your back pain is mostly due to foot deformity and conventional shoes are not a good fit, Kornfeld says orthopedic shoes may be a good solution. "If back pain is coming from unstable, hypermobile feet, then walking shoes—which are more stable and more rigid than running shoes—would be the right shoe," Kornfeld says. "If the back pain is coming from rigid, non-flexible feet, then a soft, shock-absorbing running shoe would work best." In other words: It's not always (or ever) one-size-fits-all.

INTERNATIONAL PODIATRISTS AND SPORTS MEDICINE
UK Podiatrists to Supervise Medical Care at London Marathon
 
A team of podiatry students will be taking part in the London Marathon next weekend – but they won’t be running the race. Instead, the 15 volunteers will spend the day as part of the official medical team, joining physiotherapists, doctors, nurses, and St John Ambulance staff. The university has been organising this opportunity for hands-on fieldwork in collaboration with the Royal College of Podiatry for more than 10 years.

PA Podiatrist Honored With Award in Iceland for Fish-Based Wound Treatment
 
An Excela podiatrist was honored at a conference in Iceland for his use of a wound treatment product that comes from an unlikely source: under the sea. Patrick Roberto, DPM of Excela Health’s Advanced Wound Center traveled to Reykjavik at the end of February for the Northern Lights Wound Workshop after being recognized as a recipient of an Aurora Award from Kerecis, a company that develops healthcare products made from fish skin.

Dr. Roberto submitted a clinical case study detailing his successful work with the Kerecis fish skin products to treat a patient with a leg wound that was difficult to heal. At the conference in Iceland, Roberto presented the case study and learned from other doctors who had also shown success with the treatment.

Student Loan Concepts to Know During Tax Season
 
With tax season approaching, many people are focused on gathering all the tax forms to make sure they don’t miss the April deadline; however, few will pause to inquire how their tax filing could impact their student loan payment. Fortunately, there are many ways that borrowers can get creative with income reporting come tax season that could impact their student loan payments substantially.
 
11B.  How could filing an extension help you save money? -  For those who are not enrolled in a repayment plan yet or have recently discovered that they are on the wrong plan and need to make a change, filing an extension on your 2022 taxes could be beneficial as well. Typically, when you enroll in a repayment plan or make changes to your repayment plan, they capitalize your interest. This means that any of your unpaid interest gets added to your principal... meaning you will be earning interest on your interest, which is less than ideal. After July 2023, they are doing away with that, and you will be free to switch repayment plans without having to capitalize any interest. For folks who made less in 2021 and get on a different plan, we are often advising them to file an extension on their taxes this year to report that lower income.

Query: Debridement Codes
 
I do some part time work in a wound care center. I frequently use CPT 97597 for coding. My biller is telling me not to bill CPT 97597 because it reimburses around $25 or less in a wound care center. However, the facility likes and even encourages me to use CPT 97597. 
 
If I do a selective debridement, can I bill CPT 99213 or CPT 99212 instead of CPT 97597?  I know that I should not bill CPT 99213-Modifier -25 if there isn’t a separate complaint. Am I required to bill 97597? Is this a scenario that I should be considering CPT 11042?

Response: The short answer is no, you cannot bill for an evaluation and management when doing a debridement. Proper coding means billing for what you do despite what an entity, in this case the wound center or your biller, says you should do. It is critical that one bills for what they do based on the service provided and as accurately as one can. Using an E/M in place of a more accurate service is actually fraudulent billing, so don’t go there.
 
As far as using CPT 11042, it again depends on what service you are providing. Is this a level 2 ulcer that merits using that code for what you debrided? Again, coding needs to describe what you did according to the code descriptor, not what you want it to say.

How far we have come as a profession sometimes shows up on what our colleagues do NOT say: Last December, I unfortunately fell while getting out of bed (wearing pajamas with sewn-in slippery slippers) and believed I might have suffered a spinal compression fracture. I had it evaluated, and my suspicions were verified with both a scan and a spinal x-ray. The doctor at the ER (who had a father who was a podiatrist) recommended a kyphoplasty to me which would use methyl methacrylate to stabilize the fractured vertebra. I have a lot of allergies and was not certain I wanted to try this out. Besides, my pain, although disabling, was not severe enough (3.5 out of 10).
 
He recommended an orthopedist and this being NV, it is difficult to get to see that doctor quickly. But I managed to find another one and got a quick appointment, to my surprise. We had a very nice talk and I told him I was a podiatrist, although...
